The 1 μg Synacthen stimulation test in the diagnosis of secondary adrenal insufficiency in patients with Rathke's cleft cyst and empty sella syndrome
CONCLUSIONS: The incidence of SAI in the general patient population with empty sella syndrome and Rathke's cleft cysts is low. The 1 μg Synacthen test seems to be a valuable tool in the diagnosis of SAI among patients with RCC and PESS. Further studies are necessary to determine the sensitivity and specificity of the 1 μg Synacthen test with the standardization of test protocol and considering the cortisol level at the 20-minute timepoint. PESS patients report dizziness and apathy more frequently than RCC patients, which does not result from the disturbance of the hypothalamic-pituitary-adrenal axis, but probably from th...

Evaluation of psoriasis patients with long-term topical corticosteroids for their risk of developing adrenal insufficiency, Cushing's syndrome and osteoporosis
CONCLUSIONS: Our study is the first study that we know of that investigated these three side effects. We have shown that the development of CS, adrenal insufficiency and osteoporosis in patients who use topical steroids for a long time depends on the weekly TCS dosage and the risk increases when it exceeds the threshold of 50 grams per week. therefore, our recommendation would be to avoid long-term use of superpotent steroids and to choose from the medium-potent group if it is to be used.PMID:38156462 | DOI:10.1080/09546634.2023.2298880 (Source: Journal of Dermatological Treatment)

Comment on: Patient and operative factors influence delayed discharge following bariatric surgery in an enhanced recovery setting
In the late 1990s, the Danish surgeon Kehlet published a seminal paper describing an approach to reduce the intra- and postoperative surgical stress response with the goal to improve recovery and reduce postoperative morbidity (1). The central tenant was a multimodal approach, wherein many small changes that on their own would not cause a clinical impact, when combined could cause drastic improvement in patient recovery. Key elements included preoperative treatment of alcohol misuse and malnutrition, minimally invasive surgery, multimodal pain control avoiding narcotics, and early mobilization. Idiopathic isolated adrenocorticotropic hormone deficiency: a systematic review of a heterogeneous and underreported disease
We report the largest case series of acquired idiopathic IAD to date. Our systematic review highlights the lack of a clear definition and diagnostic work-up. Based on the findi ngs in this review a proposition is made for a flowchart to diagnose acquired idiopathic IAD. (Source: Pituitary)

Comparison of clinical characteristics of a pediatric cohort with combined pituitary hormone deficiency caused by mutation of the PROP1 gene or of other origins
CONCLUSION: Patients with the PROP1 mutations present a clinical picture significantly different from that of other forms of congenital hypopituitarism. Certain specific clinical results may lead to the successful identification of children requiring diagnostics for the PROP1 gene mutation.PMID:38147295 | DOI:10.1007/s42000-023-00510-1 (Source: Hormones)

Perioperative growth hormone levels as an early predictor of new-onset secondary adrenal insufficiency following transsphenoidal pituitary tumor resection
CONCLUSION: Early morning GH level on POD1 can predict NOSAI with fair accuracy when perioperative stress dose glucocorticoid is administered. Prediction accuracy can be improved by considering CSF leakage, DI, and perioperative changes in GH and PRL in the final regression model.PMID:38148260 | DOI:10.1016/j.asjsur.2023.12.120 (Source: Asian Journal of Surgery)
