Prevalence and genetic characteristics of  Staphylococcus aureus isolates from cell phones of medical students from Iran
Acta Microbiol Immunol Hung. 2024 Apr 15. doi: 10.1556/030.2024.02193. Online ahead of print.ABSTRACTAlthough mobile phones as a rapid communication vehicle can lead to improved quality of healthcare, they can also facilitate the transmission of pathogens to patients. This current research focuses on genetic diversity, and genes involved in resistance and biofilm production of Staphylococcus aureus isolates from mobile phones of medical students. Antibiotic resistance profiling and polymerase chain reaction (PCR) amplification of antibiotic resistance and biofilm-related genes were investigated and statistically analyzed. ...

Patients with idiopathic pulmonary fibrosis and refractory cough have traction bronchiectasis and distorted airway architecture: a retrospective case review study
The objective of this study was to investigate the clinical features of IPF patients with refractory cough and to clarify mechanisms for cough in these patients. We retrospectively reviewed the files of patients with the diagnosis of IPF at Kanazawa University Hospital and compared the clinical features of IPF patients with refractory cough with the clinical features of IPF patients without refractory cough. Among a total of 23 patients with IPF, 10 patients (43.5%) had chronic cough. Of the ten patients, seven patients had concomitant conditions that could lead to cough. Of these seven patients, the cough of four patients...

COVID-19 Pandemic: Impact and rapid reaction of Urology
Actas Urol Esp. 2020 Sep;44(7):450-457. doi: 10.1016/j.acuro.2020.04.006. Epub 2020 Apr 24.ABSTRACTThe COVID-19 pandemic caused by the SARS-CoV-2 virus has caused tens of thousands of deaths in Spain and has managed to breakdown the healthcare system hospitals in the Community of Madrid, largely due to its tendency to cause severe pneumonia, requiring ventilatory support. This fact has caused our center to collapse, with 130% of its beds occupied by COVID-19 patients, thus causing the absolute cessation of activity of the urology service, the practical disappearance of resident training programs, and the incorporation of a...

Cardiology: What You May Have Missed in 2023
Ann Intern Med. 2024 Apr 16. doi: 10.7326/M24-0581. Online ahead of print.ABSTRACTCardiology and all its subspecialties continue to push the envelope in developing new treatment strategies for a wide variety of diseases. After screening more than 1300 articles, we highlight a selection of important cardiology articles published in 2023. Starting with prevention, we note articles that look at the effect of semaglutide in patients with obesity as well as a first-in-class drug, bempedoic acid, on cardiovascular outcomes. We have also examined new evidence comparing conservative management with invasive management of frail, ol...

Innovative pathways allow safe discharge of mild acute pancreatitis from the emergency room
World J Gastroenterol. 2024 Mar 21;30(11):1475-1479. doi: 10.3748/wjg.v30.i11.1475.ABSTRACTAcute pancreatitis (AP) is a leading cause of gastrointestinal-related hospitalizations in the United States, resulting in 300000 admissions per year with an estimated cost of over $2.6 billion annually. The severity of AP is determined by the presence of pancreatic complications and end-organ damage. While moderate/severe pancreatitis can be associated with significant morbidity and mortality, the majority of patients have a mild presentation with an uncomplicated course and mortality rate of less than 2%. Despite favorable outcomes...

Pro-Con Debate: Universal Versus Selective Continuous Monitoring of Postoperative Patients
Anesth Analg. 2024 May 1;138(5):955-966. doi: 10.1213/ANE.0000000000006840. Epub 2024 Apr 15.ABSTRACTIn this Pro-Con commentary article, we discuss use of continuous physiologic monitoring for clinical deterioration, specifically respiratory depression in the postoperative population. The Pro position advocates for 24/7 continuous surveillance monitoring of all patients starting in the postanesthesia care unit until discharge from the hospital. The strongest arguments for universal monitoring relate to inadequate assessment and algorithms for patient risk. We argue that the need for hospitalization in and of itself is a su...

Multimodal Treatment of Chylous Fistula: A Retrospective Case-Control Study
This study presents the treatment modalities performed on patients who developed CF over a 10-year period. During the observation period, CF developed in 29 patients, 16 of whom were women. The mean age was 55.76± 13.48. Lymphatic duct injury was mostly seen in the abdomen (58.6%) and the most common reason was nephrectomy (20.7%). Extended lymphatic dissection due to malignancy was performed in 82.7% of all cases. Chylous leakage started postoperatively on 3.78±3.94 days (range: 1-19 days). Fasting, total parenteral nutrition (TPN), and somatostatin treatment were applied to all patients, and 75.8% of the fistulas were ...

Converting IMPROVE bleeding and VTE risk assessment models into a fast-and-frugal decision tree for VTE prophylaxis
Blood Adv. 2024 Apr 15:bloodadvances.2024013166. doi: 10.1182/bloodadvances.2024013166. Online ahead of print.ABSTRACTCurrent hospital VTE (venous thromboembolism) prophylaxis for medical patients has been characterized by both underuse and, increasingly, overuse. The American Society of Hematology (ASH) has endorsed the use of risk assessment models (RAM) as an approach to individualize VTE prophylaxis as a way of balancing overuse (excessive risk of bleeding) and underuse (risk of avoidable VTE). ASH has endorsed IMPROVE (International Medical Prevention Registry on Venous Thromboembolism) risk assessment models - the on...
