Chronic pain enhances excitability of corticotropin-releasing factor-expressing neurons in the oval part of the bed nucleus of the stria terminalis
We previously reported that enhanced corticotropin-releasing factor (CRF) signaling in the bed nucleus of the stria terminalis (BNST) caused the aversive responses during acute pain and suppressed the brain re... (Source: Molecular Brain)

Decision Making of the In-season Athlete with Anterior Shoulder Instability
In-season management of anterior shoulder instability in athletes is a complex problem. Athletes often wish to play through their current season, though recurrent instability rates are high, particularly in contact sports. Athletes are generally considered safe to return to play when they are relatively pain-free, and their strength and range of motion match the uninjured extremity. If an athlete is unable to progress toward recovering strength and range of motion, surgical management is an option, though this is often a season-ending decision. Complete Cam Resection Results in Best Outcomes After Hip Arthroscopy for Femoroacetabular Impingement
An aspherical hip can mechanically damage the joint and lead to chondrolabral injury, functional pain, and potentially arthritis. Hip arthroscopy to address femoroacetabular impingement (FAI) and make the hip spherical, with treatment of concomitant labral pathology, can result in excellent outcomes in patients with this pathology. The goal is complete cam resection. Cam pathology comes in many shapes, locations, and sizes — from smaller, anterolaterally located lesions to larger, pistol group lesions that extend more superiorly and lateral. Hip Arthroscopy in Patients with Spine Pain: Outcomes Vary Compared to Patients with Isolated Hip Pathology, but Many Hip-Spine Patients Do Just as Well
Low back pain present concurrently with hip impingement or labral pathology is frustrating for both patient and surgeon. It is difficult to distinguish true hip pathology from secondary low back symptoms, even with the use of diagnostic injections, cross-sectional imaging, electromyography, and/or nerve conduction studies. In addition, even properly indicated hip arthroscopy can exacerbate sciatic nerve dysfunction, a known complication from traction, and altered gait in the early postoperative period can aggravate sacroiliac and lumbar pathology. Do plasma-rich protein injections have a role in orthopaedics? A systematic review and meta-analysis of the randomized control trials
We evaluated the effectiveness of plasma-rich protein injections (PRP) in terms of pain, patient-reported outcomes and function in degenerative joint disease and tendinopathy. The search strategies were ran in Ovid Medline and Ovid Embase in October 2023 by a clinical librarian. Results were limited to the last 10 years. The search terms used were ‘platelet rich plasma’ and ‘orthopaedics’. After screening and the eligibility process this resulted in 18 randomized control trials (RCTs). We analysed 18 level 1 RCTs in regard to gluteal tendinopathy, hip osteoarthritis, patella tendinopathy, knee osteoarthritis, Achi...

Factors Associated With Pain Catastrophizing in Patients With Chronic Neuropathic Pain: A Cross-Sectional Study
Chronic neuropathic pain caused by lesions or diseases of the somatosensory nervous system, which lasts for more than 3 months continuously or intermittently (Tao et al., 2021), is the most prevalent pain disorder group. It includes symptoms such as hyperalgesia, spontaneous pain, and paresthesia. Chronic neuropathic pain is characterized by its persistence, severity, and lack of controllability (Tao et al., 2021). Although drug therapy is the first-line treatment, its level of efficacy is far from optimal, and adverse drug reactions are common (Noori et al., 2019). (Source: Pain Management Nursing)

The effects of neck exercise in comparison to passive or no intervention on quantitative sensory testing measurements in adults with chronic neck pain: A systematic review
by Daniel Osborne, Ferozkhan Jadhakhan, Deborah Falla BackgroundPrevious systematic reviews have identified the benefits of exercise for chronic neck pain on subjective reports of pain, but not with objective measures such as quantitative sensory testing (QST). A systematic review was conducted to identify the effects of neck specific exercise on QST measures in adults with chronic neck pain to synthesise existing literature and provide clinical recommendations. MethodsThe study protocol was registered prospectively with PROSPERO (PROSPERO CRD42021297383). For both randomised and non-randomised trials, the following databa...

Comparison of the efficacy and complications of tolterodine and α-adrenergic receptor blockers in improving ureteral stent-related symptoms: A systematic review and meta-analysis
ConclusionThis meta-analysis suggests that tolterodine is superior to α-adrenergic receptor blockers in improving physical pain symptoms after ureteral stent placement, while α-adrenergic receptor blockers are more effective than tolterodine in enhancing work performance. Additionally, the incidence of dry mouth is higher with the use of tolterodine compared to α-a drenergic receptor blockers. However, higher-quality randomized controlled trials are needed to further investigate this issue. (Source: PLoS One)

Spontaneous Abdominal Wall Endometriosis: Minimally Invasive Surgery for Excision and Repair
A 35 year-old gravida 2 para 1 female with a pacemaker in situ due to congenital cardiac heart disease and biopsy-proven stage IV endometriosis presented with chronic pain and a “bulge” in the right lower quadrant. Per the operative report of her initial surgery, an adherent, firm nodule was palpable in the right lower anterior abdominal wall and left in situ because etiology and extent of the disease was unclear. Physical exam revealed a palpable 4cm abdominal wall mas s. Her pacemaker was MRI-incompatible, so abdominal ultrasound and CT abdomen/pelvis were obtained but revealed inconclusive findings. Meta-Analysis: Touch Tied to Improved Mental, Physical Health
People who received touch interventions, such as massage or gentle stroking for adults and skin-to-skin contact for infants, experienced moderate reductions in pain, depression, and anxiety, according to a meta-analysis of more than 130 studies that involved about 13  000 participants. The results suggest that “touch interventions can be systematically employed across the population to preserve and improve our health,” the researchers wrote in Nature Human Behavior. Cancer Pain Management in Patients with Opioid Use Disorder
AbstractPurpose of reviewUpdated cancer pain management recommendations considering the rise of opioid use disorder (OUD) and opioid overdose deaths. Few studies examine cancer pain management specific to patients with OUD. This narrative review aims to review recent literature on the topic, synthesize current recommendations, and formulate practical guidance.Recent findingsWe explored Google Scholar, PubMed, and institutional databases using keywords: cancer pain, pain management, and OUD for key literature. Themes found were multimodal cancer pain management, opioid selection, opioid and OUD stigma, and barriers to medic...
