Effects of perioperative low-dose naloxone on the immune system in patients undergoing laparoscopic-assisted total gastrectomy: a randomized controlled trial
Low immune function after laparoscopic total gastrectomy puts patients at risk of infection-related complications. Low-dose naloxone (LDN) can improve the prognosis of patients suffering from chronic inflammat... (Source: BMC Anesthesiology)

Improving HCC Surveillance with abbreviated MRI: A Call to Integrate and Innovate?
Hepatocellular carcinoma (HCC) remains a formidable challenge in oncology, largely due to its asymptomatic nature in early stages and poor prognosis once advanced. Despite limited evidence, current recommendations for the surveillance of HCC are based on semiannual liver ultrasound (with or without serum alpha fetoprotein) in patients with high risk of liver cancer, i.e., mostly cirrhosis and subgroups with chronic hepatitis B infection (1-3). However, the sensitivity of this strategy is suboptimal for the detection of HCC at the earliest possible stage. The use of ultrasound has been challenged by several metanalyses (4,5...

Cirrhosis Decompensation: Clinical and Prognostic Analysis from a New Perspective
The research article "New Clinical and Prognostic Features of Decompensated Cirrhosis Modes" by Marta Tonon et al. profoundly motivated me. By prospectively monitoring 617 outpatient cirrhosis patients, this study identified distinctions between acute decompensation (AD) and non-acute decompensation (NAD) in terms of prognosis and clinical manifestations [1]. Nonetheless, several facets of this research remain to be investigated in greater depth. (Source: Journal of Hepatology)

Need for risk stratification in treatment of patients with autoimmune hepatitis
We read with keen interest the study by Plagiannakos CG and colleagues1, which explored the association between sustained elevated aminotransferase levels and long-term outcomes in patients with autoimmune hepatitis (AIH)1. This large multicenter cohort study provides robust evidence for good treatment response and clinical event-free survival in AIH patients. While previous research suggested that lower serum immunoglobin G (IgG) levels could predict the biochemical response of AIH patients2, Plagiannakos CG et al. (Source: Journal of Hepatology)

Predictors for Lymph Node Metastasis and Survival of Patients with T1b Esophageal Adenocarcinoma Treated with Surgery and Endoscopic Therapy: An Analysis of Surveillance, Epidemiology, and End Results Database
Limited data exists regarding the long-term outcomes of endoscopic therapy (ET) with or without chemoradiation therapy (CRT) for T1b esophageal adenocarcinoma (EAC). Our aim was to identify the risk factors for lymph node metastasis (LNM) in T1b EAC and assess how the chosen treatment modality affects overall survival (OS) and cancer-specific survival (CSS). Surgical Considerations in Portal Hypertension
This review provides an in-depth exploration of portal hypertension (PH) and its implications in various surgical procedures. The prevalence of clinically significant PH is 50% to 60% in compensated cirrhosis and 100% in decompensated cirrhosis. The feasibility and safety of hepatic and nonhepatic surgical procedures in patients with PH has been shown. Adequate preoperative risk assessment and optimization of PH are integral parts of patient assessment. The occurrence of adverse outcomes after surgery has decreased over time in this specific population, due to the development of techniques and improved perioperative multid...

Noninvasive Assessment of Portal Hypertension
The progressive use of noninvasive tests (NITs) has changed the way hepatologists diagnose and manage patients with chronic liver disease, mainly because of their easiness to use and the ability to be repeated during follow-up. Liver stiffness measurement is the NIT with more scientific evidence. NITs have demonstrated to be useful to detect not only liver fibrosis but also the presence of clinically significant portal hypertension. Moreover, current evidence supports they can also be useful to evaluate the prognosis of patients with chronic liver disease. (Source: Clinics in Liver Disease)

Evidence and Choice: The BCLC Vision for Tailoring Clinical Decision-Making
We would like to begin by thanking Trevisani et al [1] for the considerable time and effort devoted to evaluating the strengths and limitations of the BCLC approach [2]. They propose a ‘hierarchical approach’, aiming to bridge the gap between evidence-based data and clinical practices in areas lacking comprehensive data. In contrast, the BCLC approach [2] advocates for a patient-centric model, rather than the treatment prioritization suggested by Trevisani et al. [1]. It's imp ortant to note that the BCLC 2022 [2] update's specifically in its ‘Clinical Decision Making’ section, allows for personalized management. (...

UMobileNetV2 model for semantic segmentation of gastrointestinal tract in MRI scans
by Neha Sharma, Sheifali Gupta, Deepali Gupta, Punit Gupta, Sapna Juneja, Asadullah Shah, Asadullah Shaikh Gastrointestinal (GI) cancer is leading general tumour in the Gastrointestinal tract, which is fourth significant reason of tumour death in men and women. The common cure for GI cancer is radiation treatment, which contains directing a high-energy X-ray beam onto the tumor while avoiding healthy o rgans. To provide high dosages of X-rays, a system needs for accurately segmenting the GI tract organs. The study presents a UMobileNetV2 model for semantic segmentation of small and large intestine and stomach in MRI image...

Endoscopy for Altered Anatomy in Inflammatory Bowel Disease
A significant proportion of patients with Crohn's disease or ulcerative colitis will ultimately necessitate surgical interventions, despite advances in diagnosis, medical therapy, and endoscopic interventions. These surgeries lead to substantial alterations in bowel anatomy. The present review aims to emphasize the key role of conducting a comprehensive endoscopic evaluation of both anatomic landmarks and endoscopic features for accurate clinical interpretation, thereby impacting decisions regarding medical or endoscopic interventions. Endoscopy in Special Situations in Inflammatory Bowel Disease
Endoscopy for a hospitalized patient with acute severe ulcerative colitis should be limited to flexible sigmoidoscopy. If, during pregnancy, inflammatory bowel disease (IBD) is active or if a new diagnosis of IBD is suspected, endoscopy may be needed to guide medical decision-making. The determination to proceed with endoscopy during pregnancy is nuanced and requires the ability to discern appropriate indications, knowledge regarding safety of bowel preparation and anesthesia. Pediatric patients with IBD represent a unique subset of IBD with distinctive diagnostic, therapeutic, and monitoring considerations. (Source: Gastr...
