Cellular strategies to induce immune tolerance after liver transplantation: Clinical perspectives
World J Gastroenterol. 2024 Apr 7;30(13):1791-1800. doi: 10.3748/wjg.v30.i13.1791.ABSTRACTLiver transplantation (LT) has become the most efficient treatment for pediatric and adult end-stage liver disease and the survival time after transplantation is becoming longer due to the development of surgical techniques and perioperative management. However, long-term side-effects of immunosuppressants, like infection, metabolic disorders and malignant tumor are gaining more attention. Immune tolerance is the status in which LT recipients no longer need to take any immunosuppressants, but the liver function and intrahepatic histol...

Cancers, Vol. 16, Pages 1645: Clinical Prediction Models for Prognosis of Colorectal Liver Metastases: A Comprehensive Review of Regression-Based and Machine Learning Models
Tsoulfas Colorectal liver metastasis (CRLM) is a disease entity that warrants special attention due to its high frequency and potential curability. Identification of “high-risk” patients is increasingly popular for risk stratification and personalization of the management pathway. Traditional regression-based methods have been used to derive prediction models for these patients, and lately, focus has shifted to artificial intelligence-based models, with employment of variable supervised and unsupervised techniques. Multiple endpoints, like overall survival (OS), disease-free survival (DFS) ...

Comprehensive study of the interplay between immunological and metabolic factors in hepatic steatosis
Int Immunopharmacol. 2024 Apr 23;133:112091. doi: 10.1016/j.intimp.2024.112091. Online ahead of print.ABSTRACTThe pathophysiology of hepatic steatosis is thoroughly reviewed in this comprehensive report, with particular attention to the complex interactions between inflammatory pathways, insulin resistance, lipid metabolism, metabolic dysregulation, and immunological responses in the liver including non-alcoholic fatty liver disease (NAFLD), non-alcoholic steatohepatitis (NASH), and hepatocellular carcinoma (HCC). The study highlights the role of immune cell regulation in disease progression and explores the potential of i...

Psychometric Analysis for a Standardized Approach When Evaluating Liver Transplant Candidates with Acute Alcohol-Related Hepatitis
AbstractPurpose of ReviewTo summarize the role of psychometric instruments in the evaluation and management of patients with acute alcoholic hepatitis (AAH) evaluated for liver transplantation (LT).Recent FindingsPsychometrics can inform the care of patients with AAH in the transplantation setting by enhancing diagnostic accuracy and allowing a more precise understanding of the patient ’s mental health status. The predictive value of the psychometric scale in LT candidates with AAH remains low and controversial.SummaryCombining psychometrics with clinical judgment and other diagnostic tools ensures a more comprehensive a...

Longer Term Follow-up on Acuity Circle Allocation Strategies in Liver Transplantation
AbstractPurpose of ReviewThis review summarizes longer term follow-up of acuity circle –based distribution for livers, which was implemented on February 4, 2020.Recent FindingsAfter 2  years of policy implementation, the likelihood of transplant increased, while removals for death or being too sick decreased. The median transplant score was unchanged, and the variance in the median MELD at transplant decreased for OPTN regions, DSA, and state. Concurrently, median distance from donor to transplant hospital and cold ischemia times increased. A slight increase in liver non-use rate and decrease in liver utilization rate h...

Primary Care Utilization, Preventative Screening, and Control of Metabolic Syndrome in Metabolic Dysfunction-Associated Steatohepatitis Liver Transplant Recipients
CONCLUSION: More research is needed to investigate barriers to screening and management of MetS conditions in this high-risk patient population in the primary care setting as well as to optimize post-transplant care coordination.PMID:38650519 | DOI:10.1177/21501319241247974 (Source: Primary Care)

Decorin suppresses stemness and migration potential of malignant peripheral nerve sheath tumor through inhibiting epidermal growth factor receptor signaling
Biochim Biophys Acta Mol Basis Dis. 2024 Apr 21:167181. doi: 10.1016/j.bbadis.2024.167181. Online ahead of print.ABSTRACTCancer stem cells (CSCs) play pivotal roles in the growth, invasion, metastasis, chemo-resistance in malignant peripheral nerve sheath tumor (MPNST). The current characterization of CSCs in MPNST is not complete. Decorin is a critical regulator of microenvironment, but its expression and function in CSCs of MPNST has not been studied. In the current study, Decorin levels and its relationship with lung and liver metastasis were determined in clinical specimens. Decorin expression in CD133-positive or CD44...

Cardiopulmonary Resuscitation Outcomes and Trainee Perception of Code Status Discussions in Patients with Cirrhosis
ConclusionsWe identified significant trainee uncertainty about outcomes in patients with cirrhosis. These deficits improved after an educational intervention and gave providers more confidence in holding informed code status conversations with patients with cirrhosis, a population that faces barriers to adequate code discussions. Hepatic and Extrahepatic Characteristics of Autoimmune Hepatitis: A 23-year Hospital-Based Cohort Study
ConclusionIn an Asian country endemic for viral hepatitis, the female-to-male and baseline cirrhosis rates of AIH patients were lower than expected, while over 60% of the patients eventually developed cirrhosis. The high posttherapy relapse rate warrants cautious monitoring, particularly for patients with high baseline Alk-p levels. Baseline age, cirrhosis status and ANA titers are crucial for outcomes. Single is not combined: the role of Co and Ni bioavailability on toxicity mechanisms in liver and brain cells
In this study, we focused on the treatment of liver carcinoma (HepG2) and astrocytoma (CCF-STTG1) cells as a model for the target sites of these two metals. We investigated their effects in single and combined exposure on cell survival, cell death mechanisms, bioavailability, and the induction of oxidative stress. The combination of CoCl2 and NiCl2 resulted in higher Co levels with subsequent decreased amount of Ni compared to the individual treatment. Only CoCl2 and the combination of both metals led to RONS induction and increased GSSG formation, while apoptosis and necrosis seem to be involved in the cell death mechanis...

Gelatin nanocarriers assembled by a self-immolative cross-linker for targeted cancer therapy
Int J Biol Macromol. 2024 Apr 20;268(Pt 1):131722. doi: 10.1016/j.ijbiomac.2024.131722. Online ahead of print.ABSTRACTWith a number of outstanding properties, gelatin is an ideal candidate for assembling nanoplatforms in biomedical applications. Generally, gelatin nanocarriers are cross-linked by aldehydes to improve their stability in water solution. However, aldehydes could cause multiple toxicities and their cross-linking products are uncontrollable. Here, we first used a self-immolative cross-linker to assemble gelatin nanocarriers for the controlled release of drugs and targeted cancer therapy. The cross-linker contai...
