Pulmonology: What You May Have Missed in 2023
Ann Intern Med. 2024 Apr 16. doi: 10.7326/M24-0613. Online ahead of print.ABSTRACTThe field of pulmonology saw significant advances in 2023. The publications highlighted in this article address advances and changes in practice related to asthma, chronic obstructive pulmonary disease (COPD), interstitial lung disease, pleural disorders, and sleep-disordered breathing. One article reviews data examining the efficacy of vaccination against respiratory syncytial virus, a respiratory viral illness that has had devastating effects globally. Four studies evaluate the role of various therapies in COPD, including dupilumab, ensifen...

Use of inhaled corticosteroids and the risk of hospitalisation for pneumonia in children with asthma: a nationwide cohort study
Conclusions In this nationwide cohort study, we found no evidence of an association between ICS use and the risk of hospitalisation for pneumonia among children with asthma, as compared with no use. (Source: Thorax)

Impact of sex on severe asthma: a cross-sectional retrospective analysis of UK primary and specialist care
Conclusions Females had increased symptoms and were more likely to be obese despite higher FEV1% predicted and lower type 2 biomarkers with consistent and clinically important differences across both datasets. (Source: Thorax)

Antibiotics Not Associated with Shorter Duration or Reduced Severity of Acute Lower Respiratory Tract Infection
ConclusionsIn this large prospective study in the US primary and urgent care setting, antibiotics had no measurable impact on the severity or duration of cough due to acute LRTI. Patients had unrealistic expectations regarding the duration of LRTI and the effect of antibiotics which should be the target of antibiotic stewardship efforts. Higher paracetamol levels are associated with elevated glucocorticoid concentrations in hair: findings from a large cohort of young adults
This study addresses this gap by investigating associations between steroid hormones (cortisol, cortisone, and testosterone) and paracetamol concentrations measured in human hair, indexing a timeframe of approximately three months. The data came from a large community sample of young adults (N = 1002). Hair data were assayed using liquid chromatography-tandem mass spectrometry. Multiple regression models tested associations between paracetamol and steroid hormones, while adjusting for a wide range of potential confounders, such as sex, stressful live events, psychoactive substance use, hair colour, and body mass index. Alm...

Advancements in De Quervain Tenosynovitis Management: A Comprehensive Network Meta-Analysis
CONCLUSIONS: Corticosteroid injection with a short duration of immobilization remains the primary and effective treatment for De Quervain tenosynovitis. Extracorporeal shockwave therapy can be considered a secondary option. Alternative treatment modalities, such as isolated therapeutic injection, should be approached with caution because they did not show substantial benefits over placebo.TYPE OF STUDY/LEVEL OF EVIDENCE: Therapeutic I.PMID:38613563 | DOI:10.1016/j.jhsa.2024.03.003 (Source: Hand Surgery)

Cardiovascular risk and inflammation in a population with autoimmune diseases: a narrative review
Front Immunol. 2024 Mar 28;15:1380372. doi: 10.3389/fimmu.2024.1380372. eCollection 2024.ABSTRACTJuvenile Systemic Connective Tissue Diseases (JSCTD) are a heterogeneous group of chronic autoimmune diseases, associated with dyslipidemia and increased cardiovascular risk are related. Studies from the last 10 years, from 2013 to 2022, on lipid profiles in JSCTD were collected. Different studies on lipid profiles in children affected by JSCTD were selected, because the aim is to analyze the cardiovascular risk and the possibility of atherosclerosis in these patients in whom, sometimes, corticosteroid therapies and immunosuppr...

Antitubercular drug-induced lichen planus: A case study with a mini literature review
CONCLUSION: Lichenoid drug eruptions are characterized by type IV hypersensitivity reactions, and rechallenge is required to ensure safer treatment since the risk of disseminated and multi-drug-resistant tuberculosis increases with the cessation of antitubercular therapy.PMID:38605770 | PMC:PMC11006073 | DOI:10.4103/jfmpc.jfmpc_499_23 (Source: Primary Care)

Idiopathic intracranial hypertension as the initial presenting manifestation of systemic lupus erythematosus in a child
We report a 12-year-old girl presented with chronic holocranial headache and occasional episodes of projectile vomiting for the last 6 months and then developed blurring of vision for the last month. She fulfilled the criteria for IIH. Subsequent evaluation revealed a diagnosis of SLE. The occurrence of IIH in SLE is not coincidental and is reported in 1%-5.4% of patients with SLE. Though corticosteroids have not been widely used in IIH, underlying SLE warranted administering corticosteroids with subsequent complete resolution of IIH. Pediatricians, neurologists, intensivists, and ophthalmologists should consider SLE as a ...
