Interesting clawback from Humana
Got a "refund request" from our good friends at Humana today. Clawing back two 99204s and one 99214 because "diagnosis code(s) are inappropriate". The only code all three charts have in common is G894 chronic pain syndrome. None of them use it as a primary or even secondary diagnosis, but as a 3rd or 5th diagnosis. Clinical privileges question
I left my job recently (private practice set up) and my clinical privileges at a hospital (no direct clinical role there-- just mainly for insurance purposes) was automatically suspended due to my malpractice insurance ending for that practice. New TPI restrictions?
well CMS is at it again! making our jobs harder with every new LCD update. this time trigger point injections are coming under fire. you know, because a $50 reimbursement is really breaking their bank. maximum time allowed between 2nd MBB and 1st RFA?
After two successful Dx blocks, is there a maximum time allowed within which RFA has to be done? I have a patient in this situation, and we are holding off on RFA for a number of reasons, but wanting to know how long we can delay the RFA, without having to potential start over with MBB again, or do the RFA only to have it denied for payment. New thoracic MBB anatomy
Based on new locations of articular branches from this dissection by Maus, looks like the articular branches are not always coming off the medial branches, or if they are, are very close to DRG. Doesn't seem like targeting the MB at the sling would do much--would only hit lateral branch. TP targeting seems more medial than previous models.
