We would like to begin by thanking Trevisani et al [1] for the considerable time and effort devoted to evaluating the strengths and limitations of the BCLC approach [2]. They propose a ‘hierarchical approach’, aiming to bridge the gap between evidence-based data and clinical practices in areas lacking comprehensive data. In contrast, the BCLC approach [2] advocates for a patient-centric model, rather than the treatment prioritization suggested by Trevisani et al. [1]. It's imp ortant to note that the BCLC 2022 [2] update's specifically in its ‘Clinical Decision Making’ section, allows for personalized management.
