Algorithmic approach utilizing histology and immunohistochemistry for the current classification of diffuse glioma
CONCLUSION: Implementation of combined phenotypic-genotypic diagnosis with the use of histomorphology and immunohistochemical surrogates for molecular genetic alterations will yield more homogeneous and narrowly defined diagnostic entities which will provide better prognostication and definitive treatment. It also is cost-effective in a resource-limited setup.PMID:38322173 | PMC:PMC10839248 (Source: International Journal of Clinical and Experimental Pathology)

Newly Recognized Genetic Tumor Syndromes of the CNS in the 5th WHO Classification: Imaging Overview with Genetic Updates [BRAIN TUMOR IMAGING]
SUMMARY: The nervous system is commonly involved in a wide range of genetic tumor-predisposition syndromes. The classification of genetic tumor syndromes has evolved during the past years; however, it has now become clear that these syndromes can be categorized into a relatively small number of major mechanisms, which form the basis of the new 5th edition of the World Health Organization book (beta online version) on genetic tumor syndromes. For the first time, the World Health Organization has also included a separate chapter on genetic tumor syndromes in the latest edition of all the multisystem tumor series, including t...

Quantification of T2-FLAIR Mismatch in Nonenhancing Diffuse Gliomas Using Digital Subtraction [BRAIN TUMOR IMAGING]
CONCLUSIONS: T2-FLAIR digital subtraction maps may be a useful, automated tool to obtain objective segmentations of tumor subregions based on quantitative thresholds for classifying IDH-mutant astrocytomas using the percentage T2 FLAIR mismatch volume with 100% specificity and exploring T2-FLAIR mismatch/T2-FLAIR nonmismatch subregion characteristics. Conversely, the addition of T2-FLAIR subtraction maps did not enhance the sensitivity or specificity of the visual T2-FLAIR mismatch sign assessment by experienced radiologists. Treatment outcome of IDH1/2 wildtype CNS WHO grade 4 glioma histologically diagnosed as WHO grade II or III astrocytomas
ConclusionsWHO grade 2 and 3 IDH1/2 wt astrocytomas, treated according to the same treatment protocols, have a similar OS. Age, extent of resection, and strong EGFR expression were the most important treatment related prognostic factors. (Source: Journal of Neuro-Oncology)

A subregion-based RadioFusionOmics model discriminates between grade 4 astrocytoma and glioblastoma on multisequence MRI
ConclusionThe performance of subregions defined by clustering was comparable to that of subregions that were manually defined. Fusion of features from the edematous subregions of multiple MRI sequences by the RFO model resulted in differentiation between grade 4 astrocytoma and glioblastoma. Suppression of antitumor immune signatures and upregulation of VEGFA as IDH-mutant gliomas progress to higher grade
CONCLUSIONS: Here, the authors have provided a transcriptomic analysis of a progression cohort for IDH-mutant glioma. Compared with lower-grade tumors, grade 4 astrocytomas displayed alterations that may inform the timing of antiangiogenic and immune-based therapy as these tumors progress.PMID:38301244 | DOI:10.3171/2023.11.FOCUS23694 (Source: Neurosurgical Focus)
