[Surgical Therapy of Pulmonal Echinococcosis].
Abstract Human echinococcosis is a rare zoonotic infection caused by larvae of the tapeworm species Echinococcus. The most relevant two species to humans are Echinococcus multilocularis and the dog tapeworm Echinococcus granulosus. The latter causes cystic echinococcosis, which plays a dominant role in thoracic surgery due to its pulmonal involvement. The parasite develops characteristic hydatic cysts mostly in liver and lung. In 2016 a rise in cases of cystic echinococcosis in Germany was recorded, a probable cause could have been the refugee wave. The infection and advanced stages of the disease does not always ...

[Preperitoneal Umbilical Hernia Mesh Plasty (PUMP): Indications, Technique and Results].
CONCLUSION: Surgeons must work with the implant that best suits their patients' needs and that also provides good results and adequate working comfort. The PUMP technique performs well for ventral hernias sized between 2 and 4 cm without the need of midline reconstruction due to diastasis of the rectus muscles. It enables a local extraperitoneal mesh augmentation without the risk of intraperitoneal complications. PUMP repair lowers the risk of recurrence in comparison with suture repair without increasing the risk of complications. [Delphi Expert Consensus of the German Thoracic Surgery Society on the Management of Chest Tube Drainage].
CONCLUSION: Our manuscript depicts the results of a Delphi process in 2017 - 1018 involving experts of the German Thoracic Society from high volume departments certified for thoracic surgery. A very high rate of consensus was documented for topics such as chest tube insertion, peri-interventional management and removal algorithm. The most important topic for which there was no consensus was the case of persistent air leakage. The resulting expert recommendations of the Delphi process could be used as a starting point for internal clinical procedures. PMID: 31394582 [PubMed - as supplied by publisher] (Source: Zent...

[Surgical Management of Obstetric Rectovaginal Fistulas: the Gynaecological Point of View].
Abstract Obstetric injuries can rarely lead to formation of rectovaginal fistula (RVF). The aim of this retrospective study was to evaluate the causes and management of obstetric rectovaginal fistulas in patients treated at the Department of Obstetrics and Gynecology, Tuebingen, from January 2015 to April 2019. Five of 12 surgically treated rectovaginal fistulas were primary fistulas, and 7 patients had recurrence of a fistula. The surgery was carried out by using a vaginal approach following birth or in the context of a subsequent delivery. The latter procedure was chosen in patients at high risk of recurrence or...

[Adequate Perioperative Use of Antibiotics in Thoracic Surgery].
CONCLUSION: The commonly used antibiotics for perioperative prophylaxis in thoracic surgery cover only some of the pathogens responsible for preoperative airway colonisation and postoperative pneumonia. Therefore, perioperative antibiotic prophylaxis should be given as a single shot just before surgery and postoperative pneumonia should be treated as a hospital acquired pneumonia with respect to the local pathogen and resistance pattern. [Residual Pleural Space after Lung Resection].
Abstract Approximately 10% of patients exhibit a residual pleural space after anatomical lung resection. The most common causes are related to interstitial lung diseases associated with reduced compliance and air leaks. If no complication occurs by the end of the 4th week, an uncomplicated course (absorption of air) can be expected. In the event of pleural space infection and/or bronchopleural fistula, there is a risk of aspiration, with the development of life-threatening pneumonia. In such cases, surgical treatment is indicated. The choice of surgical procedure is an individual decision, whereby the general cond...

[Recent Developments in the Regional Treatment of Peritoneal and Pleural Tumours (incl. HIPEC and HITOC)].
Abstract The multimodal treatment of limited peritoneal metastases may improve prognosis in selected patients (pseudomyxoma peritonei, malignant peritoneal mesothelioma, colorectal, gastric and ovarian cancer) provided complete cytoreduction can be performed. Additive intraoperative hyperthermic intraperitoneal chemotherapy (HIPEC) is often performed. If the treatment is undertaken in experienced (and certified) centres, associated mortality is low. Intrapleural hyperthermic chemoperfusion (HITOC) can be performed in patients with pleural malignancies, mainly for malignant pleural mesothelioma or metastases from a...

