"Emergency Room Incision and Drainage of Perirectal Abscess is Equivalent to Incision and Drainage in the Operating Room"
In this study, for patients presenting to the ED, bedside drainage was found to be an adequate management strategy to achieve complete drainage without a significant increase in the rate of complications when compared to operative drainage.PMID:38684322 | DOI:10.1177/00031348241248785 (Source: The American Surgeon)

Management of Injuries to the Parenchymal Abdominal Organs
Zentralbl Chir. 2024 Apr 29. doi: 10.1055/a-2301-7951. Online ahead of print.ABSTRACTThe most common organs affected by abdominal trauma are the spleen and the liver, often in combination. Pancreatic injuries are rare. In the case of blunt abdominal trauma, which is much more common, a clinical and laboratory examination as well as sonography should be performed. In the initial assessment, the circulatory situation must be screened. If there is haemodynamic instability and presentation of free fluid, an emergency laparotomy is indicated. If the situation is stable or stabilised and a pathological sonography is present, it ...

Electrographic monitoring for seizure detection in the neonatal unit: current status and future direction
Pediatr Res. 2024 Apr 29. doi: 10.1038/s41390-024-03207-2. Online ahead of print.ABSTRACTNeonatal neurocritical intensive care is dedicated to safeguarding the newborn brain by prioritising clinical practices that promote early identification, diagnosis and treatment of brain injuries. The most common newborn neurological emergency is neonatal seizures, which may also be the initial clinical indication of neurological disease. A high seizure burden in the newborn period independently contributes to increased mortality and morbidity. The majority of seizures in newborns are subclinical (without clinical presentation), and h...

Association between opioid use disorder and palliative care: a cohort study using linked health administrative data in Ontario, Canada
CMAJ. 2024 Apr 28;196(16):E547-E557. doi: 10.1503/cmaj.231419.ABSTRACTBACKGROUND: People with opioid use disorder (OUD) are at risk of premature death and can benefit from palliative care. We sought to compare palliative care provision for decedents with and without OUD.METHODS: We conducted a cohort study using health administrative databases in Ontario, Canada, to identify people who died between July 1, 2015, and Dec. 31, 2021. The exposure was OUD, defined as having emergency department visits, hospital admissions, or pharmacologic treatments suggestive of OUD within 3 years of death. Our primary outcome was receipt of...

Urgent one-stage endoscopic treatment for choledocholithiasis related moderate to severe acute cholangitis: A propensity score-matched analysis
CONCLUSION: Urgent one-stage ERCP provides the advantages of a shorter ICU stay, a shorter duration of antibiotic use, and a shorter hospital stay.PMID:38681983 | PMC:PMC11045478 | DOI:10.3748/wjg.v30.i15.2118 (Source: World Journal of Gastroenterology)

Appendectomy versus antibiotic treatment for acute appendicitis
CONCLUSIONS: Antibiotics may be associated with higher rates of unsuccessful treatment for 76 per 1000 people, although differences may not be clinically significant. It is uncertain if antibiotics increase length of hospital stay by half a day. Antibiotics may reduce wound infections. A third of the participants initially treated with antibiotics required subsequent appendectomy or two-thirds avoided surgery within one year, but the evidence is very uncertain. There were too few data from the included studies to comment on major complications.PMID:38682788 | PMC:PMC11057219 | DOI:10.1002/14651858.CD015038.pub2 (Source: Co...

Autoimmune encephalitis mediated by postvaccination and infection of SARS-CoV-2 in a patient with a narcolepsy type 1
CONCLUSION: The patient fulfilled the diagnostic criteria for autoimmune encephalitis with subacute onset. COVID-19 infection and vaccination could constitute a risk in a patient with narcolepsy as in this case and, could help to provide better understanding of the implication of immune-mediated processes in the pathophysiology of the diseases.PMID:38682764 | DOI:10.33588/rn.7809.2023306 (Source: Revista de Neurologia)

Transfusion management and hemoglobin-based oxygen carrier treatment in a patient with anti-Rh17 antibody
DISCUSSION: Overall difficulties in this case included the challenge of finding compatible units, dilemma of transfusing incompatible units in a patient with severe anemia and obtaining alternatives to blood products. This case report demonstrates the successful use of HBOC-21 in treating life-threatening anemia.PMID:38682958 | DOI:10.1111/trf.17855 (Source: Transfusion)

Emergency Physician Performed Ultrasound-Guided Abdominal Paracentesis: A Retrospective Analysis
CONCLUSIONS: Emergency physicians with training in real-time needle guidance with ultrasound were able to use POCUS to perform ultrasound-guided paracentesis in the emergency department with a high success rate and no fatal complications. Based on our experience, we recommend performing ultrasound-guided paracentesis using a linear transducer, with attention to identifying vessels near the procedure site and maintaining sterile technique.PMID:38681156 | PMC:PMC11044928 | DOI:10.24908/pocus.v9i1.16668 (Source: Pain Physician)

Nitrous oxide-induced myeloneuropathy in a Thai adolescent: a case report
Paediatr Int Child Health. 2024 Apr 29:1-4. doi: 10.1080/20469047.2024.2344403. Online ahead of print.ABSTRACTNitrous oxide, an inhalational anaesthetic, is popular with adolescents worldwide as an accessible recreational drug which induces a euphoric effect. However, chronic abuse leads to serious complications such as myeloneuropathy and bone marrow suppression by inactivation of vitamin B12. A 17-year-old girl presented with nitrous oxide-induced myeloneuropathy. She reported chronic nitrous oxide inhalation for 10 months and was admitted to the emergency department on account of repeated falls for 2 weeks. She also had...

A comparison of healthcare utilization and outcomes following skin vs. serum-specific IgE allergy testing
Conclusions: Allergy testing, regardless of the testing method used, is associated with decreases in ED and UR visits at 1-year follow-up. sIgE allergy testing is associated with lower testing cost and fewer allergist/immunologist visits, compared to skin testing.PMID:38682798 | DOI:10.1080/13696998.2024.2349471 (Source: Journal of Medical Economics)
