Coronary artery disease in renal transplant recipients: an angiographic study.
Authors: Paizis IA, Mantzouratou PD, Tzanis GS, Melexopoulou CA, Darema MN, Boletis JN, Barbetseas JD Abstract BACKGROUND: Cardiovascular disease is the leading cause of mortality in renal transplant recipients (RT). Coronary artery disease (CAD) in such patients is poorly studied. METHODS: Between 2012 and 2017, 50 patients with a functioning renal graft (>6 months) were subjected to coronary angiography in our institution. They were matched (for age, gender, diabetes and indication for angiography) with 50 patients with end-stage renal disease (ESRD) undergoing chronic dialysis and 50 patients with normal ...

"Hearts that strain": Distinguishing athlete's heart from hypertensive disease in the echo lab and beyond. Hellenic J Cardiol. 2018 Jun 28;: Authors: Tsioufis C Abstract Individuals with hypertension that engage in regular exercise comprise a special patient group that needs a careful approach to differentiate hypertensive cardiac damage from physiologic cardiac adaptations. Echocardiography is the diagnostic modality of choice in such cases. Hypertensive left ventricular hypertrophyis expected to resemble isometric exercise-associated hypertrophy in some but not all cases. On the other side,...

The importance of left heart disease as a cause of pulmonary hypertension in COPD.
Authors: Nashat H, Gatzoulis MA Abstract Chronic obstructive pulmonary disease is a common respiratory condition that presents in varying degrees of severity and can be complicated by further co-morbidities. Up to a third can also have pulmonary hypertension, which is an important risk factor associated with an increase in morbidity and mortality. The etiology of the pulmonary hypertension contributes. Correctly considering and identifying the type of pulmonary hypertension will influence management and outcome in patients with COPD. Do we need biomarkers for diabetics progressing to heart failure?
Authors: Dimitriadis K, Tsioufis C, Tousoulis D Abstract There is a cardiovascular unfavorable duet consisting of arterial hypertension and diabetes that is related with increased risk of heart failure. The latter is highly prevalent in this setting of dysregulation of glucose metabolism and high hemodynamic load and identifying predictors of incidence of heart failure with preserved ejection fraction (HFpEF) is a research and clinical need. Towards this end the variability of glucose regulation represent a novel dynamic way to comprehend and study the impact of glycemia. The results of the published study highligh...

Mechanisms of testosterone deficiency-related endothelial dysfunction.
Authors: Antonopoulos AS, Antoniades C Abstract Evidence from clinical studies suggests that patients with low testosterone levels are at increased cardiovascular disease risk. Even though the exact mechanisms remain poorly understood, low plasma testosterone is associated with a pro-atherogenic lipid profile, insulin resistance and increased levels of pro-inflammatory mediators, and vascular dysfunction which is typically observed in patients with hypogonadism. Furthermore, recent evidence suggests that testosterone deficiency has also direct adverse effects on endothelium and nitric oxide bioavailability. Observa...

Current perspectives on the diagnosis and management of dilated cardiomyopathy Beyond heart failure: a Cardiomyopathy Clinic Doctor's point of view.
Authors: Bakalakos A, Ritsatos K, Anastasakis A Abstract Left ventricular enlargement and dysfunction are fundamental components of dilated cardiomyopathy. Dilated cardiomyopathy is a major cause of heart failure and cardiac transplantation. A wide variety of etiologies underlie acquired and familial DCM. Familial disease is reported in 20% to 35% of cases. A genetic substrate is recognized in at least 30% of familial cases. A recently proposed scheme, defines DCM as a continuum of subclinical and clinical phenotypes. The evolution of classification systems permitted use of effective treatment strategies in disorde...

Natriuretic peptides in acute coronary syndromes: a new role to predict recurrent ischemic event-related mortality?
Authors: Bistola V, Polyzogopoulou E, Parissis J Abstract Risk stratification in acute coronary syndromes (ACS) have been based previously on severity of acute clinical presentation, presence of cardiovascular comorbidities and abnormalities of ischemia-specific biomarkers most importantly troponins. Natriuretic peptides (NPs), which become elevated upon increased myocardial wall stress, are established diagnostic and prognostic biomarkers in patients with heart failure. Recently, their prognostic potential in ACS has been reported, specifically as predictors of future new-onset heart failure or left ventricular sy...
