Inconclusive Findings in Non-Vitamin K Antagonist Oral Anticoagulants Study
To the Editor We read with interest the nationwide population-based cohort study with meta-analysis by Tsai et al. The study findings showed that patients who received non-vitamin K antagonist oral anticoagulants (NOACs) before a stroke, compared with those receiving warfarin and those without oral anticoagulant treatment (non-OAC), did not experience a higher risk of intracranial hemorrhage (ICH), major bleeding events, or mortality when treated with intravenous alteplase for acute ischemic stroke. The presence of left atrial or left atrial appendage thrombosis (LAT) is a contraindication to catheter ablation (CA) and cardioversion (CV), procedures commonly used to restore sinus rhythm in symptomatic atrial fibrillation (AF) patients [1,2]. Transesophageal echocardiography (TEE) is useful to identify LAT and avoid the risk of thrombosis systemic or cerebral embolization due to restoration of sinus rhythm leading to either stroke or asymptomatic cerebral emboli [1,2].
