What the pediatric nurse needs to know about the Impella cardiac assist device
Perfusion. 2024 Apr;39(1_suppl):95S-106S. doi: 10.1177/02676591241237761.ABSTRACTBackground: Cardiogenic shock in children still carries a high mortality risk despite advances in medical therapy. The use of temporary mechanical circulatory supports is an accepted strategy to bridge patients with acute heart failure to recovery, decision, transplantation or destination therapy. These devices are however limited in children and extracorporeal membrane oxygenation (ECMO) remains the most commonly used device. Veno-arterial ECMO may provide adequate oxygen delivery, but it does not significantly unload the left ventricle, and ...

Pediatric Diffuse Leptomeningeal Glioneuronal Tumors: Diagnosis, Follow-up and Treatment Options
CONCLUSION: Further work with larger cohorts is required to determine a common algorithm for DLGNT treatment and follow-up. This analysis may keep this entity in mind in patients with pediatric communicating hydrocephalus and may present insight into diagnosis, follow-up, and treatment options.PMID:38650559 | DOI:10.5137/1019-5149.JTN.43742-23.2 (Source: Turkish Neurosurgery)

Percutaneous Ventriculoatrial Shunting as a Salvage Method in the Pediatric Hydrocephalus Patients with Peritoneal Problems
CONCLUSION: Percutaneous ventriculoatrial shunting under the guidance of intraoperative fluoroscopy and ultrasound is a safe, effective, and easy alternative in patients with peritoneal complications and a history of multiple operations.PMID:38650557 | DOI:10.5137/1019-5149.JTN.43472-23.2 (Source: Turkish Neurosurgery)

Characterization of Ceftriaxone-Resistant < em > Haemophilus influenzae < /em > Among Korean Children
CONCLUSION: The emergence of ceftriaxone-resistant H. influenzae with ceftriaxone MIC values of up to 0.75 mg/L was observed even in children in South Korea, with most associated with S385T and L389F substitutions. The N526K mutation alone does not significantly impact ceftriaxone resistance. Further large-scale studies are essential to investigate changes in antibiotic resistance patterns and factors influencing antibiotic resistance in H. influenzae isolated from pediatric patients in Korea.PMID:38651222 | DOI:10.3346/jkms.2024.39.e136 (Source: Journal of Korean Medical Science)

Delta-S-Cys-Albumin as a Marker of Pediatric Biospecimen Integrity
We report application of a previously developed assay in adults-the ΔS-Cys-Albumin assay, which facilitates estimation of plasma and serum exposure to thawed conditions-to a population of pediatric EDTA plasma samples from patients aged 3-18 years to determine the assay's applicability, estimate its reference range for pediatric samples, and assess the impact of pre-centrifugation delay at 0°C. In addition, the effect of plasma thawed-state exposure to a range of times at 23°C, 4°C, and -20°C on ΔS-Cys-Albumin was evaluated. Using 98 precollected and processed pediatric EDTA plasma specimens, no difference was found ...

Analyzing the Quality of Life in Individuals with Fragile X Syndrome in Relation to Sleep and Mental Health
This study included 119 individuals with Fragile X Syndrome who were given different cognitive examinations by a neuropsychologist or by parent-proxy questionnaires. This study focused on the Pediatric Quality of Life Inventory (PedsQoL), the Anxiety, Depression and Mood Scale (ADAMS), the Children's Sleep Habits Questionnaire (CSHQ), but did include other cognitive tests (Vineland Adaptive Behaviour Scales, Nonverbal IQ, Autism Diagnostic Observation Schedule). We identified significant associations between decreases in emotional, social and school domains of PedsQoL and the ADAMS subtests of Generalized Anxiety, Manic/Hy...

Adalimumab Dose Reduction and Withdrawal in Stable Non-Infectious Pediatric Uveitis: An Open-Label, Prospective, Pilot Study
CONCLUSIONS: In PNIU children who reached remission for 6 months, ADA dose reduction and withdrawal were associated with a high risk of inflammation recurrence. Timely adjustment of ADA to the last effective dosage frequency can regain control of the inflammation. Detection of ADA serum levels in patients with recurrence may help find the appropriate interval of ADA use.PMID:38652891 | DOI:10.1080/09273948.2024.2343084 (Source: Ocular Immunology and Inflammation)

The Use of the RETeval Portable Electroretinography Device for Low-Cost Screening: A Mini-Review
Klin Monbl Augenheilkd. 2024 Apr;241(4):533-537. doi: 10.1055/a-2237-3814. Epub 2024 Apr 23.ABSTRACTElectroretinography (ERG) provides crucial insights into retinal function and the integrity of the visual pathways. However, ERG assessments classically require a complicated technical background with costly equipment. In addition, the placement of corneal or conjunctival electrodes is not always tolerated by the patients, which restricts the measurement for pediatric evaluations. In this short review, we give an overview of the use of the RETeval portable ERG device (LKC Technologies, Inc., Gaithersburg, MD, USA), a modern ...

Positive Airway Pressure and Metabolic Markers in Children With Obstructive Sleep Apnea
CONCLUSIONS: Although evidence on effects of PAP on metabolic markers in children with OSA is encouraging, available literature is limited. Longitudinal studies are still required to further assess the long-term influence of PAP on metabolic and inflammatory markers, particularly in children with obesity.PMID:38653555 | DOI:10.4187/respcare.11679 (Source: Respiratory Care)

Pediatric and Young Adult Trauma Recidivism
CONCLUSIONS: Pediatric/young adult trauma recidivism is associated with specific characteristics including male, African American race, penetrating trauma, and uninsured status. Recidivists are primarily presenting from a zip code with low socioeconomic status. It is critical to develop targeted interventions to help this population in trauma prevention.PMID:38653577 | DOI:10.1177/00031348241248698 (Source: The American Surgeon)

Metabolic dysfunction-associated steatotic liver disease: Evolution of the final terminology
Eur J Intern Med. 2024 Apr 22:S0953-6205(24)00173-0. doi: 10.1016/j.ejim.2024.04.013. Online ahead of print.ABSTRACTThe medical term nonalcoholic fatty liver disease (NAFLD) was coined in 1986 for a condition that has since become the most prevalent liver disorder worldwide. In the last 3 years, the global professional community launched 2 consecutive efforts to purge NAFLD from the medical dictionary and recommended new terms based on disease pathophysiology rather than distinction from similar conditions featuring liver steatosis. A consensus by renowned clinical scholars primarily residing in the Asian-Pacific region in...

Perforated Appendicitis in Children: Management, Microbiology, and Antibiotic Stewardship
Paediatr Drugs. 2024 Apr 24. doi: 10.1007/s40272-024-00630-0. Online ahead of print.ABSTRACTAlthough appendicitis has been described for more than 300 years, its optimal management remains a topic of active investigation. Acute appendicitis is the most common cause of peritonitis in children, and rates of perforated appendicitis are much higher in children than in adults. Increased risk for perforated appendicitis in children is related to a delay in diagnosis due to age, size, access to care, and more. Surgical options include immediate appendectomy versus nonoperative management with intravenous antibiotics ± a drainage...