Abstract In this review, the current state of stereotactic body radiation therapy (SBRT) is presented as a local therapy for patients with oligometastatic, oligoprogressive and oligorecurrent liver metastases - focusing on recent publications from German-speaking countries. In addition to precise imaging, modern SBRT techniques also use gating and tracking techniques that have made local therapy of liver metastases more effective. In combination with optimisation of central tumour dose, local control rates of up to 90% have been achieved with minimal side effects in less than 1% of patients and 5-year survival rat...

[Systemic Therapy of Metastatic Colorectal Cancer].
Abstract The successful management of metastatic colorectal cancer (mCRC) is an interdisciplinary challenge, involving medical oncology, surgery and also interventional specialists. Systemic therapy of mCRC is based on regimens of chemotherapeutic combinations plus monoclonal antibodies targeting EGFR and VEGF. EGFR-directed antibodies can guided by biomarkers, namely RAS mutation and sidedness of the primary tumor. In pretreated patients, several targetable molecular markers (i.e. BRAF mutation, MSI-status and Her2) should be tested. BRAF inhibitors, checkpoint-inhibitors and her2-targeted combinations (although ...

[Ablative Modalities in the Treatment of Liver Metastases].
Abstract In the absence of extrahepatic metastases, improved survival can be achieved with surgical and interventional techniques. In resectable patients the resection of liver metastases can be performed in curative intention. Interventional techniques are defined either as percutaneous or intraoperative laparoscopy, applications of thermal ablation. In this review, we concentrate on the most established ablative techniques of radiofrequency ablation (RFA) and microwave ablation (MWA) for the treatment of colorectal liver metastases. These techniques allow safe ablation of metastases of up to 3 cm and have...

[Management of Rib Fractures].
Abstract The therapy of rib fractures is controversially discussed. Neither does an osteosynthesis have to be performed for each individual rib fracture, nor is the internal splinting by ventilation or respiratory therapy sufficient for each patient. Rib fractures are common in polytrauma patients after car and motorcycle accidents or falls from great heights. However, rib fractures are also increasingly occurring in older patients with falls from low heights. Anamnesis and clinical examination are groundbreaking for the diagnosis and therapy decision of rib fractures. In radiological diagnostics, projection radio...

[Quality Assurance Using Administrative/Routine Data for Cholelithiasis - Changes Over Time and Current Status].
CD Abstract Quality assurance using administrative/routine data (QSR) is a relatively new measure to assess outcome quality. This approach is methodologically distinct from external quality assurance, as well as from quality assurance based upon registries. Since 2011, QSR for cholelithiasis within AOK patients has been overseen by the Scientific Institute of the AOK (WIdO). Following the introduction of an expert panel in 2013, numerous changes were put into place, whereby the indicator rates for transfusion/bleeding, various complications as well as the overall indicator were reduced. Interestingly, the risk ad...

CONCLUSION: With open source software it is possible to establish a modern e-learning resource. In order to maximise the reach of the e-learning beyond the own faculty, it is crucial not to use proprietary standards, but to respect a platform-independent technical design and to make the content strictly public accessible. In future development, greater emphasis should be put on personalising the e-learning experience in order to better understand the effects on the learning results. [Establishment of Multidimensional Structured Training Curriculum in Surgery].
Abstract In Germany, medical education is defined and controlled by the medical association; however hospitals are responsible for its implementation. Surgical trainees do not only need to acquire theoretical knowledge, but also practical skills. For years, young surgeons were trained primarily in the operating theatre. Nowadays, ethical concerns as well as time and budget limitations and legislated restrictions on surgeons' working hours argue for modern training concepts. Therefore, we have developed a training curriculum to ensure a structured surgical education. A standardised surgical education leading to wel...
