Patient acceptable symptom state and treatment failure threshold values for work productivity and activity Impairment and EQ-5D-5L in osteoarthritis
ConclusionPASS and TF threshold values for WPAI and EQ-5D-5L might be useful for meaningful interpretation of these measures among people with OA. The observed baseline dependency of estimated thresholds limits their generalizability and values should be applied with great caution in other settings/populations. News at a glance: Plants ’ tree of life, conserving U.S. federal lands, and entertaining invertebrate sentience
CONSERVATION Interior Department boosts restoration The largest manager of public lands in the United States last week announced it will manage them to promote ecological stewardship, while continuing to allow extractive commercial uses such as mining, oil drilling, and cattle grazing. The Bureau of Land Management (BLM) oversees nearly 1 million square kilometers of federal land, about 10% of the country’s landmass, mostly in Western states. Under a rule issued on 18 April, BLM will now assess the ecological condition of all hectares it manages , not just grazing land. The agency will a...

Surgical treatment of painful neuroma in amputated and non-amputated patients: does the level of neurotomy affect clinical outcomes?
CONCLUSION: DN seemed to give better results in amputees but there were confusing factors related to associated lesions. In other situations, the non-inferiority of PN was demonstrated. PN could be of interest for treating neuromas of superficial sensory nerves, for avoiding direct revision of a well-fitted stump and in conjunction with TMR.PMID:38662211 | DOI:10.1007/s00068-024-02529-4 (Source: Hand Surgery)

Transdermal Opioids and the Quality of Life of the Cancer Patient: A Systematic Literature Review
CONCLUSIONS: Transdermal therapy was suggested to have several advantages over oral opioid therapy in enhancing cancer patients' QoL. These benefits span various dimensions, including pain management, physical functioning, mental health, vitality, overall patient improvement, anger/aversion, strength/activity, general QoL, cognitive and emotional functions, fatigue, and insomnia.PMID:38659244 | DOI:10.1177/10600280241247363 (Source: The Annals of Pharmacotherapy)

Bile Acid Diarrhea
Korean J Gastroenterol. 2024 Apr 25;83(4):133-142. doi: 10.4166/kjg.2023.119.ABSTRACTDiarrhea is a very common gastrointestinal symptom, and the presence of higher concentrations of bile acid in the colon leads to bile acid diarrhea (BAD). In BAD patients, a portion of bile from the small intestine that is normally controlled by enterohepatic circulation is present at a high concentration in the lumen of the large intestine, resulting in increased motility and secretion of the large intestine. The prevalence of BAD is estimated to be 1-2% of the general population, and it comprises one-third of the instances of diarrhea-pr...

Oncolytic adenovirus in treating malignant ascites: a phase II trial and longitudinal single-cell study
Mol Ther. 2024 Apr 23:S1525-0016(24)00252-1. doi: 10.1016/j.ymthe.2024.04.029. Online ahead of print.ABSTRACTMalignant ascites is a common complication resulting from the peritoneal spread of malignancies, and currently lacks effective treatments. We conducted a phase II trial (NCT04771676) to investigate the efficacy and safety of oncolytic adenovirus H101 and virotherapy-induced immune response in 25 patients with malignant ascites. Oncolytic virotherapy achieved an increased median time to repeat paracentesis of 45 days (95% confidence interval 16.5-73.5 days), compared to the preset control value of 13 days. Therapy wa...

The Qi of Body, Mind, and Spirit: A Contextual Perspective on the Elusive Nature of Qi
CONCLUSIONS: Qi-nature can vary among gross, subtle, and causal levels. These levels can coexist and overlap during treatments of patients. This tripartite explanation may represent the Oriental pictograms better and be consistent with the philosophical root of Chinese Medicine-the Tao.PMID:38659729 | PMC:PMC11036150 | DOI:10.1089/acu.2023.0071 (Source: Acupuncture in Medicine)

Treating Erythromelalgia with Interosseous Membrane Stimulation: An Autonomic Basis for the Condition and Its Treatment
CONCLUSIONS: Interosseous membrane stimulation is a safe, simple, and effective treatment for erythromelalgia, which is notoriously refractory to treatment. This patient's response to treatment might have been a result of localized derangement of her autonomic nervous system. It is possible that manipulation of the autonomic milieu of an extremity is a significant factor in the mechanism of action of interosseous membrane stimulation.PMID:38659724 | PMC:PMC11036148 | DOI:10.1089/acu.2023.0098 (Source: Acupuncture in Medicine)

The Association Between Unintended Pregnancy and Perinatal Outcomes in Low ‐Risk Pregnancies: A Retrospective Registry Study in the Netherlands
This study used data (N = 9803) from a Dutch nationally representative registry of people with low-risk pregnancies receiving primary midwife-led care in the Netherlands between 2012 and 2020. Using logistic (mediation) regression analyses adjusted for potential confounders we investigated associations between unintended pregnancy and neonatal outcomes (low Apgar score, small for gestational age, and prematurity), parental morbidity (hypertension and gestational diabetes mellitus), and obstetric interventions (induction of labor, pain medication, assisted vaginal birth, and cesarean birth) and whether delayed initiation of...

Roles of Neuronal Protein Kinase C ε on Endoplasmic Reticulum Stress and Autophagic Formation in Diabetic Neuropathy
AbstractIn chronic diabetic neuropathy (DN), the cellular mechanisms of neuropathic pain remain unclear. Protein kinase C epsilon (PKC ε) is an intracellular signaling molecule that mediates chronic pain. This paper addresses the long-term upregulated PKCε in DN associated with endoplasmic reticulum (ER) stress and autophagic formation and correlates to chronic neuropathic pain. We found that thermal hyperalgesia and mechanical a llodynia course development were associated with PKCε upregulation after DN but not skin denervation. Pathologically, PKCε upregulation was associated with the expression of inositol-requiring...

Ganglionic Local Opioid Analgesia at the Superior Cervical Ganglion: MRI-Verified Solution Spread
ConclusionFor the transoral blocking technique, a strict laterodorsal needle direction is recommended to prevent possible block failures. A total volume of 2  ml injected into the parapharyngeal space and its “medial compartment” is recommended. Higher volumes may lead to uncontrolled distribution patterns.Trial registrationClinicaltrials.gov identifier NCT05257655; date of registration 2022-02-25; patient enrollment date from 2023-01-09 to 2023-08-31.
