Association between opioid use disorder and palliative care: a cohort study using linked health administrative data in Ontario, Canada
CMAJ. 2024 Apr 28;196(16):E547-E557. doi: 10.1503/cmaj.231419.ABSTRACTBACKGROUND: People with opioid use disorder (OUD) are at risk of premature death and can benefit from palliative care. We sought to compare palliative care provision for decedents with and without OUD.METHODS: We conducted a cohort study using health administrative databases in Ontario, Canada, to identify people who died between July 1, 2015, and Dec. 31, 2021. The exposure was OUD, defined as having emergency department visits, hospital admissions, or pharmacologic treatments suggestive of OUD within 3 years of death. Our primary outcome was receipt of...

Medicinal chemistry strategies toward broad-spectrum antiviral agents to prevent next pandemics
Eur J Med Chem. 2024 Apr 25;271:116442. doi: 10.1016/j.ejmech.2024.116442. Online ahead of print.ABSTRACTThe pandemic and tremendous impact of severe acute respiratory syndrome coronavirus 2 alert us, despite great achievements in prevention and control of infectious diseases, we still lack universal and powerful antiviral strategies to rapidly respond to the potential threat of serious infectious disease. Various highly contagious and pathogenic viruses, as well as other unknown viruses may appear or reappear in human society at any time, causing a catastrophic epidemic. Developing broad-spectrum antiviral drugs with high...

Autoimmune encephalitis mediated by postvaccination and infection of SARS-CoV-2 in a patient with a narcolepsy type 1
CONCLUSION: The patient fulfilled the diagnostic criteria for autoimmune encephalitis with subacute onset. COVID-19 infection and vaccination could constitute a risk in a patient with narcolepsy as in this case and, could help to provide better understanding of the implication of immune-mediated processes in the pathophysiology of the diseases.PMID:38682764 | DOI:10.33588/rn.7809.2023306 (Source: Revista de Neurologia)

Appendectomy versus antibiotic treatment for acute appendicitis
CONCLUSIONS: Antibiotics may be associated with higher rates of unsuccessful treatment for 76 per 1000 people, although differences may not be clinically significant. It is uncertain if antibiotics increase length of hospital stay by half a day. Antibiotics may reduce wound infections. A third of the participants initially treated with antibiotics required subsequent appendectomy or two-thirds avoided surgery within one year, but the evidence is very uncertain. There were too few data from the included studies to comment on major complications.PMID:38682788 | DOI:10.1002/14651858.CD015038.pub2 (Source: Cochrane Database of Systematic Reviews)

"Emergency Room Incision and Drainage of Perirectal Abscess is Equivalent to Incision and Drainage in the Operating Room"
In this study, for patients presenting to the ED, bedside drainage was found to be an adequate management strategy to achieve complete drainage without a significant increase in the rate of complications when compared to operative drainage.PMID:38684322 | DOI:10.1177/00031348241248785 (Source: The American Surgeon)

Management of Injuries to the Parenchymal Abdominal Organs
Zentralbl Chir. 2024 Apr 29. doi: 10.1055/a-2301-7951. Online ahead of print.ABSTRACTThe most common organs affected by abdominal trauma are the spleen and the liver, often in combination. Pancreatic injuries are rare. In the case of blunt abdominal trauma, which is much more common, a clinical and laboratory examination as well as sonography should be performed. In the initial assessment, the circulatory situation must be screened. If there is haemodynamic instability and presentation of free fluid, an emergency laparotomy is indicated. If the situation is stable or stabilised and a pathological sonography is present, it ...

Evaluating the impact of  the Community Helpers Program on adolescents 12-18 years old in Edmonton, Canada
CONCLUSION: Findings show contextual effect of CHP; i.e., being potentially exposed to the program reduced the likelihood of anxiety- and depression-related visits. Costs of CHP implementation could be compared with the avoided costs to assess economic benefits of implementing CHP.PMID:38683287 | DOI:10.17269/s41997-024-00878-6 (Source: Canadian Journal of Public Health)

Resuscitation Update - What's New?
Anasthesiol Intensivmed Notfallmed Schmerzther. 2024 Apr;59(4):214-225. doi: 10.1055/a-2082-8685. Epub 2024 Apr 29.ABSTRACTPerfect, uninterrupted basic life support (BLS) is the key for successful cardiopulmonary resuscitation (CPR) in out-of-hospital cardiac arrest (OHCA). Time plays an important role in the treatment of OHCA. This applies both to the time until the start of BLS and the reduction of all pauses during resuscitation, especially chest compressions. In 2022, the rate of bystander-CPR showed an absolute increase of 4% compared to previous years. The bystander-CPR rate is currently above 50%. Compared to OHCA i...
