Association between chronic liver disease and adverse in-hospital outcomes in patients undergoing CABG: A propensity score-matched analysis of a 5-year national inpatient sample data from 2016-2020
This study investigates the association between CLD and in-hospital outcomes in CABG patients using retrospective data from the National Inpatient Sample (NIS) database (2016-2020) including 7,945 CLD patients undergoing CABG that were propensity score matched with an equivalent number of patients with no CLD undergoing CABG. Echocardiography In Sepsis: Can One Snapshot Tell The Whole Story?
It is with great interest that we read the single-center retrospective cohort study by Blank and Blank on the impact of echocardiography on mortality in sepsis in Critical Care Medicine. (1) Not only does this study add to the body of literature on sepsis, a major contributor to hospital morbidity and mortality, it also provokes an important discussion on how to best utilize echocardiography to manage our patients. Amiodarone in a rush: caution on speed of delivery and new guidance
A boy aged 11 months presented to his local hospital with a short history of being febrile with vomiting. He was found to be shocked with a pulsed broad complex tachycardia of around 240 and increased work of breathing. He was managed as per the APLS (Advanced Paediatric Life Support) protocol and had short-lived terminations after his first two shocks. He was loaded with amiodarone over 5 min prior to his third shock, but promptly arrested with loss of electrical activity requiring 20 min cardiopulmonary resuscitation. This transitioned to a return of broad complex tachycardia with output and a further 20 min of shocks an...

A novel transformer-based aggregation model for predicting gene mutations in lung adenocarcinoma
AbstractIn recent years, predicting gene mutations on whole slide imaging (WSI) has gained prominence. The primary challenge is extracting global information and achieving unbiased semantic aggregation. To address this challenge, we propose a novel Transformer-based aggregation model, employing a self-learning weight aggregation mechanism to mitigate semantic bias caused by the abundance of features in WSI. Additionally, we adopt a random patch training method, which enhances model learning richness by randomly extracting feature vectors from WSI, thus addressing the issue of limited data. To demonstrate the model ’s eff...

Risk factors and mortality of candidemia in a children's public hospital in Sao Paulo, Brazil
This study aimed to estimate the incidence and risk factors associated with mortality in candidemia cases occurring in a public children's hospital in Ribeirao Preto, Brazil. It is a retrospective transversal study. Every patient under the age of 18 admitted to the study facility from January 1, 2013, to December 31, 2019, was considered potentially eligible to be included if they had candidemia. We collected clinical data from medical records. We included 113 blood cultures yielding positive results for Candida. The incidence rate was 2.12 per 1000 admissions. The most common Candida species was Candida parapsilosis. Sept...

Coping strategies of intensive care unit nurses reducing moral distress: A content analysis study
DISCUSSION: The intensive care unit nurses in dealing with ethical problems first try to solve the problem through discussion, but when they fail to resolve it peacefully, they resort to several coping strategies. Factors, such as increasing experience, lack of support from hospital managers and officials, poor communication between colleagues, the need to maintain hierarchy, fear of reprimand, and a sense of powerlessness, changed the nurses' preferred strategies. It is important for managers to provide a blame/punishment-free atmosphere for expression of ethical experiences; a supportive atmosphere in which staff can eng...

Translation into French and republication of: "Home treatment for patients with cancer-associated venous thromboembolism"
Rev Med Interne. 2024 Apr 16:S0248-8663(24)00096-1. doi: 10.1016/j.revmed.2024.04.003. Online ahead of print.ABSTRACTPatients hospitalised with acute venous thromboembolism (VTE), and notably patients with pulmonary embolism, often remain in hospital for extended periods due to the perceived risk of complications. However, several studies have shown that home treatment of selected patients is feasible and safe, with a low incidence of adverse events. This may offer clear benefits for patients' quality of life, hospital planning and cost to the health service. Nonetheless, there is a need for a VTE risk-stratification tool ...

Approach to the Neonate with Hypoglycemia
J Clin Endocrinol Metab. 2024 Apr 17:dgae267. doi: 10.1210/clinem/dgae267. Online ahead of print.ABSTRACTAfter birth, healthy neonates undergo a period of altered glucose metabolism, known as "transitional hypoglycemia". During the first 0-4 hours of life, the mean plasma glucose concentration decreases to 57 mg/dL, then by 72-96 hours of life increases to 82 mg/dL, well within the normal adult range. Recent data suggests that transitional hypoglycemia is due to persistence of the fetal beta cell's lower threshold for insulin release, resulting in a transient hyperinsulinemic state. While hypoglycemia is an expected part o...

Unlocking the Potential of Induced Pluripotent Stem Cells in Revolutionizing Cancer Therapy
This article highlights the potential of iPSC- based NK cells and dendritic cells for immunotherapy and delves into the role of iPSC-based mesenchymal cells in targeted therapy. The potential of iPSC-derived organoids as a vital tool for disease modeling and drug discovery has been showcased, and the importance of iPSC-based cancer vaccines is also emphasized. The ongoing clinical trials of iPSC-based cancer treatment have also been highlighted. Though much work remains to be done to implicate these iPSC-based therapeutic options from research labs to clinics and hospitals, ongoing studies and clinical/translational follow...

Intern Med. 2024 Apr 16. doi: 10.2169/internalmedicine.3412-23. Online ahead of print.ABSTRACTA 51-year-old woman with fever was admitted to our hospital. A computed tomography (CT) scan showed thickened colonic walls. Colonoscopy revealed erosion in the ileum and colon. Adult-onset Still's disease (AOSD) was diagnosed due to a subsequent sore throat and skin rash. Following AOSD treatment, methylprednisolone pulse therapy, followed by prednisolone and cyclosporine, was initiated. Despite achieving a temporary improvement, relapse occurred with fever, abdominal pain, with worsening CT and endoscopic findings. The reappeara...
