Comparison of High- and Low-Dose Rivaroxaban Regimens in Elderly East Asian Patients With Atrial Fibrillation
CONCLUSION: This study revealed the safety and effectiveness of either dose regimen of rivaroxaban in an Asian population for stroke prevention of AF. Considerable numbers of patients are receiving LDRR therapy in real-world practice in Asia. Both regimens were safe and effective for these patients.TRIAL REGISTRATION: ClinicalTrials.gov Identifier: NCT04096547.PMID:38442717 | PMC:PMC10911935 | DOI:10.3346/jkms.2024.39.e72 (Source: J Korean Med Sci)

Identification of a murder caused by brodifacoum poisoning based on clinical examinations and LC-MS/MS results
AbstractBrodifacoum exerts its antagonistic effect against the metabolism of vitamin K, an essential component in the synthesis of blood coagulation factors. This effect ultimately hinders the blood ’s capacity to clot effectively, rendering it a commonly employed rodenticide. Instances of lethal poisonings are exceedingly rare owing to expeditious medical intervention and treatment. Within this report, we present a case of brodifacoum-induced homicide, wherein the patient exhibited distinct clinical examinations and symptoms. Moreover, the patient’s blood sample exhibited a noteworthy brodifacoum concentration of 0.68...

Matrix gla protein and the long-term incidence and progression of coronary artery and aortic calcification in the multi-ethnic study of atherosclerosis
Matrix Gla protein (MGP) is an inhibitor of calcification that requires carboxylation by vitamin K for activity. The inactive form of MGP, dephosphorylated-uncarboxylated matrix Gla protein (dp-ucMGP), has been associated with increased calcification. However, it is not known whether there is a longitudinal relationship between dephosphorylated-uncarboxylated matrix Gla protein levels and coronary and aortic calcification in large population cohorts. Comparative effectiveness of factor Xa non-vitamin K antagonist oral anticoagulants versus phenprocoumon in patients with non-valvular atrial fibrillation
Non-vitamin K antagonist oral anticoagulants (NOACs) have largely supplanted vitamin K antagonists (VKAs) for oral anticoagulation in non-valvular atrial fibrillation (NVAF). However, data on the real-world effectiveness of NOACs vs. phenprocoumon, a VKA widely used in Germany, are limited. The RELOADED study aimed to compare effectiveness of factor Xa NOACs and phenprocoumon in NVAF in clinical practice. (Source: International Journal of Cardiology)

Vitamin K-dependent carboxylation in β-cells and diabetes
Trends Endocrinol Metab. 2024 Feb 29:S1043-2760(24)00035-3. doi: 10.1016/j.tem.2024.02.006. Online ahead of print.ABSTRACTVitamin K is an essential micronutrient and a cofactor for the enzyme γ-glutamyl carboxylase, which adds a carboxyl group to specific glutamic acid residues in proteins transiting through the secretory pathway. Higher vitamin K intake has been linked to a reduced incidence of type 2 diabetes (T2D) in humans. Preclinical work suggests that this effect depends on the γ-carboxylation of specific proteins in β-cells, including endoplasmic reticulum Gla protein (ERGP), implicated in the control of intrace...

Efficacy and safety of rivaroxaban versus apixaban for venous thromboembolism: A systematic review and meta-analysis of observational studies
ConclusionsApixaban is associated with a significantly lower risk of major bleeding compared to rivaroxaban for treatment of VTE. Given the limitations of the existing evidence, further interventional studies comparing the two drugs are needed. (Source: Journal of Thrombosis and Thrombolysis)

How the Hemostasis Laboratory can Help Clinicians to Manage Patients on Oral Anticoagulants
This article aims to overview how the hemostasis laboratory can help clinicians to manage patients on oral anticoagulants. Special interest is devoted to the international normalized ratio, used to manage patients on VKAs and to the measurement of DOAC concentrations, for which the role of the laboratory is still not very well defined and most interferences of DOACs with some of the most common hemostatic parameters are not widely appreciated. Venous thromboembolism - was is new in the revised AWMF guideline?
Dtsch Med Wochenschr. 2024 Mar;149(6):274-282. doi: 10.1055/a-2128-0005. Epub 2024 Feb 27.ABSTRACTFor the diagnosis of a lower-extremity deep vein thrombosis (LEDVT), venous duplex ultrasound is the method of first choice. If a qualified ultrasonography is not timely available, D-dimer testing, and limited ultrasound protocols (point-of-care ultrasound, POCUS) can contribute to therapeutic decision-making when clinical probability is low. A DOAC-based treatment regimen is preferable to a vitamin K antagonist for both acute therapy and secondary prophylaxis of venous thromboembolism (VTE). Treatment with DOACs is unproblema...
