Patients Selection for Left Ventriculoplasty and Mitral Valve Surgery in Non-Ischemic Dilated Cardiomyopathy
The operative indication for dilated cardiomyopathy (DCM) is controversial. Subjects were 20 non-ischemic DCM patients. Left ventriculoplasty and mitral valve repair were conducted. Cardiac index derived from forward stroke volume significantly increased after surgery (from 1.8  ± 0.6 to 2.4 ± 0.6, P = .001). One-year survival rate was 58%. Slope in preload recruitable stroke work relationship (Mw) was an independent predictor for survival after surgery (HR, 0.936; 95%CI, 0.894–0.980; P = .005). Cut-off value of Mw for one-year survival was 42.

Rationale and design of the Transcatheter Aortic Valve Replacement to UNload the Left ventricle in patients with ADvanced heart failure (TAVR UNLOAD) trial
Publication date: Available online 26 August 2016 Source:American Heart Journal Author(s): Ernest Spitzer, Nicolas M. Van Mieghem, Philippe Pibarot, Rebecca T. Hahn, Susheel Kodali, Mathew S. Maurer, Tamim M. Nazif, Josep Rodés-Cabau, Jean-Michel Paradis, Arie-Pieter Kappetein, Ori Ben-Yehuda, Gerrit-Anne van Es, Faouzi Kallel, William N. Anderson, Jan Tijssen, Martin B. Leon Background Co-existence of moderate aortic stenosis (AS) in patients with heart failure with reduced ejection fraction (HFrEF) is not uncommon. Moderate AS increases afterload, while pharmacological reduction of afterload is a pillar of contemporary...

Alcohol and arrhythmias.
Authors: Pfeiffer D, Jurisch D, Neef M, Hagendorff A Abstract The effects of alcohol on induction of arrhythmias is dose-dependent, independent of preexisting cardiovascular diseases or heart failure and can affect otherwise healthy subjects. While the probability of atrial fibrillation increases with the alcohol dosage, events of sudden cardiac death are less frequent with low and moderate consumption but occur more often in heavy drinkers with alcoholic cardiomyopathy. Men are first affected at higher dosages of alcohol but women can suffer from arrhythmias at lower dosages. Thromboembolisms and ischemic stroke c...

The Effects of Levosimendan and Sodium Nitroprusside Combination on Left Ventricular Functions After Surgical Ventricular Reconstruction in Coronary Artery Bypass Grafting Patients.
CONCLUSION: This study showed that LS and SNP combination impressive increase in left ventricular systolic and diastolic functions including LVEF. The use of this combination achieved more less inotrophics and IABP requirement. We therefore suggest preoperative and peroperative levosimendan and SNP combination. PMID: 27583039 [PubMed]

Perioperative outcomes of patients with hypertrophic cardiomyopathy undergoing non-cardiac surgery
Conclusions Patients with HCM undergoing high-risk and intermediate-risk non-cardiac surgeries have a low perioperative event rate, at an experienced centre. However, they have a higher risk of composite events versus matched patients without HCM.

Abstract 042: Association Between Preexisting Valvular Heart Disease and Takotsubo Cardiomyopathy Among Hospitalized Patients in the United States Session Title: Poster Session I
Conclusion: In this large, nationwide study, preexisting VHD was associated with worse clinical outcomes in patients hospitalized for TTCM. Our results can guide future clinical decision-making regarding prompt risk factor identification for poor prognosis in TTCM patients. Future prospective studies are needed to further evaluate this association.

Myocardial Upregulation of Cathepsin D by Ischemic Heart Disease Promotes Autophagic Flux and Protects Against Cardiac Remodeling and Heart Failure Original Articles
Conclusions Myocardial CTSD upregulation induced by myocardial infarction protects against cardiac remodeling and malfunction, which is at least in part through promoting myocardial autophagic flux.

The Dx-AF study: a prospective, multicenter, randomized controlled trial comparing VDD-ICD to VVI-ICD in detecting sub-clinical atrial fibrillation in defibrillator patients
ConclusionsThe Dx-AF Study should provide significant scientific evidence and guidance to an adequate ICD system choice and early AF detection/management hence improve clinical outcomes in a large patient population.

Clinical Profile and Consequences of Atrial Fibrillation in Hypertrophic Cardiomyopathy.
Conclusions -Transient symptomatic episodes of AF, although relatively uncommon in HCM, are unpredictable in frequency and timing, amenable to effective contemporary treatments, and infrequently progress to permanent AF. AF is not a major contributor to heart failure morbidity, nor a cause of arrhythmic sudden death, and when treated is associated with low-disease-related mortality, no different than for patients without AF. AF is an uncommon primary cause of death in HCM virtually limited to embolic stroke, supporting a low threshold for initiating anticoagulation therapy. Morphosis of Multi-Layered Mitochondria in the Myocardium of a Patient With Mitochondrial Cardiomyopathy Revealed by Electron Microscopy
A 35-year-old woman, who had been diagnosed as mitochondrial myopathy, encephalopathy, lactic acidosis, stroke-like episodes (MELAS) 10 years before, was referred for the evaluation of mitochondrial cardiomyopathy at the first onset of a stroke-like episode. The echocardiogram showed nearly normal systolic function. HE staining of the tissue obtained by endomyocardial biopsy showed marked vacuolization especially at the perinuclear region. Electron microscopy demonstrated damaged myofibril and severe abnormalities of the mitochondria such as giant mitochondrion, crescent-like mitochondria, and multi-layered mitochondria as...

Clinical Correlates and Prognostic Value of Elevated Right Atrial Pressure in Patients With Hypertrophic Cardiomyopathy.
CONCLUSIONS: Elevated RAP in patients with HCM is associated with left-sided heart failure and is an independent predictor of all-cause mortality and new-onset AF. PMID: 29526913 [PubMed - as supplied by publisher]

Invasive Hemodynamics and Rejection Rates in Patients With Cardiac Sarcoidosis After Heart Transplantation
ConclusionsPatients with CS have similar post-transplant hemodynamics as patients without CS, without evidence of right ventricular dysfunction or pulmonary hypertension. Neither significant rejection nor recurrence of sarcoid in the allograft was observed in this cohort of patients with CS. Survival is similar between patients with CS and those without CS. Heart transplant is a viable strategy in selected patients with CS with excellent outcomes.RésuméIntroductionLa transplantation cardiaque orthotopique (TCO) est de plus en plus utilisée lors d’insuffisance cardiaque terminale liée à une sarcoïdose cardiaque (SC)...

Management of patients with newly-diagnosed atrial fibrillation: Insights from the BALKAN-AF survey
ConclusionsIn BALKAN-AF survey, patients with newly-diagnosed AF were more often hospitalized, less often received OAC and were more likely to receive SAPT alone. The use of OAC for stroke prevention has not been driven by the individual patient stroke risk.

Extent of Late Gadolinium Enhancement Predicts Thromboembolic Events in Patients With Hypertrophic Cardiomyopathy.
CONCLUSIONS: LGE extent (>14.4%) is an independent predictor for thromboembolic complications in patients with HCM and might therefore be considered as an important risk marker. The risk for thromboembolic events is significantly elevated if accompanied by AF. PMID: 32238665 [PubMed - as supplied by publisher]
