Perforated Appendicitis in Children: Management, Microbiology, and Antibiotic Stewardship
Paediatr Drugs. 2024 Apr 24. doi: 10.1007/s40272-024-00630-0. Online ahead of print.ABSTRACTAlthough appendicitis has been described for more than 300 years, its optimal management remains a topic of active investigation. Acute appendicitis is the most common cause of peritonitis in children, and rates of perforated appendicitis are much higher in children than in adults. Increased risk for perforated appendicitis in children is related to a delay in diagnosis due to age, size, access to care, and more. Surgical options include immediate appendectomy versus nonoperative management with intravenous antibiotics ± a drainage...

Characterization of Ceftriaxone-Resistant < em > Haemophilus influenzae < /em > Among Korean Children
CONCLUSION: The emergence of ceftriaxone-resistant H. influenzae with ceftriaxone MIC values of up to 0.75 mg/L was observed even in children in South Korea, with most associated with S385T and L389F substitutions. The N526K mutation alone does not significantly impact ceftriaxone resistance. Further large-scale studies are essential to investigate changes in antibiotic resistance patterns and factors influencing antibiotic resistance in H. influenzae isolated from pediatric patients in Korea.PMID:38651222 | DOI:10.3346/jkms.2024.39.e136 (Source: J Korean Med Sci)

Translation and validation of traditional Chinese version of the pediatric eating assessment Tool-10
CONCLUSIONS: The Traditional Chinese version of the Pedi-EAT-10 has fair reliability and validity and can be quickly and easily completed by caregivers. The translated Ped-EAT-10 can be used as a first-line tool for assessing the need for further referral and instrumental examination.PMID:38653676 | DOI:10.1016/j.jfma.2024.04.010 (Source: J Formos Med Assoc)

New LC-MS/MS reference data for estradiol show mini-puberty in both sexes and typical pre-pubertal and pubertal patterns
CONCLUSIONS: Variability of RIs for E2 between studies illustrates importance of laboratory-specific RIs despite using a liquid chromatographic tandem mass spectrometry (LC-MS/MS) reference method. In boys, the striking E2 peak during mini-puberty as well as high pubertal E2 without phenotypic estrogenization in regular male puberty indicate that the role of E2 in children, and especially in boys requires better functional understanding.PMID:38652605 | DOI:10.1093/ejendo/lvae046 (Source: European Journal of Endocrinology)

Anesthetic challenges in patients with multicompartmental lymphatic failure after Fontan palliation undergoing transcatheter thoracic duct decompression
Paediatr Anaesth. 2024 Apr 23. doi: 10.1111/pan.14891. Online ahead of print.ABSTRACTLymphatic flow abnormalities are central to the development of protein losing enteropathy, plastic bronchitis, ascites and pleural effusions in patients palliated to the Fontan circulation. These complications can occur in isolation or multicompartmental (two or more). The treatment of multicompartmental lymphatic failure aims at improving thoracic duct drainage. Re-routing the innominate vein to the pulmonary venous atrium decompresses the thoracic duct, as atrial pressure is lower than systemic venous pressure in Fontan circulation. Tran...

Meeting speech, language and communication needs: a whole-systems, population-based approach
This article sets out the case for taking a population-health based approach to anticipating need and building whole systems around children in their functional contexts, whether home, early years setting, school or further education. The demand on services providing speech and lang uage therapy continues to increase year on year. Using a population-health approach to predicting areas of higher anticipated SLCN and establishing robust collaborative approaches to improving the context for children and young people vulnerable to SLCN, there is the potential to address a significa nt number needs within everyday contexts. (So...
