A systematic review of randomised controlled trials on topical nasal steroids
CONCLUSIONS: BDP nasal spray is one of the most frequently prescribed INC for rhinitis and rhinosinusitis. Treatment with BDP resulted in significant and clinically meaningful improvements in nasal symptoms associated with AR and CRS. BDP is well tolerated, and the safety profile is similar to that of placebo in most patients. These results, in conjunction with the significant benefit reported in subjects with CRS and AR, provide convincing evidence of the overall effectiveness of BDP for the treatment of the full spectrum of sinonasal disease.PMID:38651550 | PMC:PMC11042551 | DOI:10.14639/0392-100X-N2745 (Source: Acta Oto...

Acute Myopic Shift after a Single Dose of Acetazolamide: A Case Report and Review of the Literature
We report the case of a 32-year-old male who presented with an acute myopic shift as a result of uveal effusion following a single administration of 250 mg acetazolamide. The drug was discontinued and following cycloplegia and topical steroid therapy, we observed progressive deepening of the anterior chamber, reopening of the iridocorneal angle, and complete resolution of the myopic shift after 5 days. A literature review since 1956 identified 23 cases, including ours, which developed a myopic shift after a median time of 24 h (3 - 24) following a median dose of 500 mg (125 - 1000) acetazolamide, with about a third complic...

Safety and effectiveness of isavuconazole in real-life non-neutropenic patients
There is a notable change in the profile of patients with invasive fungal infections (IFI): there has been a progressive rise of such infections in non-neutropenic population (i.e., COVID-19, influenza, biologic agents, corticosteroids, cancer, chronic obstructive pulmonary disorder [COPD] or cirrhosis) [1 –3]. However, information concerning the management of non-neutropenic patients with IFI remains limited. Caution should be exercised before these patients are treated like those who are neutropenic, given a considerable variability in pathogenesis and clinical manifestations of IFI [4]. Antiphospholipid Patients Admitted in the Intensive Care Unit: What Must The Rheumatologist Know?
AbstractPurpose of the reviewAntiphospholipid syndrome (APS) is a rare systemic autoimmune disorder that can escalate into a 'thrombotic storm' called the catastrophic antiphospholipid syndrome (CAPS), frequently requiring ICU admission for multiple organ failure. This review aims to offer insight and recent evidence on critically-ill APS patients.Recent findingsThe CAPS classification criteria define this condition as the involvement of at least three organs/systems/tissues within less than a week, caused by small vessel thrombosis, in patients with elevated antiphospholipid antibodies levels. These criteria do not encomp...

Lumbar Spinal Stenosis: Diagnosis and Management
Am Fam Physician. 2024 Apr;109(4):350-359.ABSTRACTLumbar spinal stenosis is a clinical syndrome that affects more than 200,000 people in the United States annually. It is a common cause of chronic insidious low back pain, especially in older patient populations (mean age = 64 years). Lumbar spinal stenosis is a degenerative condition of the spine leading to narrowing in the spaces around the neurovascular bundles and the classic symptom of low back pain that radiates to the buttocks and lower extremities bilaterally. It is typically a progressive waxing and waning process that may deteriorate over years. The pain is typica...

A case of disseminated nocardiosis with orbital apex involvement and endophthalmitis
We report a case of disseminated nocardiosis with orbital apex involvement and endophthalmitis in an immunocompromised patient. The patient presented with respiratory sepsis, and later developed complete ptosis and ophthalmoplegia in the left eye. This was on the background of treatment with high-dose prednisolone. Magnetic resonance imaging showed enhancement of the entire clivus, extending into the left orbital apex and cavernous sinus. The patient was initially treated empirically for CNS tuberculosis. Bronchoscopic cultures returned positive for Nocardia farcinica, and the patient was treated with trimethoprim/sulfamet...

Sarcoid-Like Reactions to Immune Checkpoint Inhibitors: Incidence, Treatment Course, and Impact on Cancer Progression and Survival
CONCLUSIONS: SLRs are a known but understudied complication associated with ICI therapy. Multisystem SLR patients were more symptomatic and required ICI discontinuation and immune suppression. Larger studies are needed to fully evaluate the impact of SLR on cancer outcomes.PMID:38648910 | DOI:10.1016/j.rmed.2024.107640 (Source: Respiratory Care)
