An economic evaluation and incremental analysis of the cost effectiveness of three universal childhood varicella vaccination strategies for Ireland
CONCLUSION: A one-dose strategy was highly cost effective from the payer perspective, driven by a reduction in hospitalisations. Two-dose strategies were cost saving from the societal perspective. These results should be considered alongside other factors such as acceptability of a new vaccine within the overall childhood immunisation schedule, programme objectives and budget impact.PMID:38609807 | DOI:10.1016/j.vaccine.2024.04.027 (Source: Herpes)

Prehistoric or current primitive cranial operations
Prog Brain Res. 2024;284:19-29. doi: 10.1016/bs.pbr.2024.02.003. Epub 2024 Mar 26.ABSTRACTHumankind demonstrates boundless curiosity, mostly expressed through the activities of a small number of individuals, whose achievements affect all members of society. The extent and distribution of pre-historic trepanation and trepanation in contemporary unsophisticated societies are reviewed. In the great majority of cases the intention of trepanation has been therapeutic, even if the understanding of underlying pathophysiology is not the same as that which scientific societies now accept. This review demonstrates variation in surgi...

Cancer survivorship: understanding the patients ’ journey and perspectives on post-treatment needs
ConclusionThe study highlighted the lack of information and support needed by patients living with and beyond cancer. The study also highlighted the need for a personalised exercise programme designed to target the individual patient symptoms that would be ideal for the mitigation of long term symptoms and in improving QOL. Antipsychotic prescribing: national findings of children and adolescents attending mental health services in Ireland
We described the frequency of antipsychotic medication, medication type, target condition, target symptom and medication doses. We used multivariable logistic regression, adjusted with available co-variates to assess the association of being presc ribed an antipsychotic medication. Twelve percentage (n = 437) were prescribed an antipsychotic and 16–17-years (n = 211, 48.3%) was the most common age category. The commonest reason for prescribing an antipsychotic was target symptoms (i.e., off-label use) (n = 329; 75.%) and of thes e symptoms, agitation (n = 77/329; 25%) and irritability (56/239; 25%) were t...

The confidence and competence of primary school staff to administer an adrenaline auto-injector
Conclusion: Improvements in readiness to administer AAIs can be achieved through the application of more effective approaches  to teaching clinical skills, changes to school policies and practices, and consideration of the design of AAIs in order to make their operation safer and simpler. It is important that teachers have the confidence and competence to safely administer an AAI.What is Known:• Poor ability in adrenaline auto-injector use seen across population groups—healthcare professionals, patients, carers, and school staff• Training in the use of adrenaline auto-injectors has positive impact on competencyWhat ...

Comparison of sequential and mixture injections of opioids and hyperbaric bupivacaine for subarachnoid block for lower segment caesarean section: a randomised controlled study
ConclusionSequential injection of intrathecal opioids and hyperbaric bupivacaine resulted in greater early haemodynamic instability and slightly better postoperative analgesia without any difference in block height or patient satisfaction.Clinical trial registrationNCT04403724. Midwife continuity of care models versus other models of care for childbearing women
CONCLUSIONS: Women receiving midwife continuity of care models were less likely to experience a caesarean section and instrumental birth, and may be less likely to experience episiotomy. They were more likely to experience spontaneous vaginal birth and report a positive experience. The certainty of some findings varies due to possible risks of bias, inconsistencies, and imprecision of some estimates. Future research should focus on the impact on women with social risk factors, and those at higher risk of complications, and implementation and scaling up of midwife continuity of care models, with emphasis on low- and middle-...

Using oral bioaccessibility measurements to refine risk assessment of potentially toxic elements in topsoils across an urban area
This study considers how PTE bioaccessibility in soils varies spatially across urban areas and refines human health risk assessment for these PTEs using site specific oral bioaccessibility results to present the first regional assessment of risk that incorporates bioaccessibility testing. A total of 103 urban soil samples were selected for UBM testing. Results showed low bioaccessible fraction (BAF) for the PTEs from geogenic sources: Cr (0.45-5.9%), Ni (1.1-46.3%) and V (2.2-23.9%). Higher BAF values were registered for PTEs from anthropogenic sources: As (8.0-86.9%), Cu (3.4-67.8%), Pb (9.1-106.2%) and Zn (2.4-77.5%). Gr...

Healthcare practitioners' experiences and perspectives of music in perinatal care in Ireland: An exploratory survey
This study offers insights into how healthcare practitioners viewed and applied music in perinatal practice. The findings indicate high interest and positive experiences in using music as a therapeutic tool in perinatal care which highlights the need for more evidence and guidance.PMID:38599130 | DOI:10.1016/j.midw.2024.103987 (Source: Midwifery)
