Total spinal anaesthesia following obstetric neuraxial blockade: a narrative review
Total spinal anaesthesia (TSA) is a recognised complication of all neuraxial local anaesthetic (LA) techniques. It refers to a high neuraxial block causing respiratory failure and/or loss of consciousness, usually requiring airway intervention.1 –7 In contrast to other emergencies, there are few evidence-based guidelines for its prevention and treatment. We have performed a narrative review which aims to describe the epidemiology, pathophysiology and management of TSA. (Source: International Journal of Obstetric Anesthesia)

Ecology of emergency care in lower-tier healthcare providers in Ghana: an empirical data-driven Bayesian network analytical approach
This study aimed to assess the emergency care preparedness and readiness of primary healthcare providers using a robust evaluation approach. A multicentre retrospective cross-sectional study was conducted on  460 healthcare facilities  using the standardised Health Facilities Emergency Preparedness Assessment Tool (HeFEPAT). Data were analysed via Bayesian Belief network. Emergency preparedness was associated with facility location, type, ownership, and in-charge personnel. Over 70% of facilities lacked specialised emergency/critical care personnel. Although 65% of in-charges reported protocol knowledge, only 7.8% c...

Mid-aortic syndrome with major mesenteric collaterals
A 91-year-old woman was presented to our emergency room with left rib fractures because of a fall. Plain computed tomography incidentally revealed abdominal vascular abnormalities; hence, the patient was referred to our department. At the age of 77 years, she was diagnosed with mid-aortic syndrome (MAS) with intermittent claudication. During that period, enhanced computed tomography angiography showed infrarenal discontinuity of the abdominal aorta and major collateral arteries from the superior to inferior mesenteric artery (A, B). Cancers, Vol. 16, Pages 1744: Evolution of Complexity of Palliative Care Needs and Patient Profiles According to the PALCOM Scale (Part Two): Pooled Analysis of the Cohorts for the Development and Validation of the PALCOM Scale in Advanced Cancer Patients
Conclusion: According to the PALCOM scale, more complex cases are associated with greater instability and use of hospital resources and lower survival. The data also confirm that the PALCOM scale is a consistent and useful tool for describing complexity profiles, targeting referrals to the EPC and managing the intensity of shared care. Intestinal Fatty Acid Binding Protein as a Predictor of Early Mesenteric Injury Preceding Clinical Presentation: A Case Report
Diagnosing non-occlusive mesenteric ischaemia (NOMI) in patients is complicated, due to poor signs and symptoms and non-specific laboratory tests, leading to high mortality. This case study presents a rare case of a patient who developed mesenteric ischaemia after an emergency thoracic endovascular aneurysm repair (TEVAR) for a type B aortic dissection (TBAD) and peri-operative cardiogenic shock. Study outcomes revealed that intestinal fatty acid binding protein (I-FABP) identified early mucosal damage 2 days before clinical presentation. (Source: EJVES Extra)

Enhancing Congenital Cardiac Postprocedural Care: A QR Code-Enabled Medical Bracelet Pilot Study
AbstractThe study aimed to evaluate the feasibility of using QR code-enabled medical bracelets for congenital heart disease (CHD) patients after hospital discharge to ensure quick communication of vital information to other medical personnel in emergency situations. A prospective study was conducted where QR code-enabled medical bracelets were given to families of postoperative pediatric cardiac patients. The QR code linked to a secure medical information sheet detailing the patient ’s cardiac history. Post-study surveys were completed by providers and families to assess their experiences with the bracelet. Of the 20 par...

Antibiotic route and outcomes for children hospitalized with pneumonia
AbstractBackgroundEmerging evidence suggests that initial oral and intravenous (IV) antibiotics have similar efficacy in pediatric community-acquired pneumonia (CAP), but further data are needed.ObjectiveWe determined the association between hospital-level initial oral antibiotic rates and outcomes in pediatric CAP.Designs, Settings and ParticipantsThis retrospective cohort study included children hospitalized with CAP at 43 hospitals in the Pediatric Health Information System (2016 –2022). Hospitals were grouped by whether initial antibiotics were given orally in a high, moderate, or low proportion of patients.Main Outc...

Poliomyelitis and COVID-19: a repeated history
J Water Health. 2024 Apr;22(4):717-720. doi: 10.2166/wh.2024.327. Epub 2024 Feb 22.ABSTRACTThe measurement of the enterovirus and the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in sewage water is relevant in the early detection of the introduction or disappearance of these viruses in the ecosystem. We evaluated the co-circulation of the enteroviruses and SARS-CoV-2 in 81 sewage water samples collected between September 2021 and April 2023 from different regions of north and southeast Romania, at the border with Ukraine. We used, for the molecular detection of the pathogens, the multiplex real-time polymer...

Maternity healthcare professionals experiences of supporting women in decision-making for labour and birth: a qualitative study
Conclusions We found that MHCPs are committed to SDM but need better support to deliver it. Structured processes including Core Information Sets, communication skills training and decision support aids may help to consistently deliver SDM in maternity care. (Source: BMJ Open)

Healthcare and economic cost burden of emergency medical services treated non-traumatic shock using a population-based cohort in Victoria, Australia
Conclusion The direct healthcare costs and indirect loss of productivity among patients with non-traumatic shock are high. Targeted public health measures that seek to reduce the incidence of shock and improve systems of care are needed to reduce the financial burden of this syndrome. (Source: BMJ Open)

Inequalities in birth before arrival at hospital in South West England: a multimethods study of neonatal hypothermia and emergency medical services call-handler advice
Conclusions Findings identified factors associated with BBA and neonatal hypothermia following BBA. Improvements to EMS call-handler advice could reduce the number of babies arriving at hospital hypothermic. (Source: BMJ Open)
