CMR to distinguish Takotsubo Cardiomyopathy from Myocardial Infarction in Acute Course of Ischemic Stroke in a Male Patient
So far, in a clinical emergency subset, there is no optimal diagnostic tool to distinguish acute myocardial infarction (AMI) from Takotsubo cardiomyopathy (TTC). Since TTC occurs more likely in postmenopausal woman [1], when appeared in male patient, the differential diagnosis becomes far more challenging. Despite similarities in the clinical management, long-term therapy differs significantly and the fast distinction between both entities, especially in cases initially complicated by stroke remains crucial.

Embolic Stroke in Cardiomyopathy
Despite advances in the treatment of patients with heart failure, mortality is still substantial. Part of this mortality is explained by cardioembolic stroke. Patients with heart failure are predisposed to developing cardioembolic strokes owing to abnormalities in Virchow’s triad (endothelial function, relative hypercoagulable state, and static blood flow). Several randomized controlled trials have addressed whether patients with heart failure benefit from anticoagulation. Overall, the results suggest the risk of bleeding with anticoagulation outweighs any small benefit conferred by anticoagulation.

Stroke and Bleeding Risks in NOAC- and Warfarin-Treated Patients With Hypertrophic Cardiomyopathy and Atrial Fibrillation
Hypertrophic cardiomyopathy (HCM) affects>600,000 patients in the United States, and approximately 1 in 5 of them have atrial fibrillation (AF) (1). The incidence of stroke in patients with HCM is markedly increased when complicated by AF, with an annualized risk of nearly 4% (2). AF is an indication for warfarin in nearly all patients with HCM (3,4). Non–vitamin K antagonist oral anticoagulants (NOACs) may be reasonable alternatives to warfarin, but there are no explicit data to support their use (3,4).
