Cerebral small vessel disease caused by a novel heterozygous mutation in HTRA1
Cerebral small vessel disease (SVD) is a mostly sporadic disorder leading to stroke and cognitive impairment. In very rare cases homozygous mutations of the HTRA1 gene (high temperature requirement protease A1) are observed and considered to promote cerebral autosomal recessive arteriopathy with subcortical infarcts and leukoencephalopathy (CARASIL) [1,2]. HTRA1 protease activity is involved in TGF- β signaling [3]. Here, we report a case of a patient with cerebral SVD initially misdiagnosed with multiple sclerosis (MS) caused by a novel heterozygous mutation in the HTRA1 gene.

Conclusions The present study allowed the identification of brain areas activated during the performance of the SDMT in healthy subjects, and therefore it will help understanding the differences in brain activation by this task in clinical populations. Does restricted diffusion in the splenium indicate an acute infarct?
ConclusionNot every diffuse restriction observed on MRI indicates an ischemic stroke. Although radiologic images of the splenium may suggest acute ischemic infarction, the actual cause may be another pathology. Therefore, the symptoms and aetiologies of patients with splenium lesions should be considered and investigated from a wide range of perspectives.

Cocaine-induced multifocal leukoencephalopathy mimicking Balo's concentric sclerosis: A 2-year follow-up with serial imaging of a single patient
We described a patient with cocaine abuse who presented with Balo's type acute multifocal leukoencephalopathy. Magnetic Resonance Imaging (MRI) of the brain showed onion like patchy concentric ring enhancement on T1-weighted MRI with gadolinium. Balo's Concentric Sclerosis like radiological findings related to cocaine has not been reported. Levamisole is now frequently used as an ingredient in cocaine and may cause leukoencephalopathy. Common comorbidities and survival in MS: Risk for stroke, type 1 diabetes and infections
Conclusions Given the high risk for stroke in this MS population and the observed complexity among the coincident common risk factors for circulatory diseases, the high risk for type 1 diabetes and common infections raise a need to recognize patients at risk with these conditions and with the other known risk factors such as metabolic syndrome and smoking. The survival disadvantage related to circulatory diseases observed in general population is true also in MS and should be recognized to reduce the burden of disease and premature mortality in MS. : In "Evaluating the safety of β-interferons in MS: A series of nested case-control studies," de Jong et al. found that exposure to β-interferons was associated with a 1.8-fold increased risk of stroke. Commenting on the study, Dr. Jongen shares his own prospective study on adverse events in 284 patients with multiple sclerosis treated with intramuscular interferon-β-1a: one patient died from a stroke sinus thrombosis 22 months after start of treatment.
