Approval of Mycophenolate Mofetil for Prophylaxis of Organ Rejection in Pediatric Recipients of Heart or Liver Transplants: A Regulatory Perspective
Clin Pharmacol Ther. 2024 May 2. doi: 10.1002/cpt.3288. Online ahead of print.ABSTRACTOn June 6, 2022, the FDA expanded the indications for mycophenolate mofetil (MMF) to include the prophylaxis of organ rejection in combination with other immunosuppressants in pediatric recipients of allogeneic heart or liver transplants aged 3 months and older. The approved oral dosing regimen for these patients was a starting dose of 600 mg/m2 with titration up to a maximum of 900 mg/m2 twice daily. Data to support efficacy in pediatric patients were derived from established pharmacokinetic (PK) relationships across approved populations...

Augmented reality improving intraoperative navigation in minimally invasive liver surgery: an interplay between 3D reconstruction and indocyanine green
Updates Surg. 2024 May 2. doi: 10.1007/s13304-024-01857-9. Online ahead of print.ABSTRACTTechnology have helped surgeons to increase MILS feasibility, so that currently liver surgery evolution is strongly based on technological advances and the same trend is expected even further soon. Aim of the present technical report is to provide insights regarding the possible interplay between 3D reconstructions based on augmented reality and intraoperative navigation by indocyanine green fluorescence. Augmented reality methods based on reconstructions created through artificial intelligence interact synergistically. The better the ...

Deceased Donor Liver Transplantation With Hepatic Artery Aneurysm
Exp Clin Transplant. 2024 Mar;22(3):242-243. doi: 10.6002/ect.2024.0025.ABSTRACTFew case reports have documented the long-term outcomes of liver donor aneurysms, illustrating the apprehension of transplant surgeons about using these grafts. However, the presence of an aneurysm in the donor liver should not be an absolute contraindication for its use. As shown in our described patient, such grafts have the potential to achieve good results.PMID:38695594 | DOI:10.6002/ect.2024.0025 (Source: Experimental and Clinical Transplantation : official journal of the Middle East Society for Organ Transplantation)

Current State of Liver Transplantation for Cholangiocarcinoma: A Comprehensive Literature Review
Exp Clin Transplant. 2024 Mar;22(3):167-179. doi: 10.6002/ect.2023.0343.ABSTRACTCholangiocarcinoma is the second most common primary hepatic neoplasm, accounting for 10% to 20% of primary liver tumors and 3% of all gastrointestinal neoplasms. The 3 anatomic types (intrahepatic, perihilar, and distal) have distinct epidemiologies, etiopathogenesis, and clinical outcomes. Surgical resection remains the current standard of treatment, but outcomes remain poor. With the continued expansion of liver transplant programs, use of liver transplant for malignant indications has also increased, with reports of encouraging outcomes. Ho...

External validation of population pharmacokinetic models of tacrolimus in Thai adult liver transplant recipients
ConclusionBayesian forecasting is more accurate and precise than other testing methods for predicting drug concentrations. However, none of the evaluated models provides satisfactory predictive performance for generalization to Thai liver transplant patients. This underscores the need for future research to develop population PK models tailored to the Thai population. Such efforts should consider the inclusion of nonlinear pharmacokinetics and region-specific factors, including genetic variability, to improve model accuracy and applicability. Reply to the Letter to the Editor on “Prolactin is a Key Factor for Nonalcoholic Fatty Liver Disease in Obese Children”
Horm Metab Res DOI: 10.1055/a-2301-3272Dear Editor,The authors thank the writers 1 of the letter for reviewing and assessing our article 2, “Prolactin is a Key Factor for Nonalcoholic Fatty Liver Disease in Obese Children”. Overall, we agree that possible selection bias must be considered. However, in clinical studies, we found that hyperprolactinemia is mainly caused by pathological and idiopathic hyperprolactinemia. The former includes hypothalamic pituitary lesions (pituitary prolactinoma and empty sella syndrome, etc.), systemic diseases (hypothyroidism, e...

Integrated transcriptomics and histopathology approach identifies a subset of rejected donor livers with potential suitability for transplantation
Liver transplantation is an effective treatment for liver failure. There is a large unmet demand, even as not all donated livers are transplanted. The clinical selection criteria for donor livers based on hist... (Source: BMC Genomics)

< em > Klebsiella pneumoniae < /em > infections after liver transplantation: Drug resistance and distribution of pathogens, risk factors, and influence on outcomes
CONCLUSION: KPIs occur frequently and rapidly after LT. Risk factors include female sex, pre-LT diabetes, increased post-LT ALT levels, and urethral catheter duration. Prospective study of hepatitis B and D epidemiology and risk factors in Romania: A 10-year update
CONCLUSION: This study shows that HDV infection among those with HBV remains endemic in Romania and updates our understanding of HDV epidemiology and associated risk factors. It emphasizes the need for systematic screening for HDV infection and collaborative initiatives for controlling and preventing HBV and HDV infection.PMID:38689751 | PMC:PMC11056896 | DOI:10.4254/wjh.v16.i4.640 (Source: World Journal of Hepatology)

Relative carcinogenicity of tacrolimus < em > vs < /em > mycophenolate after solid organ transplantation and its implications for liver transplant care
CONCLUSION: The contrasting carcinogenic risk profiles of tacrolimus and MPA demonstrated in previous experimental studies, and its application in solid organ transplantation, is yet to be confirmed in clinical studies. Thus, the optimal choice of immunosuppression drug to use as maintenance monotherapy in LT recipients is not supported by a strong evidence base and remains unclear.PMID:38689747 | PMC:PMC11056899 | DOI:10.4254/wjh.v16.i4.650 (Source: World Journal of Hepatology)

Quantitative hepatitis B core antibody and quantitative hepatitis B surface antigen: Novel viral biomarkers for chronic hepatitis B management
World J Hepatol. 2024 Apr 27;16(4):550-565. doi: 10.4254/wjh.v16.i4.550.ABSTRACTThe management of hepatitis B virus (HBV) infection now involves regular and appropriate monitoring of viral activity, disease progression, and treatment response. Traditional HBV infection biomarkers are limited in their ability to predict clinical outcomes or therapeutic effectiveness. Quantitation of HBV core antibodies (qAnti-HBc) is a novel non-invasive biomarker that may help with a variety of diagnostic issues. It was shown to correlate strongly with infection stages, hepatic inflammation and fibrosis, chronic infection exacerbations, an...
