Renal tubular necrosis associated with anagrelide administration: a case report
AbstractA 62-year-old female patient with essential thrombocythemia experienced rapid renal dysfunction and was subsequently referred to our hospital. Further investigations did not reveal any significant abnormalities except for a slight increase in urinary β2-microglobulin levels. A renal biopsy was performed to investigate the cause of her renal dysfunction, revealing acute tubular necrosis, interstitial edema, and arteriosclerosis. No significant glomerular lesions were observed. Immunofluorescence staining and electron microscopy showed no abnorma lities. She had been using anagrelide for 4 years, and her dosage was...

Optimal timing of electrical cardioversion for acute decompensated heart failure caused by atrial arrhythmias: The earlier, the better?
AbstractThe optimal timing for electrical cardioversion (ECV) in acute decompensated heart failure (ADHF) with atrial arrhythmias (AAs) is unknown. Here, we retrospectively evaluated the impact of ECV timing on SR maintenance, hospitalization duration, and cardiac function in patients with ADHF and AAs. Between October 2017 and December 2022, ECV was attempted in 73 patients (62 with atrial fibrillation and 11 with atrial flutter). Patients were classified into two groups based on the median number of days from hospitalization to ECV, as follows: early ECV (within 8  days,n = 38) and delayed ECV (9 days or more,n =...

Association between the quantity of stroke care units and the complement of neurosurgical and neurology specialists in Japan: A retrospective study
Initial treatment in a stroke unit (SU) can reduce the mortality of stroke victims, increase the rate of discharge to home, and decrease the amount of time spent in the hospital. The effectiveness of an SU has also been proven in a meta-analysis. Stroke units have been established in various locations in Europe since 1990, and the medical system for managing the acute phase of stroke has improved1, 2. The Japanese Stroke Association recommends in its Stroke Treatment Guidelines 2021 that acute stroke patients should be treated by a multidisciplinary stroke team in an SU that provides intensive care and early rehabilitation...

Reply to Letter to the Editor on “Predictors of Early Continence after Single-Port Transvesical Robot-Assisted Radical Prostatectomy”
We appreciate the authors' Editorial Letter and concur with their comments regarding the continued minimalization of surgical invasiveness. We endorse the highlighted benefits of the single-port (SP) transvesical (TV) approach, notably its one incision, extraperitoneal surgical access, preservation of surrounding structures, minimal postoperative pain and opioid usage, and short hospital stays. In terms of functional outcomes post SP TV robot-assisted radical prostatectomy (RARP), we emphasize the marked improvement in urinary control, a benefit thus far associated solely with the multi-port (MP) retropubic-space-sparing (...
