Physiological Assessment with iFR prior to FFR Measurement in Left Main Disease
AbstractDespite guideline-based recommendation of the interchangeable use of instantaneous wave-free ratio (iFR) and fractional flow reserve (FFR) to guide revascularization decision-making, iFR/FFR could demonstrate different physiological or clinical outcomes in some specific patient or lesion subsets. Therefore, we sought to investigate the impact of difference between iFR and FFR-guided revascularization decision-making on clinical outcomes in patients with left main disease (LMD). In this international multicenter registry of LMD with physiological interrogation, we identified 275 patients in whom physiological assess...

Initial experience with a novel stent-based mechanical thrombectomy device for management of acute myocardial infarction cases with large thrombus burden
ConclusionsIn AMI patients with LTB, the deployment of the novel NeVa ™ MTD seems efficient and safe. Further randomized trials are warranted to assess whether the use of the NeVa™ device in cases with LTB improves procedural and clinical outcomes.Graphical abstract (Source: Cardiovascular Intervention and Therapeutics)

Medical Nutrition Therapy Provided by a Dietitian Improves Outcomes in Adults with Pre-Hypertension or Hypertension: A Systematic Review and Meta-Analysis
CONCLUSION: Dietitians play a critical role in improving cardiometabolic risk factors for adults with elevated BP; thus, improved payment for and access to MNT services has the potential to significantly impact public health.REGISTRATION ID AND URL: This systematic review was registered in the International Prospective Register of Systematic Reviews: PROSPERO# CRD42022351693 (https://www.crd.york.ac.uk/prospero/display_record.php?ID=CRD42022351693).PMID:38641320 | DOI:10.1016/j.ajcnut.2024.04.012 (Source: The American Journal of Clinical Nutrition)

Acute myocardial infarction due to accidental electrocution — A case report of a 26-year-old African male patient
We report a case of fatal myocardial infarction (MI) after electrocution in a 26-year-old male patient without cardiovascular risk factors. The patient was admitted to the Emergency Department of the University Clinics of Lubumbashi for management of accidental electrocution on a high-voltage power line. The admission ECG showed lesions consistent with myocardial infarction. Despite treatment, the patient died 30 minutes after admission.PMID:38640884 | DOI:10.1016/j.ancard.2024.101743 (Source: Annales de Cardiologie et d'Angeiologie)

piRNAs as emerging biomarkers and physiological regulatory molecules in cardiovascular disease
Biochem Biophys Res Commun. 2024 Apr 7;711:149906. doi: 10.1016/j.bbrc.2024.149906. Online ahead of print.ABSTRACTCardiovascular diseases (CVD) represent one of the most considerable global health threats, owing to their high incidence and mortality rates. Despite the ongoing advancements in detection, prevention, treatment, and prognosis of CVD, which have resulted in a decline in both incidence and mortality rates, CVD remains a major public health concern. Therefore, novel diagnostic biomarkers and therapeutic interventions are imperative to minimise the risk of CVD. Non-coding RNAs (ncRNAs) have recently gained increas...

Newer Pharmacologic Treatments in Adults With Type 2 Diabetes: A Clinical Guideline From the American College of Physicians
Ann Intern Med. 2024 Apr 19. doi: 10.7326/M23-2788. Online ahead of print.ABSTRACTDESCRIPTION: The American College of Physicians (ACP) developed this clinical guideline to update recommendations on newer pharmacologic treatments of type 2 diabetes. This clinical guideline is based on the best available evidence for effectiveness, comparative benefits and harms, consideration of patients' values and preferences, and costs.METHODS: This clinical guideline is based on a systematic review of the effectiveness and harms of newer pharmacologic treatments of type 2 diabetes, including glucagon-like peptide-1 (GLP-1) agonists, a ...

Construction of preclinical evidence for propofol in the treatment of reperfusion injury after acute myocardial infarction: A systematic review and meta-analysis
This study presents a comprehensive analysis of 48 relevant animal studies investigating propofol's preventive effects on MIRI. Propofol administration demonstrated a reduction in myocardial IS and decreased levels of myocardial injury biomarkers (CK-MB, LDH, cTnI). Moreover, propofol improved myocardial function parameters (+dp/dtmax, -dP/dtmax, LVEF, LVFS), exhibited favorable effects on inflammatory markers (IL-6, TNF-α) and oxidative stress markers (SOD, MDA), and reduced myocardial cell apoptotic index (AI). These findings suggest propofol exerts cardioprotective effects by reducing myocardial injury, decreasing infa...
