The full spectrum tuning of fluorescent molecules via a one-pot multicomponent reaction
Tetrahedron Lett. 2023 Oct 25;130:154748. doi: 10.1016/j.tetlet.2023.154748. Epub 2023 Sep 9.ABSTRACTFluorogenic probes for imaging enable visualization and analysis of difficult-to-reach cells and organelles. However, there are limited efficient examples of tuning these fluorescent molecules to higher wavelengths. This is vital since different tissues are sensitive to varying wavelength emissions. To address this need, we report the discovery, tuning, structure-photophysical property relationships (SPPR), and time-dependent DFT (TD-DFT) computations of 400-700+ nm fluorescent pyrido[2',1':2,3]imidazo[4,5-c]isoquinolines a...

The Effect of Sodium-Glucose Cotransporter 2 Inhibitors on Cardiovascular Outcomes in Patients with Cancer: A Systematic Review and Meta-Analysis
Clinical practice guidelines recommend the use of sodium-glucose cotransporter-2 inhibitors (SGLT-2i) to improve outcomes in patients with heart failure (HF) (1-2). Patients receiving certain cancer therapies are at increased risk of developing HF, which is associated with worse health-related quality-of-life metrics and clinical outcomes (3). In patients receiving anthracyclines, the incidence of developing clinical HF can range from anywhere between 5% to 48% depending on the cumulative anthracycline dose (4). Dual-release hydrocortisone improves body composition and the glucometabolic profile in patients with secondary adrenal insufficiency
ConclusionThis study suggests that switching to DR-HC leads to lower late-evening cortisol/cortisone exposure and a more favourable metabolic profile and body composition. In contrast, decreased osteocalcin with increasing sclerostin might indicate a negative impact on bones.Clinical trial registrationEudraCT201400203932 (Source: Endocrine)

Course of COVID-19 infection in patients with congenital adrenal hyperplasia
ConclusionPatients with CAH with close follow-up do not appear to be at increased risk of acquiring COVID-19 or to have a more severe course of COVID-19 compared to the general USA population. Obesity may increase risk of acquiring COVID-19 in patients with CAH, and overall infection risk may be lower in those receiving short-acting and circadian glucocorticoid replacement therapy. Established age-appropriate guidelines for stress dosing during infectious illnesses should be used for patients with CAH and COVID-19. COVID-19 specific guidelines are not indicated. Predictors of bone mineral density in patients receiving glucocorticoid replacement for Addison ’s disease
ConclusionStandard HC substitution does not considerably accelerate BMD loss in AD patients and their serum BTMs: CTX, osteocalcin, sclerostin, DKK-1, and ALP activity remain within the reference ranges. Independent predictors of low lumbar spine BMD, especially ALP activity, serum CTX and sclerostin, might be monitored during GC substitution. (Source: Endocrine)

The effect of age on baseline neurocognitive performance
CONCLUSIONS: Age was shown to be an independent modifier of impulse control, verbal memory, visual motor, and reaction time scores but not visual memory or symptom scores. This underscores the previous literature showing developmental differences as age increases among the adolescent athlete population. This data also indicates the need for repeat neurocognitive baseline testing every other year as baseline scoring is likely to change as athletes become older.PMID:38328998 | DOI:10.1080/02699052.2024.2307968 (Source: Brain Injury)

Global pediatric anesthesia-Anglophone West Africa perspective
This article gives the pediatric anesthesia perspective from Cameroon, Nigeria, Ghana, Liberia, and Gambia, five out of six countries in Anglophone West Africa. Over 40% of the population of most of these countries are younger than 14 years and there is an increasing need for paediatric anesthesia services.FINDINGS: Workforce density ranges from 0.08 to 0.58 physician anesthesia providers per 100,000 population. There are only 13 trained pediatric anesthetists; ratios range from 0 to 0.4 per 100,000 children, thus pediatric anesthesia services are provided by various cadres of physician and non-physician anesthesia provide...

Glucocorticoid-induced adrenal insufficiency after therapy with intravenous methylprednisolone in patients with moderate-to-severe and active Graves ’ orbitopathy: assessment with a low-dose corticotropin test
ConclusionFor the first time, our research shows that administering IVMP in 12 weekly pulses can result in GC-induced AI. We suggest that patients should undergo careful evaluation for GC-induced AI, including LDT, after therapy with IVMP according to EUGOGO guidelines. Screening for altered adrenal reserve could prevent life-threatening complications, particularly during acute stress situations. (Source: Journal of Endocrinological Investigation)
