Childhood community-acquired pneumonia
Conclusion: New insights into the aetiology and treatment of CAP in children in recent years have influenced management and are the focus of this review. In addition to reducing diagnostic uncertainty, there is an urgent need to reduce antibiotic overuse and antimicrobial resistance in children with CAP.What is Known:• Conjugate vaccines against Streptococcus pneumoniae and Haemophilus influenzae type b have shifted the epidemiology of childhood CAP to predominantly viral pathogens and Mycoplasma pneumoniae.• Clinical, laboratory, and radiological criteria cannot reliably distinguish between bacterial and viral aetiolo...

7-Year (2015 –21) longitudinal surveillance of lefamulin in vitro activity against bacterial pathogens collected worldwide from patients with respiratory tract infections including pneumonia and characterization of resistance mechanisms
ConclusionsBased on the coverage of the most important CABP pathogens and lacking cross-resistance, lefamulin may represent a valuable empirical treatment option for ambulatory and hospitalized patients with CABP, particularly in settings with high prevalence of resistance. Clinical utility of metagenomic next-generation sequencing in pathogen detection for lower respiratory tract infections and impact on clinical outcomes in southernmost China
ConclusionsThe application of mNGS can benefit clinics in pathogen identification and antimicrobial treatment stewardship. Physicians should be alert to some emerging uncommon pathogens, including Chlamydia Psittaci, Nocardia otitidiscaviarum, and rare NTM. (Source: Frontiers in cellular and infection microbiology)

Teclistamab impairs humoral immunity in heavily pretreated MM patients: importance of immunoglobulin supplementation
In conclusion, our data show severe defects in humoral immunity induced by teclistamab, the impact of which can be mitigated by the use of immunoglobulin supplementation. NCT04557098.PMID:38052042 | DOI:10.1182/bloodadvances.2023011658 (Source: Adv Data)

Vaccination coverage of children with rheumatic diseases compared with healthy controls: a retrospective case-control study
CONCLUSION: Vaccination coverage of PedRD patients has been shown to lag behind the routine vaccination schedule (71.4%). In addition to new recommendations, electronic health system records for vaccination may be appropriate for the follow-up of these patients, and the addition of reminder alerts may be useful to reduce the rate of missed vaccinations.PMID:37997766 | DOI:10.1080/00325481.2023.2287988 (Source: Postgraduate Medicine)

Plastic bronchitis caused by Haemophilus influenzae
Plastic bronchitis is characterized by sputum obstructing the bronchus and causing atelectasis. Bronchoscopic removal of the clogged bronchial cast is typically performed, but small-diameter bronchoscopes with narrow suction ports cannot achieve adequate suction. Suction using a bronchoscope as a guide sheath may be effective for treating plastic bronchitis in children. National Danish surveillance of invasive clinical Haemophilus influenzae isolates and their resistance profile
ConclusionThe overall incidence of H. influenzae in Denmark returned to stable levels after the COVID-19 epidemic, with NTHi strains dominating. The COVID-19 preventive interventions led to a major reduction in incidence. A significant negative correlation between the incidence of H. influenzae and temperature was observed. The study revealed an overall genetic beta-lactam resistance rate of 26.3%, and the concordance between genotypic and phenotypic beta-lactam resistance was high (98.2%). Antibiotics for Acute Sinusitis in Children —Reply
In Reply Mr Meng and colleagues express concern that testing for H influenzae, S pneumoniae, and M catarrhalis, which are frequently found to colonize the nasal passages of well children, might lead to increased antibiotic use. However, this will occur only if the results from our trial are applied indiscriminately to a population that was not included in our study. (Source: JAMA)

Bacterial Conjunctivitis: Current Aspects of Diagnosis and Therapy
Klin Monbl Augenheilkd. 2023 Nov 17. doi: 10.1055/a-2193-2658. Online ahead of print.ABSTRACTBacterial conjunctivitis is a leading cause of infectious conjunctivitis in children and second most common cause in adults. Although often self-limiting, it can lead to complications like corneal scarring and systemic infections in high-risk groups including newborns and immunocompromised patients. Thus, prompt diagnosis and treatment are essential for these vulnerable populations. Common bacterial causes are Staphylococcus aureus and Streptococcus pneumoniae in adults and Haemophilus influenzae and Moraxella catarrhalis in childr...
