Impacts of gait freeze on quality of life in Parkinson ’s disease, from the perspectives of patients and their carers
ConclusionsThis study shows that FoG is a significant detriment to the QoL of PwP, from the perspectives of patients and carers. This method of assessing FoG and QoL using online questionnaires has potential to enhance the reach and flexibility of this type of research. These findings will inform future studies on larger cohorts and highlight unmet clinical needs in PwP. (Source: Irish Journal of Medical Science)

Exploring access to community neurorehabilitation for people with progressive neurological conditions: a qualitative study
CONCLUSIONS: This study adds depth to our understanding of the complexity, and the roles and needs of people with PNCs and HCPs, in accessing community neurorehabilitation. Further research is needed to determine how best to empower people to access rehabilitation.PMID:38632940 | DOI:10.1080/09638288.2024.2338198 (Source: Disability and Rehabilitation)

Quality improvement project assessing the feasibility of using canister weight to estimate remaining doses in a salbutamol pressurised metered-dose inhaler
Pressurised metered-dose inhalers (pMDIs) containing salbutamol are an essential medicine for children with acute asthma. Salamol chlorofluorocarbon-free inhaler (‘Salamol’; IVAX Pharmaceuticals, Waterford, Ireland) is the most widely prescribed salbutamol pMDI in England (figure 1), costing the National Health Service £7.2 million in the 2022–23 financial year.1 In common with the other salbutamol pMDIs it lacks an integrated dose counter (IDC). Without an IDC, most children and families cannot accurately identify when a pMDI is empty.2 Thus, families may continue to use a pMDI that contains no act...

Accuracy of point-of-care testing for the diagnosis of respiratory pathogens in a paediatric intensive care setting
Respiratory tract infections contribute significantly to paediatric intensive care unit (PICU) admissions. Early diagnosis supports treatment, cohorting and antimicrobial stewardship. Rapid point-of-care (POC) multiplex PCR techniques show promise in improving antimicrobial and investigation stewardship across healthcare settings.1 However, debate exists regarding their role and accuracy.2 3 We sought to evaluate the feasibility and accuracy of respiratory pathogen POC testing in our PICU. All patients admitted to the regional Northern Ireland PICU undergo nasal/nasopharyngeal swab respiratory pathogen POC testing. The Bio...

Current management of neonatal abstinence syndrome: a survey of practice in the UK and Ireland
Conclusion and relevance Compared with the previous survey, improvements in evidence-based practices were noted, highlighting the benefits of this type of research. Nonetheless, significant variation still exists in some aspects of the management of NAS. Post-discharge follow-up varies widely, with particular deficits in ophthalmology follow-up. Predicting community acquired bloodstream infection in infants using full blood count parameters and C-reactive protein; a machine learning study
Conclusion:  We identified several models that can predict positive blood cultures in infants with suspected BSI aged 7 to 60 days. Application of these models could prevent administration of antimicrobial treatment and burdensome diagnostics in infants who do not need them. What is Known:• Bloodstream infection (BSI) in infants cause non-specific symptoms and may be difficult to diagnose.• Results of blood cultures can take up to 48 hours. What is New:• Machine learning models can contribute to clinical decision making on BSI in infants while blood culture results are not yet known. Dissonance in the face of Alzheimer's disease breakthroughs: clinician and lay stakeholder acceptance, concerns and willingness to pay for emerging disease-modifying therapies
CONCLUSIONS: This study exposes complex attitudes toward emerging DMTs for Alzheimer's disease, challenging conventional wisdom in multiple dimensions. A surprising 25% of the public expressed aversion to these new treatments, despite society's deep-rooted fear of dementia in older age. Healthcare professionals displayed nuanced concerns, prioritising clinical effectiveness and potential brain complications. Intriguingly, younger, better-educated and privately insured individuals exhibited a greater WTP, foregrounding critical questions about healthcare equity. These multifaceted findings serve as a guidepost for healthcar...
