One good tern …
Yesterday, I spotted a Sandwich Tern, Thalasseus sandvicensis patrolling the open river lock at Brownshill Staunch on the Great River Ouse. It flew back and forth over a stretch of about 100 metres for 20 minutes or so before heading off downstream. The river cuts through the RSPB Ouse Fen nature reserve (I’d walked in from the Over end rather than Needingworth). I’ve seen several Common Terns and even a Black Tern on this reserve. But, a Sandwich seemed unusual so I posted my spot to the local patch’s birding WhatsApp group when I first saw it. There was a flurry of interest and one member, Richard Thoma...

Elite access vs. public scrutiny: Medication disparities exposed
A recent New York Times opinion piece detailed a lack of available pain medications. While the DEA claims that it is not purposefully restricting legitimate medication availability, even the names of its own operations belie this statement. On Halloween 2023, the DEA launched Operation “Bottleneck,” serving immediate suspension orders to six large pharmaceutical supply companies. Need High Awareness of Symptoms of Heart Attack
High degree of awareness of heart attack symptoms in the community will go a long way in seeking early treatment. For life threatening diseases like heart attack, early recognition of symptoms and prompt reporting to the emergency department is highly desirable. But a study from United States showed that 47% were not able to recognize some heart attack symptoms while 6% were not able to recognize any heart attack symptoms. The main heart attack symptoms assessed by the study were: Chest pain or discomfort Shortness of breath Pain or discomfort in arms or shoulders Feeling weak, lightheaded, or faint Jaw, neck, or back pai...

The current STEMI paradigm: Because STEMI criteria are not met, let's wait until the myocardium is dead!!
This was sent to me by an inpatient nurse who reads this blog but wants to remain anonymous. An inpatient rapid response was called for a patient with hypotension.  The patient was originally admitted for pneumonia and had been transferred out of the ICU a day prior. He had a history of HFrEF, HTN, and AML. " When I arrived his blood pressure was 70s/40s and he was pale and profusely diaphoretic. " " He spoke Spanish but we did deduce that he had 7/10 chest pain radiating to the back. " " We couldn ’t initially get a hold of the primary physician but our hospital allows the rapid response nur...

Supported self-management – what are we managing, actually?
One of the words that has been in my vocabulary for decades is ‘management.’ No, I don’t mean the proliferative middle management layer infesting healthcare for about as long as I’ve been working in health. I mean ‘pain management’ or ‘supported self-management.’ As ever, what pops up for me is about what, exactly, is being managed? Is it pain? Well – not exactly. After all, pain does what it does, and if we focus on pain reduction alone just look where that gets us (Ballantyne & Sullivan, 2015; Cutler & Glaeser, 2021). And quickly, lest anyone think I’...

ECG in LMCA Stenosis
ECG in a person with persistent anginal pain for the past several hours showing significant ST segment depression anterolateral leads along with sinus tachycardia. ST segment elevation is noted in aVR. Such a pattern is consistent with significant left main coronary artery stenosis. Clinical evaluation and X-Ray chest showed features of pulmonary edema. Angiography done after initial stabilization showed severe stenosis of distal left main coronary artery. In addition, there were multiple lesions in all three vessels, making a standard indication for an urgent coronary artery bypass grafting. A similar ECG pattern can also...

Differentiating Between Cardiac and Non-Cardiac Chest Pain
It is not always possible to be certain about the origin of chest pain just by its characteristics as the variation between individuals is quite a bit. A medical opinion should be sought in case of any significant chest pain so that important ailment is not missed. There can be a lot of overlap between symptoms due to heart disease and disease of other nearby organs. Still some general observations are possible regarding chest pain originating from the heart. The typical pain of cardiac origin is a central chest pain which occurs on walking or other forms of exercise, known as effort angina. This pain is caused by insuffic...

Suspicions of medical child abuse delayed my child ’ s cancer diagnosis
It was November 2014. I was leaving the parking structure of my local children’s hospital when I realized, “They don’t believe us.” I didn’t know it at the time, but it would take seven more months to find the cause of my eight-year-old daughter’s unremitting abdominal pain. An ultrasound ultimately revealed a malignant mass on Read more… Suspicions of medical child abuse delayed my child’s cancer diagnosis originally appeared in KevinMD.com. A 29 year old male with chest pain, ST Elevation, and very elevated troponin T
By Magnus NossenThis ECG is from a young man with no risk factors for CAD, he presented with chest pain. How would you assess this ECG? How confident are you in your assessment? What is your next step? Note: lead format is CabreraI was sent this ECG in real time. The patient is a young adult male with chest pain. The chest pain was described as pressure like and radiation to both arms and the jaw. Symptoms were on and off. The pain was worse in the night and better when moving. The patient sought medical attention when the pain recurred for a second straight night accompanied by arm numbness as well as radiating pain. The ...

Complexity and chronic pain*
*Persistent didn’t alliterate so well! I’ve been pondering what makes persistent pain so complicated? What is it about this problem that means clinicians use terms like ‘heart-sink’, or ‘problematic’, or ‘difficult’ when they talk about people living with pain? While nociception and all the associated neurobiological processes associated with pain are undoubtedly complex (and poorly understood), I don’t think this is what people mean when they describe chronic pain is complex. After all, there are loads of body systems and disease processes that are complex. I...
