Value-based comparison of ambulatory children with respiratory diseases in an emergency department and a walk-in clinic: a retrospective cohort study in Quebec, Canada
Conclusions The incidence of return visits and cost of care were significantly higher in the ED, while antibiotic use for URTI was more frequent in the walk-in clinic. These data may help determine which setting offers the highest value to ambulatory children with acute respiratory conditions. (Source: BMJ Open)

Enterovirus-Related Parainfectious Acute Disseminated Encephalomyelitis Presenting with Dorsal Midbrain Syndrome: A Pediatric Case Report
An 8-year-old male patient presented to the pediatric clinic with complaints of cough, cold, lethargy, fever, restricted eye movements, and altered sensorium for one day. A detailed clinical examination revealed bilateral ophthalmoparesis, with left occulomotor nerve involvement leading to limitations of vertical gaze. The patient was referred to the ophthalmology clinic for assessment. Upon evaluation, it was observed that the child exhibited upward gaze palsy and convergence-retraction nystagmus as can be seen in Figure 1. (Source: Pediatric Neurology)

Investigating associations between non-adherence to guideline-recommended treatment of pediatric seizures and adverse outcomes: a Canadian feasibility study
We found evidence of substantial non-adherence to guideline-recommended treatment of pediatric seizures. Medical records do not contain sufficient information to comprehensively investigate this issue. A multi-centre prospective study is the most feasible option to examine the association between non-adherence to guideline-recommended treatment and patient outcomes. (Source: Pediatric Neurology)

A Prognostic Methylation-Driven Two-Gene Signature in Medulloblastoma
In this study, we used six datasets totaling 1679 samples including RNA gene expression and DNA methylation data from primary MB as well as control samples from healthy cerebellum. We identified methylation-driven genes (MDGs) in MB, genes whose expression is correlated with their methylation. We employed LASSO regression, incorporating the MDGs as a parameter to develop the prognostic model. Through this approach, we deri ved a two-gene signature (GS-2) of candidate prognostic biomarkers for MB (CEMIP andNCBP3). Using a risk score model, we confirmed the GS-2 impact on overall survival (OS) with Kaplan-Meier analysis. We ...

Cancers, Vol. 16, Pages 1667: Identifying Candidate Gene Drivers Associated with Relapse in Pediatric T-Cell Acute Lymphoblastic Leukemia Using a Gene Co-Expression Network Approach
Jean-Francois Peyron Pediatric T-cell Acute Lymphoblastic Leukemia (T-ALL) relapses are still associated with a dismal outcome, justifying the search for new therapeutic targets and relapse biomarkers. Using single-cell RNA sequencing (scRNAseq) data from three paired samples of pediatric T-ALL at diagnosis and relapse, we first conducted a high-dimensional weighted gene co-expression network analysis (hdWGCNA). This analysis highlighted several gene co-expression networks (GCNs) and identified relapse-associated hub genes, which are considered potential driver genes. Shared relapse-expressed genes were found to be re...

Effectiveness of atopic dermatitis patient education programs – a systematic review and meta-analysis
In this study, we conducted a systematic review of the literature to determine the effectiveness of online programming in AD education compared to in-person interventions. A comprehensive search was conducted in accordance with the Cochrane Handbook for Systematic Reviews of Interventions 2019. Studies were retrieved based on articles published up to 04 April 2023. Adherence to the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) Statement guided the reportage process for this systematic review and meta-analysis. The primary outcome of our meta-analysis was the effect of various educational modali...

The need for surgery in pediatric patients with inflammatory bowel disease treated with biologicals
ConclusionDespite initiation of biological treatment, the risk of intestinal surgery remains high in pediatric IBD patients and often the need for surgery emerges after the transition to adult IBD clinics. (Source: International Journal of Colorectal Disease)

Advances in Immune Tolerance Induction in Enzyme Replacement Therapy
AbstractInborn errors of metabolism (IEMs) are a group of genetic diseases that occur due to the either deficiency of an enzyme involved in a metabolic/biochemical pathway or other disturbances in the metabolic pathway including transport protein or activator protein deficiencies, cofactor deficiencies, organelle biogenesis, maturation or trafficking problems. These disorders are collectively significant due to their substantial impact on both the well-being and survival of affected individuals. In the quest for effective treatments, enzyme replacement therapy (ERT) has emerged as a viable strategy for patients with many o...

Efficacy of non-instrumental Endodontic treatment in primary teeth: a systematic review of clinical randomized trials
ConclusionNo difference between the NIET technique and the conventional endodontic therapy in primary molars requiring pulpectomy could be confirmed. Results of the present review need to be interpreted with caution since the quality of evidence according to the GRADE was considered as moderate to very low. Therefore, additional clinical trials on the NIET technique are recommended. (Source: Systematic Reviews)

A systematic review on the biochemical threshold of mitochondrial genetic variants [REVIEW]
Mitochondrial DNA (mtDNA) variants cause a range of diseases from severe pediatric syndromes to aging-related conditions. The percentage of mtDNA copies carrying a pathogenic variant, variant allele frequency (VAF), must reach a threshold before a biochemical defect occurs, termed the biochemical threshold. Whether the often-cited biochemical threshold of >60% VAF is similar across mtDNA variants and cell types is unclear. In our systematic review, we sought to identify the biochemical threshold of mtDNA variants in relation to VAF by human tissue/cell type. We used controlled vocabulary terms to identify articles measu...
