Emergence of OXA-48-producing Klebsiella pneumoniae in Lithuania, 2023: a multi-cluster, multi-hospital outbreak
In 2023, an increase of OXA-48-producingKlebsiella pneumoniae was noticed by the Lithuanian National Public Health Surveillance Laboratory. Whole genome sequencing (WGS) of 106 OXA-48-producingK. pneumoniae isolates revealed three distinct clusters of carbapenemase-producingK. pneumoniae high-risk clones, including sequence type (ST) 45 (n = 35 isolates), ST392 (n = 32) and ST395 (n = 28), involving six, six and nine hospitals in different regions, respectively. These results enabled targeted investigation and control, and underscore the value of national WGS-based surveillance for antim...

Decreased risk of underdosing with continuous infusion versus intermittent administration of cefotaxime in patients with sickle cell disease and acute chest syndrome
by Keyvan Razazi, Enora Berti, Jerome Cecchini, Guillaume Carteaux, Anoosha Habibi, Pablo Bartolucci, Romain Arrestier, S égolène Gendreau, Nicolas de Prost, Anne Hulin, Armand Mekontso Dessap ObjectiveUnderdosing of antibiotics is common in patients with sickle cell disease (SCD). We hypothesized that in critically-ill patients with SCD receiving cefotaxime during acute chest syndrome, the continuous infusion may outperform the intermittent administration in achieving pharmacokinetic/pharmacodynamic targets. DesignProspective before-after study. SettingsIntensive-care unit of a French teaching hospital and sickle cell d...

Biological characterization and complete genome analysis of the newly isolated Serratia liquefaciens phage vB_SlqS_ZDD2
AbstractA novel lytic phage named vB_SlqS_ZDD2 was isolated from hospital sewage using the double-layer agar method withSerratia liquefaciens ATCC 27592 as the host. BLASTn analysis showed that the genome sequence of phage vB_SlqS_ZDD2 did not resemble any other phages in the NCBI database. Phenotype and phylogeny analysis indicated that this phage might be a new member of the classCaudoviricetes. Phage vB_SlqS_ZDD2 has a dsDNA genome of 49,178 bp with 55% GC content and has 73 open reading frames. This phage exhibited strong lytic activity and a wide range of pH (3 –12) and temperature tolerance (below 70℃). (Source: ...

Viruses, Vol. 16, Pages 630: Identification of Bacteria and Viruses Associated with Patients with Acute Febrile Illness in Khon Kaen Province, Thailand
Piyada Linsuwanon The majority of cases of undifferentiated acute febrile illness (AFI) in the tropics have an undefined etiology. In Thailand, AFI accounts for two-thirds of illnesses reported to the Ministry of Public Health. To characterize the bacterial and viral causes of these AFIs, we conducted molecular pathogen screening and serological analyses in patients who sought treatment in Chum Phae Hospital, Khon Kaen province, during the period from 2015 to 2016. Through integrated approaches, we successfully identified the etiology in 25.5% of cases, with dengue virus infection being the most common cause, noted i...

HIV testing at visits to US emergency departments: 2021 update
In this updated cross-sectional analysis of the National Hospital Ambulatory Medical Care Survey, we found that among the 2.5 million more weighted emergency department (ED) visits in 2021 compared with 2020, there was an insignificant increase in HIV testing per ED visit in 2021 compared with 2020 (0.81% to 0.86%). This suggests HIV testing during ED visits did not increase in line with rebounding visit volumes after the pandemic nadir. (Source: Sexually Transmitted Infections)

A Direct Assessment of Noninvasive Continuous Blood Pressure Monitoring in the Emergency Department and Intensive Care Unit
Noninvasive continuous blood pressure monitoring has the potential to improve patient treatment in the hospital  setting. Such noninvasive devices can be applied earlier in the treatment process to empower nurses and clinicians to react more quickly to patient deterioration with the added benefit of eliminating the risks associated with invasive monitoring. However, emerging technologies must be capable of r eproducing current clinical measures for medical decision making. (Source: Journal of Emergency Nursing: JEN)

A retrospective cohort study: evaluating the efficacy of standard versus tubeless percutaneous nephrolithotomy (PCNL) in pediatric patients up to 18 years old
AbstractPercutaneous nephrolithotomy (PCNL) is an endourological method applied as the standard or tubeless method for kidney stone treatment. In a retrospective cohort study, 88 surgery units involving 75 children up to 18  years old with kidney stones who underwent the surgery for 8 years in Shahid Beheshti and Boo-ali hospitals in Hamadan with one of the two methods of standard or tubeless PCNL were evaluated and compared considering the success rate of operation, hemoglobin, hematocrit drop, need for medications, need for blood transfusion, duration of surgery, and the length of hospital stay. Among the 88 units stud...

In Response: Diagnosing functional seizures with a single video-EEG may miss epileptic seizures
We would like to thank Dr Finsterer from Austria for their keen interest in our work [1]. Our article titled “Medication use in patients with functional seizures from a public and a private hospital” [2] aimed to elucidate the pharmaceutical aspects of functional seizure (FS) management as well as to compare these management practices between a private and a public sector hospital. A Gentle Introduction to Latent Class Analysis for Researchers in Pediatrics
A recent study in The Journal of Pediatrics from O ’Connor et al reports using latent class analysis (LCA) to discover “complexity” subgroups based on cardiac medical histories of 1,482 participants diagnosed with congenital heart disease (CHD).1 Seven medical history variables (including stroke, cardiac surgeries, and cardiac-related hospital izations) were selected based on indications of CHD complexity. Applying LCA, investigators found four subgroups of complexity: mild, moderate 1, moderate 2, and severe. (Source: The Journal of Pediatrics)
