Recurrent stroke and myocardial infarction in a young woman with severe osteoporosis and hyperparathyroidism
We have taken great interest in the recently published article entitled “ Effect of calcium or vitamin D supplementation on vascular outcomes: A meta-analysis of randomized controlled trial” . Likewise, since parathormone (PTH) is a vasoactive peptide, we, herein, wish to draw attention to the fact that primary hyperparathyroidism (pHPT) can –as well– be in association with increased risk of cardiovascular diseases (i.e. myocardial infarction (MI), heart failure, thrombosis or cardioembolism, hypertension), stroke and osteoporosis . In this regard, we exemplify the clinical scenario of one of our pertinent patients.

Relative Contributions of Sympathetic, Cholinergic, and Myogenic Mechanisms to Cerebral Autoregulation Clinical Sciences
Conclusions— Our results suggest that myogenic effects occur outside the active region of autoregulation, whereas neurogenic influences are largely responsible for cerebral blood flow control within it. However, our model of cerebral autoregulation left 38% of the cerebral pressure–flow relationship unexplained, suggesting that there are other physiological mechanisms that contribute to cerebral autoregulation.

ATP-Evoked Sustained Vasoconstrictions Mediated by Heteromeric P2X1/4 Receptors in Cerebral Arteries Basic Sciences
Conclusions— This study reveals for the first time that apart from rapidly desensitizing homomeric P2X1Rs, heteromeric P2X1/4Rs contribute to the sustained component of the purinergic-mediated vasoconstriction in RMCA. Our study, therefore, identifies possible novel targets for therapeutical intervention in cerebral circulation.

Impact of Heme and Heme Degradation Products on Vascular Diameter in Mouse Visual Cortex Stroke
Conclusions We identified individual components of the oxidative bilirubin degradation that led to vasoconstriction of cerebral arterioles. The vasoconstrictive effect of Z-BOX A and Z-BOX B was mediated by BKCa channel activity that might represent a signaling pathway in the occurrence of delayed cerebral vasospasm in subarachnoid hemorrhage patients.

Amlodipine, a calcium channel blocker, is considered to have better tolerance and effectiveness than other classes of antihypertensive treatments. Limited studies have examined the cost-effectiveness of Amlodipine in the Philippines. The purpose of this study was to compare the costs and effectiveness associated with Amlodipine and Angiotensin II Receptor Blockers (Valsartan) in preventing stroke and myocardial infarction (MI) among Filipino hypertension patients.
