The role of arginine vasopressin in myocardial infarction and reperfusion.
Abstract Little attention is paid to the coronary microvasculature when treating acute myocardial infarction (MI). Microvascular obstruction (MVO) contributes to ischaemia-reperfusion (I-R) injury which hampers distal blood flow to the myocardium despite recanalisation of the culprit epicardial vessel. One of the mechanisms behind reperfusion injury is MVO due to persistent vasoconstrictor tone during reperfusion.  Arginine vasopressin (AVP) is a hormone with prominent vasoactive effect on the coronary microvessels. AVP levels are elevated as part of a stress response triggered by MI which can exert vasoconst...

How can we reverse bleeding in patients on direct oral anticoagulants?
Abstract The direct oral anticoagulants (DOACs), or non-vitamin K antagonist oral anticoagulants (NOACs), including dabigatran, which inhibits thrombin, as well as rivaroxaban, apixaban, edoxaban, and betrixaban, which inhibit coagulation factor Xa, are as-sociated with similar or lower risk of bleeding compared with warfarin. The need for reversal of their anticoagulant effect may occur in patients with life-threatening bleeding or those requiring urgent surgery. Currently, the only specific reversal agent for dabigatran, idarucizumab, is widely available, while andexanet alfa, which reverses factor Xa inhibitors...

AIM: We sought to evaluate the prevalence of DSs in relation to the socio-demographic status and selected types of cardio-vascular diseases in the adult Polish population. Reduction of left ventricular mass, left atrial size, and N-terminal pro-B-type natriuretic peptide level following alcohol septal ablation in patients with hypertrophic obstructive cardiomyopathy.
Abstract BACKGROUND: Alcohol septal ablation (ASA) is an alternative to surgical treatment in patients with hypertrophic obstructive cardiomyopathy (HOCM). Through alcohol-induced necrosis, ASA leads to an increase in left ventricular outflow tract (LVOT) diameter and a decrease in LVOT pressure gradient. AIM: We sought to assess the effect of ASA on left ventricular (LV) wall thickness and mass, left atrial (LA) size, and N-terminal pro-B-type natriuretic peptide (NT-proBNP) level. Clinical, respiratory, haemodynamic, and metabolic determinants of lactate in heart failure.
Abstract BACKGROUND: Lactate is an end-product of anaerobic cell metabolism. Although it is believed to have prognostic significance in heart failure (HF), data on the pathomechanisms that lead to lactate accumulation are scarce. AIM: We aimed to determine the clinical, respiratory, biochemical, and haemodynamic correlates of lactate in HF. METHODS: Patients diagnosed with HF hospitalised in a single cardiac centre, who underwent haemodynamic monitoring, were included in this retrospective analysis. Persistent hyperlactataemia is related to high rates of in-hospital adverse events and poor outcome in acute heart failure.
Abstract BACKGROUND: Although lactate is a well-established marker in intensive care, our understanding of its utility in acute heart failure (AHF) is modest and based on studies with a single measurement of this marker. AIM: We aimed to investigate whether persistent elevation of lactate during hospitalisation is related to a higher risk of ad- verse events. METHODS: We conducted a prospective study to assess AHF patients hospitalised in one cardiac centre. The diagnosis of persistent hyperlactataemia was based on two measurements of the marker (on admission and at 24 h of hospitalisation) and it was de...
