Superior oblique palsy as the initial manifestation of anti-contactin-1 IgG4 autoimmune nodopathy: A case report
We report a case of anti-contactin-1 neuropathy initially manifesting as isolated superior oblique palsy, aiming to broaden the clinical spectrum of the disease. A 68-year-old male with well-controlled diabetes, hypertension, and hyperlipidemia developed acute binocular vertical diplopia, progressing over two months to include distal paresthesia, sensory ataxia, ageusia, and dysarthria. Corneal fibrosis: From in vitro models to current and upcoming drug and gene medicines
Adv Drug Deliv Rev. 2024 Apr 18:115317. doi: 10.1016/j.addr.2024.115317. Online ahead of print.ABSTRACTFibrotic diseases are characterised by myofibroblast differentiation, uncontrolled pathological extracellular matrix accumulation, tissue contraction, scar formation and, ultimately tissue / organ dysfunction. The cornea, the transparent tissue located on the anterior chamber of the eye, is extremely susceptible to fibrotic diseases, which cause loss of corneal transparency and are often associated with blindness. Although topical corticosteroids and antimetabolites are extensively used in the management of corneal fibros...

Corticosteroid injection versus immobilisation for the treatment of De Quervain's tenosynovitis: a systematic review and meta-analysis
CONCLUSION: Corticosteroid injection is more effective than immobilization for De Quervain's tenosynovitis, and combining the two treatments provides additional benefit. We recommend corticosteroid injection in first line treatment and immobilization as adjuvant therapy. Further research is required regarding optimal corticosteroid and local anesthetic formulations.PMID:38642740 | DOI:10.1016/j.hansur.2024.101694 (Source: Hand Surgery)

Does lidocaine reduce pain intensity during corticosteroid injection? A double-blind randomized controlled equivalence trial
J Hand Surg Eur Vol. 2024 Apr 20:17531934241245036. doi: 10.1177/17531934241245036. Online ahead of print.ABSTRACTOf the strategies considered to limit the discomfort of corticosteroid injection, one is to inject without lidocaine to reduce the total volume and avoid acidity. In a Bayesian trial, adults receiving corticosteroid injections were randomized to receive 0.5 mL of triamcinolone with or without 0.5 mL of lidocaine. Serial analysis was performed until a 95% probability of presence or absence of a 1.0-point difference in pain intensity on the 0-10 Numerical Rating Scale was reached. Injections with lidocaine were a...

Chronic Oral Corticoteroid Use and 10-Year Incidence of Major Complications Following Total Knee Arthroplasty
Oral corticosteroids are the primary treatment for several autoimmune conditions. The risk of long-term implant, bone health, and infectious-related complications in patients taking chronic oral corticosteroids before total knee arthroplasty (TKA) is unknown. We compared the 10-year cumulative incidence of revision, periprosthetic joint infection (PJI), fragility fracture (FF), and periprosthetic fracture (PPF) following TKA in patients who had and did not have preoperative chronic oral corticosteroid use. Adrenocortical suppression in children with nephrotic syndrome treated with corticosteroids
ConclusionsA significant proportion of children with nephrotic syndrome were detected with adrenal insufficiency on ACTH stimulation test. A cumulative steroid intake of  >  0.22 mg/kg/day on an alternate day basis emerged as a risk factor for predicting adrenocortical suppression.Graphical abstractA higher resolution version of the Graphical abstract is available asSupplementary information (Source: Pediatric Nephrology)

Anterior and posterior ischemic optic neuropathy in a child with focal segmental glomerulosclerosis on hemodialysis
ConclusionsThe exact cause of our patient ’s simultaneous bilateral anterior and posterior ION, confirmed via MRI and fundoscopic examination, is unclear; however, is likely secondary to a combination of fluctuating blood pressure, anemia, anephric status, and hemodialysis. This highlights the need for close blood pressure monitoring, man agement of anemia, and more diligent ophthalmologic screening in pediatric patients on chronic hemodialysis. Dexamethasone sodium phosphate loaded nanoparticles for prevention of nitrogen mustard induced corneal injury
Exp Eye Res. 2024 Apr 17:109902. doi: 10.1016/j.exer.2024.109902. Online ahead of print.ABSTRACTNitrogen mustard (NM) is a potent vesicating chemical warfare agent that is primarily absorbed through skin, inhalation, or ocular surface. Ocular exposure of NM can cause acute to chronic keratopathy which can eventually lead to blindness. There is a current lack of effective countermeasures against ocular exposure of NM despite their imperative need. Herein, we aim to explore the sustained effect of Dexamethasone sodium phosphate (DSP)-loaded polymeric nanoparticles (PLGA-DSP-NP) following a single subconjunctival injection in...
