Determining the Pharmacokinetics of Azithromycin in Pregnant Subjects Undergoing Cesarean Delivery After Failed Labor
This opportunistic population pharmacokinetic (popPK) study evaluated azithromycin concentrations in pregnant patients at the time of cesarean section following labor, to examine azithromycin pharmacokinetcs (PK) and determine if azithromycin concentrations differ by body mass index (BMI) among birthing people. 120 patients were recruited. After a single dose of 500 mg azithromycin by intravenous infusion at the time of cesarean, specimens of maternal plasma, adipose tissue, and myometrial tissue were collected at defined time points. Characterization of the resistome and predominant genetic lineages of Gram-positive bacteria causing keratitis
Antimicrob Agents Chemother. 2024 Jan 30:e0124723. doi: 10.1128/aac.01247-23. Online ahead of print.ABSTRACTBacterial keratitis is a vision-threatening infection mainly caused by Gram-positive bacteria (GPB). Antimicrobial therapy is commonly empirical using broad-spectrum agents with efficacy increasingly compromised by the emergence of antimicrobial resistance. We used a combination of phenotypic tests and genome sequencing to identify the predominant lineages of GPB causing keratitis and to characterize their antimicrobial resistance patterns. A total of 161 isolates, including Staphylococcus aureus (n = 86), coagulase-...

155. Acceptance and Implementation of 2021 CDC Guidelines for Treatment of Uncomplicated Urogenital Chlamydial Infections by Adolescent Medicine Providers
In 2021, the CDC recommended a major change in the recommendations for treatment of urogenital chlamydia infections in adolescents and adults. These changes were based on studies that found that single-dose azithromycin (AZI), was not as effective as 7-days doxycycline (DOX) for treatment of rectal infection in men who have sex with men. There are questions if these guidelines are generalizable to adolescents. There are concerns about tolerance and compliance; DOX is associated with higher rate of side effects and there may be less compliance with a 7-day, twice/day regimen. Real-world prescription of anti-COVID-19 drugs in hospitalized patients with COVID-19 in Japan
Conclusions and relevanceThis study revealed the real-world situation of anti-COVID-19 drug prescriptions in hospitalized COVID-19 patients in Japan. A prescribed drug would depend on the latest scientific evidence, such as efficacy, safety, and approval status, at the time of prescription. Understanding the prescription of anti-COVID-19 drugs will be important for providing the most up-to-date treatments to patients and evaluating the benefit and/or risk of anti-COVID-19 drugs based on the utilization of an electronic medical record database. Obesity and its Relationship with Covid-19: A Review of the Main Pharmaceutical Aspects
Curr Pharm Biotechnol. 2024 Jan 22. doi: 10.2174/0113892010264503231108070917. Online ahead of print.ABSTRACTImportant physiological changes are observed in patients with obesity, such as intestinal permeability, gastric emptying, cardiac output, and hepatic and renal function. These differences can determine variations in the pharmacokinetics of different drugs and can generate different concentrations at the site of action, which can lead to sub therapeutic or toxic concentrations. Understanding the physiological and immunological processes that lead to the clinical manifestations of COVID-19 is essential to correlate ob...

Clofazimine as a substitute for rifampicin improves efficacy of < em > Mycobacterium avium < /em > pulmonary disease treatment in the hollow-fiber model
Antimicrob Agents Chemother. 2024 Jan 23:e0115723. doi: 10.1128/aac.01157-23. Online ahead of print.ABSTRACTMycobacterium avium complex pulmonary disease is treated with an azithromycin, ethambutol, and rifampicin regimen, with limited efficacy. The role of rifampicin is controversial due to inactivity, adverse effects, and drug interactions. Here, we evaluated the efficacy of clofazimine as a substitute for rifampicin in an intracellular hollow-fiber infection model. THP-1 cells, which are monocytes isolated from peripheral blood from an acute monocytic leukemia patient, were infected with M. avium ATCC 700898 and exposed...

Spontaneous splenic rupture associated with scrub typhus: a case report
ConclusionsClinicians should consider the possibility of splenic rupture in patients with scrub typhus who display sudden-onset abdominal pain and unstable vital signs. In addition, splenic capsular rupture and extra-capsular hemorrhage are thought to be caused by splenomegaly and capsular distention resulting from red blood cell congestion in the red pulp destroying the splenic sinus.Graphical abstract (Source: Infectious Diseases of Poverty)

Azithromycin therapy in infants hospitalized for respiratory syncytial virus bronchiolitis: Airway matrix metalloproteinase-9 levels and subsequent recurrent wheeze
CONCLUSION: Azithromycin therapy in children hospitalized with RSV bronchiolitis had a short-term anti-inflammatory effect in reducing upper airway MMP-9 levels. However, the reduction in MMP-9 levels did not relate to subsequent RW post-RSV.TRIAL REGISTRATION: This study is a secondary analysis of the Azithromycin to Prevent Wheezing following severe RSV bronchiolitis-II clinical trial registered at Clinicaltrials.gov (NCT02911935).PMID:38237675 | DOI:10.1016/j.anai.2024.01.001 (Source: Annals of Allergy, Asthma and Immunology)

Cardiovascular Safety of Azithromycin in Patients Hospitalized With COVID-19: A Prespecified Pooled Analysis of the COALITION I and COALITION II Randomized Clinical Trials
The cardiovascular safety from azithromycin in the treatment of several infectious diseases has been challenged. In this prespecified pooled analysis of 2 multicenter randomized clinical trials, we aimed to assess whether the use of azithromycin might lead to corrected QT (QTc) interval prolongation or clinically relevant ventricular arrhythmias. In the COALITION COVID Brazil I trial, 667 patients admitted with moderate COVID-19 were randomly allocated to hydroxychloroquine, hydroxychloroquine plus azithromycin, or standard of care. Periodontal disease prevalence and oral hygiene status of adults with cystic fibrosis: A case-control study
CONCLUSIONS: In this study, adults with CF had poor oral hygiene practices, with high levels of plaque and calculus. Despite this finding, adults with CF had lower levels of clinical gingivitis and periodontitis than seen in a non-CF control group. Further study is required to examine the causes of this phenomenon.PMID:38233039 | DOI:10.1111/jcpe.13944 (Source: Journal of Clinical Periodontology)

Antimicrobial susceptibility profile of Neisseria gonorrhoeae from patients attending a medical laboratory, Institut Pasteur de Madagascar between 2014 and 2020: phenotypical and genomic characterisation in a subset of Neisseria gonorrhoeae isolates
Conclusions Our report provides a detailed dataset obtained through phenotypical and genotypical methods which will serve as a baseline for future surveillance of NG. We could not confirm the occurrence of ceftriaxone-resistant isolates. Our results highlight the importance of implementing quality-assured gonococcal antimicrobial resistance surveillance in Madagascar. (Source: Sexually Transmitted Infections)
