Ongoing measles outbreak, the Federation of Bosnia and Herzegovina, December 2023 to February 2024
We report on an ongoing measles outbreak in the Federation of Bosnia and Herzegovina with 141 cases notified between week 52 2023 and week 6 2024. Among those with known vaccination status, 97% were unvaccinated and the most affected group is children under the age of 5 years (n = 87) who were not vaccinated during the pandemic years. Sixty-eight cases were hospitalised, the most common complications were measles-related pneumonia and diarrhoea. The sequenced measles viruses from four cases belonged to genotype D8. Defining a metagenomic threshold for detecting low abundances of Providencia alcalifaciens in canine faecal samples
DiscussionUsing our detection limit, the occurrence of P. alcalifaciens in faecal samples from healthy dogs was low. Given the low sensitivity of the metagenomic assay, these results do not rule out a significantly higher occurrence of this bacterium at a lower abundance. Viruses, bacteria and protozoa are responsible for most infections, which are most commonly transmitted by the faecal –oral route through water, food and person-to-person transmission. The clinical presentation conforms to four patterns: acute watery diarrhoea; bloody diarrhoea (dysentery); persistent non-bloody diarrhoea, which can include steatorrhoea; and persistent bloody diarrhoea. Microscopic colitis
describes a condition of chronic watery diarrhoea in the absence of macroscopic evidence of colonic inflammation or with only subtle endoscopic changes, but with characteristic appearances on histopathological examination of colonic biopsies. It comprises mainly two separate but related conditions of lymphocytic and collagenous colitis, with a third variant called incomplete microscopic colitis that does not fulfil the classical histological criteria. A subtotal distal gastrectomy is preferred when proper oncologic margins can be obtained. Additionally, no one form of reconstruction following gastrectomy is statistically preferred over another.
