The impact of hormones in autoimmune cutaneous diseases
CONCLUSION: Pregnancy, menopause, contraceptive use, hormone replacement therapy, and breast cancer treatment drugs result in substantial shifts in hormone levels. Additionally, hormone levels are altered by aromatase inhibitors and anti-estrogen medications. These fluctuations can modulate mechanisms influencing autoimmune skin abnormalities.PMID:38317519 | DOI:10.1080/09546634.2024.2312241 (Source: Journal of Dermatological Treatment)

"Amyopathic" MDA5-positive dermatomyositis with severe lung involvement presenting with net myositic morphological features- Insights from an autopsy study
Dermatomyositis (DM) can be associated with antibodies against the helicase MDA5 (anti-melanoma differentiation-associated gene 5) [1]. The physiological role of the pattern recognition receptor MDA5 is the identification of viral RNA and subsequent induction of type I interferon signalling [2]. (Source: Neuromuscular Disorders)

Cancer risk and mortality in hospitalised patients with idiopathic inflammatory myopathies in Western Australia
CONCLUSION: Cancer risk is not increased prior to IIM, but CIR for lung cancer is increased following DM diagnosis. As cancer reduced survival only in DM and IBM patients, these data support a strategy of limited cancer screening in IIM.PMID:38302176 | DOI:10.3899/jrheum.2023-1044 (Source: J Rheumatol)

Updates on efficacy and safety janus kinase inhibitors in juvenile dermatomyositis
Expert Rev Clin Immunol. 2024 Feb 1:1-14. doi: 10.1080/1744666X.2024.2312819. Online ahead of print.ABSTRACTINTRODUCTION: Juvenile dermatomyositis (JDM) is a rare autoimmune disease most commonly with proximal weakness due to inflammation and characteristic skin rashes. Most patients have a chronic or polycyclic disease course on standard therapy so better treatments are needed. An interferon signature is well-established in key tissues of JDM. Janus kinase inhibitors (jakinibs), which can decrease IFN signaling, are therefore appealing as a targeted therapy.AREAS COVERED: Herein is a review of the growing literature on JD...

Mitochondrial transfer and implications for muscle function in idiopathic inflammatory myopathies
Clin Exp Rheumatol. 2024 Jan 25. doi: 10.55563/clinexprheumatol/5lfq5x. Online ahead of print.ABSTRACTImpairment in cellular bioenergetics as either the cause, consequence, or major contributor of tissue damage has drawn increasing scientific curiosity across aging and chronic health conditions, with mitochondrial dysfunction emerging as a central mechanism in the pathogenesis of a variety of inflammatory and degenerative disorders. Beyond bioenergetics, mitochondria play critical regulatory roles in programmed cell death of dysfunctional/defective cells as well as in metabolite synthesis and metabolic signalling. Further,...

Concurrence of dermatomyositis and psoriasis: a case report and literature review
Dermatomyositis (DM) is a type of inflammatory myopathy with unknown causes. It is characterized by distinct skin lesions, weakness in the muscles close to the body, and the potential to affect multiple organs. Additionally, it may be associated with the presence of malignancies. The development of DM is influenced by genetic susceptibility, autoimmune response, and various external factors like cancer, drugs, and infectious agents. Psoriasis is a chronic, recurring, inflammatory, and systemic condition. Scaly erythema or plaque is the typical skin manifestation. The etiology of psoriasis involves genetic, immune, environm...

Remarkable remission of symptomatic dermatomyositis after curative breast cancer surgery
AbstractDermatomyositis (DM) is an autoimmune disease that causes proximal muscle weakness in the extremities leading to severe immobility and dysphagia. Approximately 20% of patients with DM are positive for anti-TIF-1 γ antibody and frequently accompanied by malignant tumors. Although DM remission after tumor resection has been reported, the indications for surgery in patients with severe DM are unknown. Herein, we report a case of a 79-year-old Japanese woman who presented with breast cancer and anti-TIF-1γ an tibody-positive DM. She became bedridden shortly after DM onset. Although pulsed steroid therapy, intravenous...

Anti ‐MDA5 antibody‐associated clinically amyopathic dermatomyositis diagnosed after the onset of pulmonary hypertension
AbstractIdiopathic inflammatory myopathies (IIM) are a group of highly heterogeneous systemic autoimmune diseases, of which clinically amyopathic dermatomyositis (CADM) is a distinct sub-type. Pulmonary hypertension (PH) is a life-threatening medical condition that can occur as a complication of connective tissue diseases. Herein, our report first suggests that PH can develop in CADM. A 48-year-old woman came to our hospital due to a 3  months history of facial edema and shortness of breath. Relevant examinations revealed Gottron's sign, normal creatinine kinase levels, elevated levels of mean pulmonary artery pressure, ...

Man with swollen arm
Clinical Introduction A previously healthy 39-year-old male presented to the emergency department with intermittent right arm swelling for 1 month, which worsened over 4 days (figure 1). He is a right-handed chef and often raised his right arm at work. He denied fever or recent trauma. On examination, radial artery pulse was palpable. There was no cervical or axillary lymphadenopathy. His vital signs were stable, while the other physical and neurologic examinations were unremarkable. Question What is the most likely diagnosis? Compartment syndrome. Lymphedema. ‘Paget-Schroetter’ syndrome. Dermatomyositis. Answe...

The spectrum of idiopathic inflammatory myopathies in Western Australia: epidemiological characteristics and mortality over time
AbstractTo determine long term overall and subgroup specific incidence rates and associated mortality for idiopathic inflammatory myopathies (IIM) in a population wide study. We included patients hospitalised between 1980 and 2015 with incident IIM as defined by relevant diagnostic codes for dermatomyositis (DM) polymyositis (PM), inclusion body myositis (IBM), other IIM and overlap myositis (OM) in the Western Australia Health Hospital Morbidity Data Collection (n = 846). Trends over time for annual incidence rate per million population (AIR) were analysed by least square regression and Kaplan–Meier survival and mor...
