Management of haematogenous bone and joint infection in developing countries
Haematogenous pyogenic bone and joint infection is rarely seen in developed countries, but in the developing world it is still a major and common problem. New forms of acute fulminating osteomyelitis are likely to appear due to immunosuppression in the wake of the worldwide spread of HIV infection, AIDS, and the increased use of steroids, antibiotics, and immune activators. Osteomyelitis due to candida and cryptococcus infection in certain regions, are occasionally observed, whilst osteomyelitis that is resistant to treatment is associated with diabetes mellitus. Is There a Learning Curve for the 5-Year Implant Survival Rate of Total Hip Arthroplasty Using the Direct Anterior Approach with a Traction Table? - A Prospective Cohort Study
The direct anterior approach (DAA) for total hip arthroplasty (THA) is attracting attention as a minimally invasive surgery, but the learning curve to master this approach is a concern, and its effect on long-term results is unknown. The purpose of this prospective cohort study was to clarify how the learning curve affects the five-year results of DAA THA with a traction table. (Source: The Journal of Arthroplasty)

Over the Past Decade, Preoperative Anemia Has Become a Greater Predictor of Transfusions after Total Knee Arthroplasty
With advancements in perioperative blood management and the use of tranexamic acid, the rate of transfusions after total knee arthroplasty (TKA) has substantially decreased. As these principles are refined, other modifiable risk factors, such as preoperative anemia, may play an increasingly important role in transfusion risk for patients undergoing TKA. Minimum 15-Year Survival Of A Biconvex Inlay Patellar Component In Primary Total Knee Arthroplasty: An Analysis Of 2,530 TKA From A Single Institution
Routine patellar resurfacing during primary total knee arthroplasty (TKA) remains controversial. To our knowledge, there are no studies reporting the long-term performance of a cemented biconvex all-polyethylene inlay component implanted at the time of primary TKA. The purpose of this study was to examine the 15-year survivorship and long-term clinical outcomes of this biconvex inlay patella used at our institution. (Source: The Journal of Arthroplasty)

Questionable firms tempt young doctors with 'easy' publications
Last year, physician Rupak Desai co-authored more than three dozen conference abstracts in Circulation , the American Heart Association’s (AHA’s) flagship journal. The works marked a modest fraction of his publications in 2023 , which totaled 162. But Desai, scholarly productivity notwithstanding, is not employed by a hospital, university, nor any other type of scientific institution. Based in Atlanta, Desai runs a business that offers junior doctors from around the world a chance to beef up their CVs before applying for coveted residency or fellowship positions at hospitals or physi...

Development and validation of a machine learning model to improve precision prediction for irrational prescriptions in orthopedic perioperative patients
CONCLUSION: The RF model showcased significantly high level of proficiency in predicting irrational prescriptions among orthopedic perioperative patients, outperforming other models by a considerable margin. It effectively enhanced the efficiency of pharmacist interventions, displaying outstanding performance in assisting pharmacists to intervene with irrational prescriptions.PMID:38698685 | DOI:10.1080/14740338.2024.2348569 (Source: Expert Opinion on Drug Safety)

Technology Behind Cell Therapy Augmentation of Fracture Healing: Concentrated Bone Marrow Aspirate
J Am Acad Orthop Surg. 2024 May 15;32(10):e476-e481. doi: 10.5435/JAAOS-D-24-00020.ABSTRACTWith an aging population, and an anticipated increase in overall fracture incidence, a sound understanding of bone healing and how technology can optimize this process is crucial. Concentrated bone marrow aspirate (cBMA) is a technology that capitalizes on skeletal stem and progenitor cells (SSPCs) to enhance the regenerative capacity of bone. This overview highlights the science behind cBMA, discusses the role of SSPCs in bone homeostasis and fracture repair, and briefly details the clinical evidence supporting the use of cBMA in fr...

An in silico approach to elucidate the pathways leading to primary osteoporosis: age-related vs. postmenopausal
Biomech Model Mechanobiol. 2024 May 3. doi: 10.1007/s10237-024-01846-2. Online ahead of print.ABSTRACTNumerical models of bone remodelling have traditionally been used to perform in silico tests of bone loss in postmenopausal women and also to simulate the response to different drug treatments. These models simulate the menopausal oestrogen decline by altering certain signalling pathways. However, they do not consider the simultaneous effect that ageing can have on cell function and bone remodelling, and thus on bone loss. Considering ageing and oestrogen decline together is important for designing osteoporosis treatments ...

Computer-simulated TOUCH prosthesis cup malposition and solutions
CONCLUSION: Our computer-based measurements suggested options for intraoperative cup revision depending on cup shape and size and on type of misalignment with resulting bone defects. These suggestions, however, need to be confirmed in anatomic specimens before introducing them into clinical practice.PMID:38701947 | DOI:10.1016/j.hansur.2024.101712 (Source: Hand Surgery)

Incidence and management of secondary deformities after megaendoprosthetic proximal femur replacement in skeletally immature bone sarcoma patients
CONCLUSIONS: Patients aged < 10 years were prone to develop secondary hip dysplasia and genu valgum following proximal femur replacement in this study. Management of secondary deformities should depend on remaining skeletal growth. Stress shielding was observed in almost all skeletally immature patients.PMID:38700674 | DOI:10.1007/s00402-024-05334-1 (Source: Hand Surgery)
