Predicting Amputation Rates in Acute Limb Ischemia: Is the Neutrophil-Lymphocyte Ratio a Reliable Indicator?
Conclusions Based on our findings, the main factor affecting the need for amputation in ALI patients was the symptom-to-door time. Biochemical and hematological parameters had no effect on amputation in ALI.PMID:38686104 | PMC:PMC11057397 | DOI:10.7759/cureus.59253 (Source: Atherosclerosis)

Letermovir primary cytomegalovirus prophylaxis in allogeneic hematopoietic cell transplant recipients: could infection and disease no longer be a significant problem?
Conclusions: LET primary prophylaxis proved effective in preventing cs-CMVi and CMVd and reducing hospitalizations in allo-HCT adults. Blips can occur during prophylaxis and do not require LET discontinuation. Keywords: letermovir; cytomegalovirus; hematopoietic cell transplantation. Beyond silence: evolving ultrasound strategies in the battle against cardiovascular thrombotic challenges
AbstractCardiovascular thrombotic events have long been a perplexing factor in clinical settings, influencing patient prognoses significantly. Ultrasound-mediated acoustic therapy, an innovative thrombolytic treatment method known for its high efficiency, non-invasiveness, safety, and convenience, has demonstrated promising potential for clinical applications and has gradually become a focal point in cardiovascular thrombotic disease research. The current challenge lies in the technical complexities of preparing ultrasound-responsive carriers with thrombus-targeting capabilities and high thrombolytic efficiency. Additional...

Limb Ischemic Necrosis Secondary to Microvascular Thrombosis: A Brief Historical Review
Semin Thromb Hemost DOI: 10.1055/s-0044-1786356Ischemic limb injury can be broadly classified into arterial (absent pulses) and venous/microvascular (detectable pulses); the latter can be divided into two overlapping disorders—venous limb gangrene (VLG) and symmetrical peripheral gangrene (SPG). Both VLG and SPG feature predominant acral (distal) extremity ischemic necrosis, although in some instances, concomitant nonacral ischemia/skin necrosis occurs. Historically, for coagulopathic disorders with prominent nonacral ischemic necrosis, clinician-scientists implicated depletion of natural anticoagulants, especially invol...

Impact of Early Cytomegalovirus Reactivation after Allogeneic Hematopoietic Stem Cell Transplantation on Relapse in Patients with Myelodysplastic Syndrome: A Nationwide Retrospective Study from Adult Myelodysplastic Syndrome Working Group of the JSTCT
Cytomegalovirus (CMV) reactivation is a well-recognized major complication associated with morbidity and mortality in patients undergoing allogeneic hematopoietic stem cell transplantation (HSCT).1-5 Although different prophylactic and pre-emptive approaches have been developed to manage CMV reactivation in allogeneic HSCT recipients, several studies still emphasized the negative association between CMV reactivation and various post-transplant outcomes, including non-relapse mortality caused by infectious complications or graft-versus-host disease (GVHD) and overall survival. Chimeric Antigen Receptor T-Cell Access in Patients with Relapsed/Refractory Large B-Cell Lymphoma: Association of Access with Social Determinants of Health and Travel Time to Treatment Centers
Large B-cell lymphoma (LBCL) is the most common type of non-Hodgkin lymphoma, with a US incidence rate of 5.5 per 100,000 men and women per year and a 5-year overall survival rate of 64.7%.1 Although two-thirds of patients with LBCL are cured with standard chemoimmunotherapy regimens, 30%-40% of patients will either have progression of disease or relapse after initial response.2,3 (Source: Biology of Blood and Marrow Transplantation)

Lymphocytes in patients with chronic active epstein-barr virus infection exhibited elevated pd-1/pd-l1 expression and a prevailing th2 immune response.
Conclusions: To summarize, EBV might evade the immune system by inducing PD-1/PD-L1 expression in peripheral T and NK cells, and promoting Th2 immune response dominance. Additionally, it may establish long-term persistence by infecting memory T cells in CAEBV. (Source: Mediterranean Journal of Hematology and Infectious Diseases)
