Pilot of a Telehealth Hospice Transition Intervention for Children and Young Adults with Cancer
1. Participants will be able to evaluate the feasibility and acceptability of a hospice telehealth home-based intervention for pediatric and young adult patients with cancer at the end of life using data obtained from the Technology Acceptable Model 2 survey.2. Participants will be able to assess the self-reported partnership, collaboration, and care coordination between the patient's hospice and hospital providers using the Assessment of Interprofessional Team Collaboration Scale II survey. Predicting in ‐hospital mortality among patients admitted with a diagnosis of heart failure: a machine learning approach
AbstractExisting risk prediction models for hospitalized heart failure patients are limited. We identified patients hospitalized with a diagnosis of heart failure between 7 May 2013 and 26 April 2022 from a large academic, quaternary care medical centre (training cohort). Demographics, medical comorbidities, vitals, and labs were collected and were used to construct random forest machine learning models to predict in-hospital mortality. Models were compared with logistic regression, and to commonly used heart failure risk scores. The models were subsequently validated in patients hospitalized with a diagnosis of heart fail...

Stewardship Prompts to Improve Antibiotic Selection for Pneumonia
This clinical trial examines the effect of an antibiotic stewardship bundle (education, feedback, and real-time multidrug-resistant organism risk-based CPOE prompts) vs routine stewardship on antibiotic selection during the first 3 hospital days in adults with pneumonia. Harnessing the Electronic Health Record to Improve Empiric Antibiotic Prescribing
Approximately half of hospitalized adults in the US receive antibiotics, with pneumonia and urinary tract infection (UTI) being the 2 most frequent indications. Estimates suggest about 30% of all antibiotics prescribed in US hospitals are either unnecessary or suboptimal, and extended-spectrum antibiotic use is common. Unnecessary antibiotic use contributes to numerous harms, including drug-related adverse effects, increased risk of drug resistance, and higher costs. Childhood-onset hypertrophic cardiomyopathy caused by thin-filament sarcomeric variants
This study describes the natural history and outcomes of children with thin-filament-associated HCM and compares it to thick-filament-associated disease. Longitudinal data were collected from 40 children under 18 years with a disease-causing variant in a thin-filament protein from a single quaternary referral centre. Twenty-one (female n=6, 35.5%) were diagnosed with HCM at a median age of 13.0 years (IQR 8.3–14.0). Over a median follow-up of 5.0 years (IQR 4.0–8.5), three (14.3%) experienced one or more major adverse cardiac events (MACE) (two patients had an out-of-hospital arrest and eight appropriate implan...

Iron and risk of dementia: Mendelian randomisation analysis in UK Biobank
Discussion Genetic evidence supports a causal relationship between higher TSAT and risk of clinically defined non-AD and vascular dementia, in European and African groups. This association appears to be independent of apolipoprotein-E 4. (Source: Journal of Medical Genetics)

Sleep studies in obese children with obstructive sleep apnea: Pulse oximetry as a diagnostic tool
This study assesses whether oximetry is a reliable diagnostic tool for obstructive sleep apnea in obese children. We reviewed the medical records of obese children with a suspected diagnosis of obstructive sleep apnea who underwent oximetry and cardiorespiratory polygraphy or full polysomnography at Sheffield Children's Hospital between January 2010 and March 2022. We compared oximetry results with the apnea-hypopnea index from cardiorespiratory polygraphy or full polysomnography for each patient. A total of 60 patient records were included in the analysis. The sensitivity of oximetry in diagnosing obstructive sleep apnea ...

Novel Skin Prep Technique Reduces OR Preparation Times in a Randomized Trial for Podiatric and Orthopedic Procedures
Surg Technol Int. 2024 Apr 18;44:sti44/1751. Online ahead of print.ABSTRACTThe primary objectives of any high-volume surgery department should be patient safety, block time utilization and operating room efficiency. Reducing preparation time in the OR prior to actual surgery can improve operating room efficiency and utilization, but only if patient safety can be maintained. With this goal, this study evaluated a novel skin preparation technique using a device named ULTRAPREP™, a sterile, medical-grade plastic bag that is applied to the upper or lower extremity in the pre-operative holding area which allows for skin disin...

An appropriate way to provide such exposure is through distributed clinical training. This usually entails students training in smaller groups along the continuum of care, away from tertiary academic hospitals. This also means students are away from their existing academic and social support systems. It is evident that knowl...

Acute myocardial infarction with e ‐cigarette or vaping‐use associated lung injury in a young Filipino vape user
We present a 22-year-old Filipino male with sudden chest pain. He had no known comorbidities but had a two-year history of daily vape use. Work-up revealed elevated cardiac markers, anteroseptal ST-elevation myocardial infarction, hypokinesia of the anterior wall and interventricular septum, and an ejection fraction of 30%. Chest radiography showed consolidation pneumonia but culture studies and Biofire Pneumonia Panel were negative for microbial detection. Coronary angiography revealed chronic total obstruction of the mid-left anterior descending (LAD) and right coronary arteries (RCA). Percutaneous coronary angioplasty o...
