Safety and Efficacy of Surgical Implantation of Intrathecal Drug Delivery Pumps in Patients With Cancer With Refractory Pain
CONCLUSION: Our study demonstrates that ITP implantation for the treatment of cancer-related pain is a safe and effective method of pain palliation with a low complication rate. Future prospective studies are required to determine the optimal timing of ITP implantation.PMID:38700319 | DOI:10.1227/neu.0000000000002978 (Source: Cancer Control)

Distribution of Nerve Fibers in Abdominal Wall Endometriosis and Their Clinical Significance
CONCLUSION: This study demonstrated an increased immunoreactive nerve fiber density located in AWE lesions compared to normal abdominal wall scars. Further high-quality studies are needed to investigate the mechanisms responsible for pain in women with endometriosis.PMID:38699067 | PMC:PMC11063460 | DOI:10.2147/JPR.S453148 (Source: Cancer Control)

Conversion ratios: Why is it so challenging to construct opioid conversion tables?
J Opioid Manag. 2024 Mar-Apr;20(2):169-179. doi: 10.5055/jom.0853.ABSTRACTStandardizing opioid management is challenging due to the absence of a ceiling dose, the unknown ideal therapeutic plasma level, and the lack of an clear relationship between dose and therapeutic response. Opioid rotation or conversion, which is switching from one opioid, route of administration, or both, to another, to improve therapeutic response and reduce toxicities, occurs in 20-40 percent of patients treated with opioids. Opioid conversion is often needed when there are adverse effects, toxicities, or inability to tolerate a certain opioid form...

Opioid use and disposal at 2 weeks post-surgery: Brief communication regarding excess opioids and disposal habits
CONCLUSION: Two-thirds of the patients reported having excess opioids, highlighting the issue of an overabundance of unused prescription opioids in America. Utilization of opioid-free pain management strategies and drug disposal kits should be explored to reduce the number of unused and improperly disposed opioids.PMID:38700391 | DOI:10.5055/jom.0854 (Source: Journal of Opioid Management)

The Gluteus Deep Investing Fascia Compartment Block: A Novel Technique for Posterior Femoral Cutaneous Nerve Block
CONCLUSION: The GDIF block appears to be a promising technique for anesthetic management, alone or as part of the CLEFT block. Further research with a larger patient population is necessary to validate these findings.PMID:38699287 | PMC:PMC11064292 | DOI:10.2147/LRA.S455702 (Source: Local and Regional Anesthesia)

Enhanced Recovery After Surgery (ERAS) Spine Pathways and the Role of Perioperative Checklists
Adv Tech Stand Neurosurg. 2024;49:73-94. doi: 10.1007/978-3-031-42398-7_5.ABSTRACTEnhanced recovery after surgery (ERAS) proposes a multimodal, evidence-based approach to perioperative care. ERAS pathways have been shown to help reduce complications, hospital length of stay (LOS), 30-day readmission rates, pain scores, and ultimately surgical costs, while improving patient satisfaction scores and outcomes in multiple surgical subspecialties [1-6]. Numerous specialties have implemented ERAS programs across the globe, providing a foundation for spine surgeons to begin the process themselves. Over the last few years, a signif...

Decision Making of the In-season Athlete with Anterior Shoulder Instability
In-season management of anterior shoulder instability in athletes is a complex problem. Athletes often wish to play through their current season, though recurrent instability rates are high, particularly in contact sports. Athletes are generally considered safe to return to play when they are relatively pain-free, and their strength and range of motion match the uninjured extremity. If an athlete is unable to progress toward recovering strength and range of motion, surgical management is an option, though this is often a season-ending decision. Complete Cam Resection Results in Best Outcomes After Hip Arthroscopy for Femoroacetabular Impingement
An aspherical hip can mechanically damage the joint and lead to chondrolabral injury, functional pain, and potentially arthritis. Hip arthroscopy to address femoroacetabular impingement (FAI) and make the hip spherical, with treatment of concomitant labral pathology, can result in excellent outcomes in patients with this pathology. The goal is complete cam resection. Cam pathology comes in many shapes, locations, and sizes — from smaller, anterolaterally located lesions to larger, pistol group lesions that extend more superiorly and lateral. Hip Arthroscopy in Patients with Spine Pain: Outcomes Vary Compared to Patients with Isolated Hip Pathology, but Many Hip-Spine Patients Do Just as Well
Low back pain present concurrently with hip impingement or labral pathology is frustrating for both patient and surgeon. It is difficult to distinguish true hip pathology from secondary low back symptoms, even with the use of diagnostic injections, cross-sectional imaging, electromyography, and/or nerve conduction studies. In addition, even properly indicated hip arthroscopy can exacerbate sciatic nerve dysfunction, a known complication from traction, and altered gait in the early postoperative period can aggravate sacroiliac and lumbar pathology.
