Patient characteristics and outcomes of acute myocardial infarction presenting without ischemic pain: Insights from the Atherosclerosis Risk in Communities Study
CONCLUSIONS: Future research should focus on differences in treatment delay across ischemic pain presentations rather than sex differences in acute coronary syndrome presentation.PMID:36713888 | PMC:PMC9879363 | DOI:10.1016/j.ahjo.2022.100239

The epidemiology, mechanisms, diagnosis and treatment of cardiovascular disease in adult patients with HIV
This article discusses the complex interactions involving HIV infection, the novel and traditional risk factors for CVD, and the antiretroviral HIV therapies which can contribute to CVD in HIV-infected people. In addition, the treatment of HIV+ patients with acute myocardial infarction, stroke, and cardiomyopathy/heart failure are discussed. Current recommended ART and their major side effects are summarized in table format. All medical personnel must be aware of the increasing incidence of CVD on the morbidity and mortality in HIV infected patients and must be watchful for the presence of CVD in their patients with HIV.PM...

Incident cardiovascular events and imaging phenotypes in UK Biobank participants with past cancer
Conclusions Cancer history is linked to increased risk of incident CVDs and adverse cardiac remodelling independent of shared vascular risk factors.

Risk assessment in hypertrophic cardiomyopathy: contemporary guidelines hampered by insufficient evidence
If it were not for the great variability among individuals, medicine might as well be a science and not an art. (Sir William Osler, 1892) The sudden unexpected death of an individual at any age has a profound effect on the family and the wider community and this is particularly significant when it affects a young person. Hypertrophic cardiomyopathy remains one of the common causes of sudden cardiac death (SCD) in young individuals.1 In a cohort of 744 patients with hypertrophic cardiomyopathy from the USA and Italy, where 86 (12%) died over an 8 (SD±7) year period of follow-up, three main modes of death were identif...

Treating Anemia in Older Adults With Heart Failure With a Preserved Ejection Fraction With Epoetin Alfa: Single-blind Randomized Clinical Trial of Safety and Efficacy Original Articles
Conclusions— Administration of epoetin alfa to older adult patients with heart failure and a preserved ejection fraction compared with placebo did not change left ventricular end-diastolic volume and left ventricular mass nor did it improve submaximal exercise capacity or quality of life. Clinical Trial Registration— URL: http://www.clinicaltrials.gov. Unique identifier: NCT00286182.

Sympathetic Neural and Hemodynamic Responses to Upright Tilt in Patients With Pulsatile and Nonpulsatile Left Ventricular Assist Devices Original Articles
Conclusions— Heart failure patients with continuous, nonpulsatile LVADs have marked sympathetic activation, which is likely due, at least in part, to baroreceptor unloading. We speculate that such chronic sympathetic activation may contribute to, or worsen end-organ diseases, and reduce the possibility of ventricular recovery. Strategies to provide some degree of arterial pulsatility, even in continuous flow LVADs may be necessary to achieve optimal outcomes in these patients.

Four-dimensional blood flow-specific markers of LV dysfunction in dilated cardiomyopathy
Conclusion Despite equivalent LVSVs, HF patients with mild LV remodelling demonstrate altered diastolic flow routes through the LV and impaired preservation of inflow KE at pre-systole compared with healthy subjects. These unique flow-specific changes in the flow route and energetics are detectable despite clinical compensation, and may prove useful as subclinical markers of LV dysfunction.

Tissue Doppler imaging and prognosis in asymptomatic or mildly symptomatic patients with hypertrophic cardiomyopathy
Conclusion Assessment of the diastolic function by TDI is useful for risk stratification in HCM patients with no or mild symptoms. TDI measurements should be incorporated into the clinical management of HCM.

Evaluation of epicardial and intra-myocardial fat in a patient with mitochondrial cardiomyopathy
Mitochondrial cardiomyopathy is one of the main manifestations in patients with primary mitochondrial respiratory chain disease characterized by mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes (MELAS) and carries a similar prognosis. The phenotype frequently includes hypertrophic cardiomyopathy; dilated cardiomyopathy and left ventricular (LV) non-compaction are rarely present . Cardiac magnetic resonance imaging (MRI) with gadolinium-based contrast agents has been widely used for evaluating the morphology and function of the left ventricle and injured myocardial tissue . Cine imaging with a stan...

Predictors of Long Term Outcomes in Symptomatic Hypertrophic Obstructive Cardiomyopathy Patients Undergoing Surgical Relief of Left Ventricular Outflow Tract Obstruction.
CONCLUSIONS: Symptomatic adult HCM patients undergoing surgery for relief of LVOT obstruction have low event rate during long-term follow up; with worse outcomes predicted by increasing age and presence of residual AF during follow up. PMID: 23770748 [PubMed - as supplied by publisher]

Stress cardiomyopathy (tako-tsubo) triggered by nervous system diseases: A systematic review of the reported cases
Conclusions: TRLVD is a complication of neurological diseases, in particular in female patients in post-menopausal phase. The predilection for neurological damage at or close to the insular cortex highlights the pivotal role of sympathetic over-activation. Many other similarities with tako-tsubo support the inclusion in a single SCM category.

Anticoagulation in Patients with Dilated Cardiomyopathy, Low Ejection Fraction, and Sinus Rhythm: Back to the Drawing Board
Summary Heart failure patients present an important thrombo‐embolic risk, including symptomatic or silent peripheral arterial embolism, pulmonary embolism, and stroke. Patients in sinus rhythm who have concomitant depressed (<35%) left ventricular ejection fraction have a 4% rate of embolic events. Several prospective randomized trials of anticoagulation in this group of patients were either underpowered or had a short period of follow‐up. Even though in two studies warfarin had a slight advantage over aspirin (in the WATCH and WARCEF trials), it was at the cost of an increased risk in major hemorrhage. To decrease ...
