Sent a threatening letter from a company for too many 99214
Recently got a letter from a third party company (don't recall the name), comparing my billing codes with others of the same specialty. While it was worded as nicely as possible it indicated that i was billing more 99214 and less 99213 than the average joe psychiatrist. It also suggested I could be audited if this trend continues, it did not specify any insurance companies. I work in a rural hospital and see the sickest of the sick kids, comorbidity is the rule. I'm not concerned my... Read more (Source: Student Doctor Network)

Personal Experience Transferring Programs
Hi All, Just wanted to share with you my experience in transferring residencies within the same specialty. Just some back ground, I am an IMG that took 3 attempts to match into family medicine. I had low board scores and a repeat on step 2 CS. On my successful match cycle, I applied to 150 FM programs and had 11 interviews and matched at my number 7 on the rank list. I matched into a rural FM program which was far away from my wife who was in another state. The program was malignant and... Read more (Source: Student Doctor Network)

M4 considering EM - job avaliability and lifestyle in rural or suburban areas
Hello all, I am an M4 who is currently in the bargaining stage of grief regarding my desire to do EM. My story is the usual one; scribed as a pre-med, loved my third year EM rotations, am a very good personality fit, enjoy acuity and dealing with the undifferentiated patient, etc. I also enjoy doing interventions and seeing them have an effect fairly quickly and initial stabilization of the very sick. I am also interested in crit care, and I had some notion of doing EM and go into a... Read more (Source: Student Doctor Network)

What is the NNT for an EM physician to have a better outcome...
Comparing physicians and PAs as solo providers in a rural... : Journal of the American Academy of PAs f the host institution were analyzed. Although minor variances in metrics were appreciated, transfers, 72-hour returns, and death rates were all similar among provider types. Results from this work do not demonstrate a clinically meaningful difference among ED metrics between physicians and PAs... journals.lww.com Just came off shift where I worked alongside an EM doc a few years out of 3 year residency. Job as General surgeon in critical access hospital
I am entering my fifth year of general surgery residency and have started looking for jobs for next year. I have always planned to end up in either a rural area or smaller city. A lot of the more heavily advertised jobs in these areas are for small critical access hospitals in rural areas. They typically have more frequent call and relatively high compensation. Having never rotated at a place like that (our “rural” rotation is not all that rural), I would really like to hear what it is like... Read more (Source: Student Doctor Network)

M1 interested in FM - what's the catch?
Hi everyone! I'm an incoming M1 at a T5 school, and am very interested in FM (rural in particular). I've shadowed a lot of FM's (and other specialties), and had really great experiences growing up with my FM docs. Would be really interested in sports medicine but would be happy doing FM in general too. The thing is, it seems like literally no other peers seem to think FM is a good idea, especially at top schools. Is there something I'm hugely missing out/being naive about on why FM is... Where to practice on secondary apps?
Hello. I am applying to a mix of DO and MD in Texas. A lot of secondaries are asking where I want to practice. I am interested in family medicine/primary care, but I want to personally live in a sur-urban area. I come from a city where I volunteered and saw plenty of lower income and underserved people, and Dallas has its own health issues too. Is it bad to say I want to live and practice here? I wouldn't mind going into rural medicine and I even scribed there as part of my job, but I... Read more (Source: Student Doctor Network)

I know of a job opening in Kansas
It's at an independent pharmacy in a small city in a rural area. I can provide further details via PM. Pay is OK, and the benefits aren't as good as in a chain, but there aren't any metrics or any of that junk. (Source: Student Doctor Network)

Is it bad that I "lied" to get in??
So I got an acceptance from a rural D.O. school in the south which I am going to be attending. In my interview, they asked where I was planning to serve as a physician. To make them like me, I said I was going to serve in the same area as their school (in the south). But I really don't want to. I plan on doing my residency on the west coast area, or somewhere close there. I kinda panicked during the interview and wanted to tell them what they wanted to hear so they'd be more likely to accept... Read more (Source: Student Doctor Network)

Is My Practice Building Too Slowly?
Hi All, I recently started a new job, 75% outpatient and 25% inpatient. Hospital-based employed, mostly private insurance. It’s in an area with poor psychiatry access and a relatively large catchment area (rural). I’ve been on the job six months and seem to be building slowly, and I’m hoping to have a reality check from those who have more experience starting up a new panel of patients. I’ve been working six months, and my days are mostly 10-12 patients (3 new, the rest follow ups), with... Read more (Source: Student Doctor Network)

Alternatives to applying EM - M3 - > EM/critical care fellowship vs IM/PCCM?
Hello all, This is an extremely common story, but I am a M3 going into M4 who was, until the ACEP jobs report, very much set on EM. I imagine it would still be possible to get a job, but the jobs would likely be in less desirable/rural locations. As l would like to work and live in a more metropolitan area, I am thinking of alternatives to EM. I am interested in critical care as one of those alternatives. With regards to things I like about the ED, I think I would miss the workup of the... Read more (Source: Student Doctor Network)
