Evaluation of health-related quality of life in patients receiving outpatient parenteral antimicrobial therapy (OPAT) in a UK setting
CONCLUSIONS: OPAT is associated with improved patient-reported quality of life and facilitates early return to work or school. Nevertheless, it is crucial to closely monitor patients with a lower baseline quality of life to optimize their overall OPAT experience.PMID:38511820 | DOI:10.1080/14787210.2024.2334059 (Source: Expert Review of Anti-Infective Therapy)

Early switch from intravenous to oral antibiotic therapy in patients with cancer who have low-risk neutropenic sepsis: the EASI-SWITCH RCT
CONCLUSIONS: Non-inferiority for early oral switch could not be proven due to trial under-recruitment. The findings suggest this may be an acceptable treatment strategy for some patients who can adhere to such a treatment regimen and would prefer a potentially reduced duration of hospitalisation while accepting increased risk of treatment failure resulting in re-admission. Further research should explore tools for patient stratification for low-risk de-escalation or ambulatory pathways including use of biomarkers and/or point-of-care rapid microbiological testing as an adjunct to clinical decision-making tools. This could ...

Postpartum Necrotizing Myositis With Endometrial Prolapse
CONCLUSIONS: Although rare, postpartum necrotizing myositis should be considered in postpartum patients presenting with fever, abdominal pain, severe infection symptoms, and abnormal vaginal discharge. Culture and sensitivity testing are recommended to direct appropriate antibiotic therapy.PMID:38513235 | DOI:10.1097/AOG.0000000000005556 (Source: Obstetrics and Gynecology)

Risk of infections related to endovascular catheters and cardiac implantable devices in hemodialysis patients
J Vasc Access. 2024 Mar 20:11297298241240502. doi: 10.1177/11297298241240502. Online ahead of print.ABSTRACTPatients requiring dialysis are extremely vulnerable to infectious diseases. The high burden of comorbidities and weakened immune system due to uremia and previous immunosuppressive therapy expose the patient on dialysis to more infectious events than the general population. The infectious risk is further increased by the presence of endovascular catheters and implantable cardiologic devices. The former is generally placed as urgent vascular access for dialysis and in subjects requiring hemodialysis treatments withou...

Practical Guidance for Clinical Microbiology Laboratories: Microbiologic diagnosis of implant-associated infections
Clin Microbiol Rev. 2024 Mar 20:e0010423. doi: 10.1128/cmr.00104-23. Online ahead of print.ABSTRACTSUMMARYImplant-associated infections (IAIs) pose serious threats to patients and can be associated with significant morbidity and mortality. These infections may be difficult to diagnose due, in part, to biofilm formation on device surfaces, and because even when microbes are found, their clinical significance may be unclear. Despite recent advances in laboratory testing, IAIs remain a diagnostic challenge. From a therapeutic standpoint, many IAIs currently require device removal and prolonged courses of antimicrobial therapy...
