Actiq for chronic pain
So I inherited a patient on actiq and fentanyl patch for>6 years. She states its the only thing that works. I told her Actiq is usually only for cancer pain and she showed me the REMS registration that says it can be used for non cancer pain. I am still planning on telling her to find another provider because I don't feel comfortable having patients on that. Is that unreasonable of me? Losing over 20 to 30k a day in my stocks.
I made a huge mistake last Wednesday. I had made a ton of money when the sp500 was at a new high. I could have paid off all my debt and moved on in life. However, I got greedy and stayed in stocks. Maternity leave in fellowship
Hi all! Have a question regarding fellowship but since there is not a general page for fellows, I am posting here. I had my first baby my first year of fellowship. I used all of my vacation time, and another 4 weeks no pay which I have to make up at the end of my fellowship. I am now pregnant again, and still bitter about my short, unpaid maternity leave. M-F Hospitalist
I just wanted some advice. I have a unique opportunity to take a job in a nearby southern state, working Monday-Friday as full time hospitalist (Average 10pts/day). I will also be covering the medical aspect of a Geri psych unit that averages about 5pts. They only have to be seen twice per week and prn. Only downside is I am the sole hospitalist 24hrs a day Monday-Friday so I take all night call, but only have to attend emergencies at night, no admissions. ED docs cover hospital on weekends... RO APM may be implemented this year

Why are medical professions so hesitant to protect their income?
I've been a pre-law student with a business degree working for a law firm until recently, and I've noticed a huge difference in mindset/willingness to acknowledge that this is a job from the medical side of things. If you talk to a law or business student, almost always, they'll unashamedly say money or owning their own business is why they're going into that field. Was working on our consult service and had an interesting case of substance-induced vs. first-break schizophrenia as the primary diagnosis, but during one of the interviews the patient happened to mention having the belief that his penis had been gradually retracting into his body for the past year. Further questioning seemed to be pretty spot-on with Koro as a diagnosis, but my attendings were mixed on whether this was an appropriate diagnosis because the patient was white and the culture... I've been looking for doctors to shadow in my area. I found emails for a couple of DOs and was going to ask if I could shadow but I'm wondering, is it a bad idea to shadow DO if I plan to apply MD? I figured it wouldn't matter because a doctor is a doctor. But it looks like if you're applying DO, you're pretty much are required to shadow a DO. Is it like that with MD as well? 3rd year DO student needing some advice with where to do my auditions.
Context: 3rd DO at midwest school. Average scores (228/560). No red flags, solid ECs with EM leadership and some research. Should I take pathophysiology or developmental biology?
If you've taken these classes, which did you prefer and why? I'm currently taking human physio, so taking pathophysio next quarter could be a nice segue into what goes wrong with the body, but I also enjoyed the reproductive system/fetal development, so developmental biology seems interesting as well.
