You only ECHO Twice
We report on an 80-year-old woman with a cerebral stroke. Successful thrombectomy revealed a histopathologically confirmed papillary fibroelastoma as the embolic source. Although it was not detectable on initial echocardiography, the tumour was discovered by transesophageal echocardiography. This case emphasises the importance of considering primary cardiac tumours as potential sources of emboli in ischaemic stroke. Keywords.PMID:38655734 (Source: Praxis)

The Association of CHADS-P2A2RC Risk Score With Clinical Outcomes in Patients Taking P2Y12 Inhibitor Monotherapy After 3 Months of Dual Antiplatelet Therapy Following Percutaneous Coronary Intervention
CONCLUSIONS: The CHADS-P2A2RC risk score is valuable in discriminating high-ischemic-risk patients. Even in such patients with a high risk of ischemic events, P2Y12 inhibitor monotherapy was associated with a lower incidence of bleeding without increased risk of ischemic events compared with prolonged DAPT.TRIAL REGISTRATION: ClinicalTrials.gov Identifier: NCT02079194.PMID:38654565 | PMC:PMC11040264 | DOI:10.4070/kcj.2023.0268 (Source: Korean Circulation Journal)

Regarding the predictor of perioperative stroke/TIA in carotid endarterectomy patients
We read Monaco's recent study1 with great interest. The authors assessed the relationship between thrombin receptor activator peptide 6 (TRAP), adenosine-5 ′-diphosphate (ADP), and arachidonic acid (ASPI) tests measured using multiple electrode aggregometry and the rate of stroke/transient ischemic attack (TIA) in patients undergoing carotid endarterectomy (CEA). They used a logistic LASSO regression model to identify factors independently associated with stroke/TIA. They concluded that an ADP>72 U and the need for a shunt during CEA were the only 2 variables independently associated with perioperative stroke/TIA. (Sourc...
