Intractable Postoperative Pancreatic Fistula: Undefined Type of Pancreatic Fistula Managed by Puestow-Like Procedure
J Clin Med Res. 2024 Apr;16(4):182-188. doi: 10.14740/jocmr5123. Epub 2024 Apr 30.ABSTRACTPancreatoduodenectomy (PD) is a very complex and highly challenging operation for surgeons worldwide. It is the surgical procedure of choice for the management of benign and malignant diseases of the periampullary region. Although mortality rate following this complicated surgery has fallen to 1-3%, morbidity rate following PD remains high, with almost 30-40% of patients developing at least one complication. Postoperative pancreatic fistula (POPF) is one of the most common complications following PD. Therefore, Pancreatico-enteric ana...

What We Do and What We Should Do Against Malnutrition in Spinal Cord Injury: A Position Paper From Italian Spinal Cord Injury Network Rehabilitation Centers
This study utilized a cross-sectional survey to assess the strategies employed in seven spinal units across Italy to address the nutritional needs of patients with SCI during the acute, post-acute, and post-discharge phases of the condition. Eight clinicians (five physiatrists, two internists, and one urologist) and one nurse participated in the survey. Following the survey completion, the participants were invited to partake in a round table session to delve deeper into the questionnaire results to gather their opinions and gain insights into clinical practices related to the various challenges surrounding the management ...

Asthma and obstructive sleep apnea: Unveiling correlations and treatable traits for comprehensive care
Chron Respir Dis. 2024 Jan-Dec;21:14799731241251827. doi: 10.1177/14799731241251827.ABSTRACTAsthma and obstructive sleep apnea (OSA) are common respiratory disorders. They share characteristics such as airway obstruction, poor sleep quality, and low quality of life. They are often present as comorbidities, along with obesity, gastroesophageal reflux disease (GERD), and allergic rhinitis (AR), which impacts the disease's control. In recent years, there has been discussion about the association between these conditions and their pathophysiological and clinical consequences, resulting in worse health outcomes, increased healt...

Conservative management of placenta previa ‐percreta with bladder invasion: A case report
We present a case of a 35-year-old woman (G2 P1) who was referred to the Akbar-Abadi hospital at 13  weeks of gestation. Color Doppler ultrasound indicated complete placenta previa-percreta with bladder invasion. After induction of fetal demise, bilateral uterine and bladder artery endovascular embolization was conducted for the patient. After 48 h, under ultrasound guidance, surgical resectio n of residual percreta tissue was conducted as much as possible. Eight weeks later, a follow-up sonography showed the minimum residual placenta tissue and she regained menstrual cycles after 2 months. This case indicated that t...

A fortunate bitten tongue —Hypothyroidism despite repeatedly normal plasma thyrotropin levels
Hypothyroidism despite repeatedly normal plasma thyrotropin levels in a patient with hypophyseal tumor. Key Clinical MessageHypophyseal dysfunction may be overlooked by the currently generally accepted laboratory routine for the differential diagnosis in patients suffering from symptoms of depression or dementia.AbstractHypothyroidism is an important cause of depression and potentially reversible cognitive impairment. Whereas the determination of the plasma concentration of thyrotropin (TSH) is generally considered part of the laboratory screening tests for dementia, the measurement of total or free triiodothyronine (T3, F...

Calcification within the urachus: A case report of rare presentation of urachal cyst with calculus formation
Key Clinical MessageUrachal anomalies are rare congenital lesions of the genitourinary tract and are important causes of pediatric and adolescent hospital presentations. It can mimic many other causes of intraabdominal pathology and fever, and pose diagnostic challenges, often aided by imaging, is crucial to prevent complications such as infection and calculus formation. Surgical intervention, preferably laparoscopic, is the primary treatment, with complete excision necessary to avoid potential malignant transformation. (Source: Clinical Case Reports)

Wearing face masks while climbing stairs influences respiratory physiology
In this study, healthy adults climbed 5 floors with and without a mask. Various cardio-respiratory parameters were measured, including O2 −saturation (O2−Sat) and end-tidal CO2 (EtCO2), at baseline and on the top floor. Subjective indexes, such as Borg’s scale, were evaluated. Thirty-two volunteers (16 males), median age 39 years (IQR 32.5–43), median BMI = 23.6 (IQR 21.5–25.1), with good fitness levels, participated. Compar ing baseline to end-activity, median (IQR): O2−Sat change was −1.0% (−2–0) without mask, versus −3.0% (−4–0) with mask, p = 0.003; EtCO2 + 7.0 (+3.3–+9) without mask, versus +...

Effects of the delivery of physiotherapy on the treatment course of elderly fallers presenting to the emergency department: Protocol for a randomized clinical trial
This study will follow a prospective longitudinal design involving participants aged 75 years and over. We plan to recruit a total n = 336 patients admitted t o the ED with a ‘fall’ chief complaint. After consent, participants will be randomized into either the ‘PT-group’ (receiving a prescription and execution of PT within the ED), or to the ‘no-PT group’ (no delivery of PT within the ED). The PT intervention will involve a standardized asses sment of motor capacities using validated clinical examinations, and the delivery of rehabilitative exercises based on individual needs. Outcomes will be recorded from th...

Cohort profile: the Environmental Reproductive and Glucose Outcomes (ERGO) Study (Boston, Massachusetts, USA) -- a prospective pregnancy cohort study of the impacts of environmental exposures on parental cardiometabolic health
Purpose Pregnancy and the postpartum period are increasingly recognised as sensitive windows for cardiometabolic disease risk. Growing evidence suggests environmental exposures, including endocrine-disrupting chemicals (EDCs), are associated with an increased risk of pregnancy complications that are associated with long-term cardiometabolic risk. However, the impact of perinatal EDC exposure on subsequent cardiometabolic risk post-pregnancy is less understood. The Environmental Reproductive and Glucose Outcomes (ERGO) Study was established to investigate the associations of environmental exposures during the perinatal peri...

Prevalence and risk factors associated within 48-hour unplanned paediatric intensive care unit readmissions: An integrative review
Unplanned paediatric intensive care unit (PICU) readmission is associated with increased morbidity/mortality, hospital length of stay, and health service cost and is recognised as a key performance indicator of quality-of-care delivery. However, research evidence on unplanned PICU readmission risk factors is limited, and results were inconsistent across studies. (Source: Australian Critical Care)

IJERPH, Vol. 21, Pages 603: Protocol for Evaluating Remote Patient Blood Pressure Monitoring Adapted to Black Women and Birthing Persons
las Evans Cardiovascular disease is the leading cause of maternal death among Black women in the United States. A large, urban hospital adopted remote patient blood pressure monitoring (RBPM) to increase blood pressure monitoring and improve the management of hypertensive disorders of pregnancy (HDP) by reducing the time to diagnosis of HDP. The digital platform integrates with the electronic health record (EHR), automatically inputting RBPM readings to the patients’ chart; communicating elevated blood pressure values to the healthcare team; and offers a partial offset of the cost through insurance plans....
